    1748-54 JEROME AVENUE, BRONX, N.Y. (CORNER OF EAST 175 ST.) On August 21, 1988, the weather was warm and sunny. In the New York City Fire Department (FDNY) in the Bronx, routine tasks were being conducted and Multi-Unit Drills were taking place. At 1748 Jerome Avenue in the Bronx, a single story taxpayer stood at the corner of E. 175 Street. The building was 50 feet x 100 feet. The building contained six stores, four fronting on Jerome Avenue, and two fronting on East 175 Street. In the cellar of this building, there was an illegal social club called “El Hoyo” (aka: “The Cave” or “The Hole”). The only entrance to this illegal club was off of the Jerome Avenue side. There was a stairway approximately 10 feet from the front of the building. The stairway entrance was flush with the floor and the wooden stairway was steep. Inside the club in the cellar, the ceiling was low (7 feet). There were candles burning in the club. There was a bar towards the front of the occupancy. There was no sprinkler system, automatic alarms or fire extinguishers in the social club. There was no emergency lighting. A Bouncer searched a man attempting to enter the club with the container on his person. The Bouncer found the container, and then ejected the patron. The Bouncer kept the container filled with gasoline. It was being stored behind the bar.

On July 12, 1979, the Landmarks Preservation Commission held a public hearing on the proposed designation as a Landmark of the Park Plaza Apart­ ments and the proposed designation of the related Landmark Site (Item No.6). The hearing had been duly advertised in accordance with the provisions of law. One witness spoke in favor of designation. There were no speakers in opposition to designation. DESCRIPTION AND ANALYSIS The Park Plaza is one of the first Art Deco apartment houses to have been built in the Bronx. Its designer, Marvin Fine, knew of and consciously synthesized the major elements of the new modernistic skyscraper style being developed in Manhattan by Raymond Hood and William Van Alen, and adapted them to the lowrise apartment houses of New York's residential neighbor­ hoods. One of the handsomest Art Deco buildings in the Bronx, the Park Plaza was a pioneering work which helped change the face of the borough.     2016 COMMUNITY HEALTH NEEDS ASSESSMENT EXECUTIVE SUMMARY Community Health Needs Assessment, 2016 Update Purpose of the Community Health Needs Assessment This 2016 Community Health Needs Assessment (CHNA) updates the CHNA completed in 2013 to meet the requirements of Section 9007 of the 2010 federal law, The Affordable Care Act (“ACA”). The ACA requires that any tax-exempt, IRS- designated 501(c) (3) hospital complete or update a publicly-available, comprehensive CHNA every three years in order to document the extent to which it understands the unique characteristics and needs of the local communities it serves, and responds to these needs by delivering meaningful and effective community benefit through clinical services and other programming. Required Components A CHNA report has five required components: 1) Definition of community served 2) A prioritized description of the significant health needs of the community 3) Transparency in the process and methods used to conduct the CHNA, including how it took into account input from the community served and prioritized community health needs 4) A description of the resources potentially available to address the identified significant prioritized community health needs 5) An evaluation of the impact of actions taken to address the significant health needs identified in the previous CHNA report (June 2013). A CHNA report is considered complete when it is adopted by a governing body of the facility and made widely available to the public. Community Served NYC Health + Hospitals serve all New Yorkers in every neighborhood in New York City regardless of their ability to pay.
