As a unit of a multicenter clinical trial coordi - nated by the National Reserch Council we studied two groups of patients,in the last three years.In the first group of undred patients affected by ma- lignat melanoma with stage I-IV we were able to de tect the 75% of well documented lesions employing a 99mTC labelled F(ab')2 fragments of MoAb raised a- gainst melanoma (HMW MAA)225.28S(TECNEMAB 1-SORIN BIOMEDICA).In addition a certain number of unknown metastases were also detected and confirmed after- wards. The second group consisted of two-undred pa tients with gastro-intestinal,breast and lung carci^ noma.In these cases we used a 111 In and or 131 I labelled F(ab')i) fragments of a MoAb Raised against CEA(clone F023C5-S'?RIN-Biomedica) with best results in patients bearing colon retto carcinoma and lung cancer.More in detail we obtained the 8<~-% and the 91% of positive scans respectively.In order to im- prove the sensitivity and the tumour/BK ratio some patients with G.I. cancer were also injected intra peritoneally.The results are encouraging,specially in the field of liver and abdomen recurrences. 18 CAN THE TUMOR SCINTIGBAPHY WITH POSITIVE CONTRAST USING M-LIGAND COMPLEXES BE IMPROVED? 1 2 11 K.Schomacker , W.G.Franks , G.J.Beyer , D.Mocker , R.MUnze1 Central Institute of Nuclear Research Dresden, GDR p Medical Academy "Carl Gustav Carus" Dresden, GDR The best known M-ligand-complex for positive tumor- scintigraphy is Ga-citrate up to now. We determined that the Ga-tumor-scintigraphy can be improved by application of inactive iron due to an enhancement of the tumor/background-ratio by the factor 2-3. Furthermore we proved that the use of Yb or ' Tin instead of Ga leads to an enhancement of the tumor/ background ratios by the factor 10 - 15 to comparable time points. Investigations of the biological consequences after the variation of the physicochemical properties of tumor-seeking M-ligand-complexes demonstrated that the tumor seems to accumulate the metallic radionuc- lides preferable in protein-bound form. The kind of the transport protein does not play a deciding role. 19 SCINTIGRAPHIC RECOGNITION OF SMALL CUTANEOUS LYMPHATIC VESSELS K.Buchali, H.J.Blesin, H.Schiirer, H. Winter Klinik fUr Nuklearmedizin, Bereich Medizin (Charite), Humboldt-Universita't zu Berlin, DDR In high risk malignant melanoma an exact planning of operation procedure requires recognition of small lymphatic vessels from the tumour to the first lymph node station. This aim was achieved by intracutaneous injection round the tumour of a 99m-Tc-labelled antimony sulphid microcolloid. Lymphatic vessels can be outlined ivithin 5 to 30 min. Results in 53 patients reveal unequivocal detection oT lynphatic vessels especially in tumours of the trunk skin. Tumours near to lymph nodes (less than 5 cm) lead to in- sufficient evaluation of scintigrams. In the lower extre- mity, lymphatics from foot and calf drain into vessels, accompanying saphenous and femoral veins. Subcutaneous in- jection of the tracer gives no reasonable results. 20 METHOD FOR CHECKUP EVALUATION IN POSTINTRACORONARY THROMBOLYSIS THERARPY OF INFARCT PATIENTS A. e) Helou, A. Welkl" 6900 Heidelberg, Boxbergrlng 15, FRS :: Medical Clinic, University of Erlangen-Nureroberg, FRG In the search for a nonburdenscme method of checking post- Intracoronary thrcmbolysis therapy of myocardlal Infarction, a comparative study was carried out by point evaluation of levography, erosive anglography, thai 11un sclntlgraphy and radlonucllde ventrlculography after subdivision of the myocardlun Into 7 segments. 201 0 points = normoklnesla and nocmal Tl fixation 1 point = moderately reduced Tl fixation and hypoklnesla 2Q. 2 points = greatly reduced Tl flcatlon and aklnesta 3 points = absence of Tl fixation and dysklnesla. It was possible to reopen 100% occluded vessels In 59% of the cases. Aneurysms developed In 57?! of patients with 100§ occluded vessels and in whom Intracoronary strepto- klnase therapy had been unsuccessful. In patients with successful Intracoronary streptokinase therapy, the rate of aneurysm formation was only 25fc. JhPi'e was a close cor- relation (p 0.001) between total Tl points and elevation of the ST as well as between radlonucllde ventrlculography and levography. A correlation was found between tha ejection fraction in both techniques as well as between coronary angiography and elevation of the ST wave.jlhere was an appreciable difference between the total Tl or radlo- nucllde ventrlculography points and LCGR-angIo-levography points In five patients. This Is attributable to the time- span between the two tests (more than two months). 21 THALLIUM 201 MUSCLE SCINTIGRAPHY IN THE MANAGEMENT OF ARTERIAL OCCLUSIVE DISEASE MEDICAL TREATMENT P.Thouvenot, C.Chevreaud, M.H.Laurens, D.Burdin, J.Robert Department of Nuclear Medicine, 18,rue Lionnois 54000 NANCY France Skull (5), thighs (T) and calves (C) scintigraphies were performed using 2 mCi Thallium 201 IV in thirty patients :-.u('fVriiij». from st.agn 11,111 and IV arterial occlusive disease (Pniii ,ii rv1:-. rlasRification). Two indexes were calculated : H=T/C - C/S (T/C and C/S were the absolute values of radioartivity ratios measured before medical treatment), and R = A(C/S) - &(T/C)(where £>. expressed the relative variation of the ratios after treatment, compared to initial values). Angiography was performed on all patients prior to treatment and the results were quantified as 1 in the case of obliteration and 0.5 in case of stenosis of major arterial axes. Clinical improvement was evaluated by a 100 percent increase in walking distance for stage II disease, abolition of pain at rest for stage III disease, and reepithe liezation of trophic lesions for stage IV disease- Results : The correlation observed between the values of P and angio^raphy results was p^0.025 (Student t-test). The mean of P (measured before treatment)was 1.75 for the improved p.-itiont.s and 0.56 for the others (p<0.005). After treatment, H was +58 percent for improved patients versus -13 in which the treatment was inefficient (p^0.025). Conclusion : P expresses the microcirculatory effects of arteri.il lesions. It is a prognostic index : satisfactory proximal perfusion (high T/C), associated with lesions which are essentially peripheral (low C/S) predicting a positive effect, of medical treatment. R measures the increase in peripheral circulation (increase in C/S) at the expense of the proximal region (decrease in T/C), and quantifies the results of treatment. 22 A.BOSSUYT, M.VERHAS, G.uEMEURISSE, 3.MERTENS, M.VANDERSHUEREN, 3.PATERNOT, A.CAPON and M.3QNCKHEER CORTICAL REPERCUSSION Uř DEEP INTARCTS IN STROKE PATIENTS.A 123 IOOOAMPHETAMINE SPÉCT AND ÍJJ XENON INHALATION STOOT Akademische Ziekenhui» l/UB and HOpítal universitaire Brugmann ULB (Belgium) Thirty stroke patients with deep cerebral infarcts were studied.A 123 iodoamphetamine (IMP) single photon emission tomography (SPECT) and a 133 Xenon (Xe) inhalation cerebral blood flow (rCBF) measurements were carried out within a month after the onset of a stroke.The deep cerebral lesion was well documented on CT scan. The IMP SPECT was started JO' after injection IV of 4 mCi, a time delay that is representative of regional cerebral blood flow.The data were expressed as interhemisphenc ratios. The results indicated a good correlation between 123 IMP and 1J3 Xc.In 14* of cases 123 IMP and/or Hi Xe revealed a cortical tCBt lesion not seen on CT.IMP scans may be userul for the clinician in the demonstration of a functional impairement. un the other hand a cerebellar diaachisis was noted in 11% or our cases.Thia phenomenon is discussed. 23 RETROSPECTIVE ANALYSIS OF THALLIUM-201 STRESS SCINTIGRAMS ON THE BASIS OF 136 CORONAROGRAPHICALLY PROVEN CASES 3. Csernay Department of Nuclear Medicine, 2nd Department of Medicine, Department of Heart Surgery University Medical School of Szeged, Szeged, Hungary The clinical effectiveness of 201-T1 stress scintigraphy for the detection of transitory ischaemic perfusion defects was examined on the basis of data from 136 patients under- going coronary angiography. The sensitivity in the investi- gated group was found to be 80 per cent, and the specifi- city 66 per cent. The relatively low specificity value may be partially due to the low number of true negative patients. However, non-coronary heart disease, an anomalous coronary anatomy and coronary spasms during the exercise test may also lower the specificity. Te results lead us to suggest 201-T1 stress scintigraphy mainly for the investigation of patients with an average probability of having coronary heart disease; patients with known coronary narrowings, in order to detect haemodynamic consequences; and patients following aorto-coronary by-pass surgery, for the examina- tion of graft patency. 24 LEFT VENTRICULAR FUNCTION AND MYOCARDIAL PERFUSIO.!
