Thank you for your request for information received on 17 September 2015 in relation to the and Darlington NHS Foundation Trust (Trust). We are dealing with your request under the provisions of the Freedom of Information Act 2000.

You requested information regarding the adult community nursing service. Specifically you asked for:

The following information with regard to your adult community nursing service (Community Matrons, District Nurses and support staff):

1. What is the weighted population that you cover?

Answer: The whole of County Durham and Darlington for which the total actual population split is as follows:

Co. Durham: 513,000

Darlington: 106,000

2. How many referrals do you receive per annum?

Answer: 66,800.

3. How many community nurses and support staff do you employ, split by band?

Answer:

5. Please provide your service structure chart?

Answer: See attached.

6. What is the size of the total community nursing caseload?

Answer: Current Patients on overall Caseload: 46,494.

7. What is the average caseload per wte?

Answer: Caseloads are managed by Band 6 District Nurse’s with a team of Community Staff Nurses and Healthcare Assistants. On average the numbers of patients on the caseload varies from 90 – 434 in relation to GP alignment. On average the caseload per wte Band 6 is 228.

Community Matrons vary especially since the introduction of predominate Care home work for Darlington and the North, therefore, caseload depend on the number of patients residing in care home between on average 30 – 90.

Currently on average Community Matrons in the North have between 2 – 10 house- bound community patients. Darlington Community Matrons have between 2 - 40 housebound community patients

8. On average, how many patients are the community nurses expected to see per day?

Answer: The number of patients a community nurses is expected to see per day varies depending on whether the patients resides in care homes/shelter housing etc. and according to the patients assessed need e.g. length of time to obtain a venous blood sample compared to end of life care or bilateral compression bandaging. Therefore, on average the expected visits can range from 6 – 20+

9. What % of time do your community nurses spend on face to face direct patient care?

Answer: The % of time spend on direct patient care varies according to patient need but the general principle is that Band 7’s operate 20/80, Band 6’s 60/40, Band 5’s 80/20 and Band 3’s 90/10.

10. Have you identified the competencies required for staff at each band? If yes please provide?

Answer: Staff training requirements are reviewed during staff’s annual appraisal and a development plan is agreed. A training need analysis is provided to Learning and Development each year to ensure that training available meets the needs of the service. All qualified staff undertake 2 days mandatory training annually.
