E-mail: [email protected]. Study objective: The antivenom currently available for treatment of systemic black widow envenomation (latrodectism) is composed of equine whole immunoglobin. Although considered effective, it has been associated with anaphylaxis and 2 reported fatalities. We test the efficacy and safety of new equine antivenom composed of purified F(abʹ)2 antibody fragments. Methods: A randomized, double-blind, placebo-controlled trial was conducted at 16 sites across the United States. Subjects aged 10 years or older with moderate to severe pain because of black widow spider envenomation received F(abʹ)2 antivenom or placebo. The primary outcome measure was treatment failure, which was defined as failure to achieve and maintain clinically significant reduction in pain for 48 hours posttreatment. Secondary measures of pain intensity differences and summed pain intensity difference were computed. Adverse events were recorded. Results: Sixty patients were treated (29 antivenom and 31 placebo). The mean age was 39 years and 68% were male. There were 15 treatment failures in the antivenom group and 24 in the placebo group (P¼.019).Differencesinpainintensity difference between groups were lower at each postbaseline point, and the mean summed pain intensity difference was greater for the antivenom group (difference 2,133; 95% confidence interval 177 to 4,090). No deaths or serious drug-related adverse events were detected. Conclusion: The F(abʹ)2 antivenom met the predefined primary outcome of reduced treatment failures. Secondary outcomes of pain intensity difference and summed pain intensity difference also supported efficacy. The rate of symptom improvement in the placebo group was higher than expected, which may be related to enrollment criteria or placebo effect.     ANTIVENIN- latrodectus mactans Merck Sharp & Dohme Corp. ---------- ANTIVENIN (LATRODECTUS MACTANS) (Black Widow Spider Antivenin) Equine Origin DESCRIPTION Antivenin (Latrodectus mactans) is a sterile, non-pyrogenic preparation derived by drying a frozen solution of specific venom-neutralizing globulins obtained from the blood serum of healthy horses immunized against venom of black widow spiders (Latrodectus mactans). It is standardized by biological assay on mice, in terms of one dose of Antivenin neutralizing the venom in not less than 6000 mouse LD50 of Latrodectus mactans. Thimerosal (mercury derivative) 1:10,000 is added as a preservative. When constituted as specified, it is opalescent, ranging in color from light (straw) to very dark (iced tea), and contains not more than 20.0 percent of solids. Each single-dose vial contains not less than 6000 Antivenin units. One unit of Antivenin will neutralize one average mouse lethal dose of black widow spider venom when the Antivenin and the venom are injected simultaneously in mice under suitable conditions. CLINICAL PHARMACOLOGY The pharmacological mode of action is unknown and metabolic and pharmacokinetic data in humans are unavailable. INDICATIONS AND USAGE Antivenin (Latrodectus mactans) is used to treat patients with symptoms due to bites by the black widow spider (Latrodectus mactans). Early use of the Antivenin is emphasized for prompt relief. Local muscular cramps begin from 15 minutes to several hours after the bite which usually produces a sharp pain similar to that caused by puncture with a needle. The exact sequence of symptoms depends somewhat on the location of the bite. The venom acts on the myoneural junctions or on the nerve endings, causing an ascending motor paralysis or destruction of the peripheral nerve endings.

The accident occurred at the victim’s home, in Aragua de Barcelona, Anzoátegui State. The 31-year-old female victim was bitten twice on the left scapular region, in quick succession (within seconds). She developed a hyperactive state of the central, autonomic and peripheral nervous systems with minor local symptoms. Key-words: Latrodectus geometricus. Araneae. Theridiidae. Latrodectism. Venezuela. RESUMO Descreve-se o primeiro caso de envenenamento por Latrodectus geometricus na Venezuela. O acidente ocorreu na residência, em Aragua de Barcelona, Estado Anzoátegui. A paciente de 31 anos de idade foi picada, consecutivamente duas vezes, em segundos, na região escapular esquerda.
