Activity Overview The Clinical Sessions Package is intended for board certified pharmacists in need of recertification credit and is designed based on the content outline developed by the Board of Pharmacy Specialties (BPS). Using a case-based approach, the activities provide high-level, interactive discussions on contemporary and emerging issues in geriatric pharmacy.

The Geriatric Pharmacy Clinical Sessions Recertification Package consists of 4 activities (see table below) and provides up to 5.0 hours of continuing pharmacy education and/or recertification credit.

These activities are part of the ASHP and ACCP professional development program for BCGP recertification approved by the BPS. For pharmacists who maintain more than one certification, three of the activities are approved for multiple specialties (see table below).

Recertification Credit* Board certified pharmacists are eligible to receive up to 5.0 hours of recertification credit for completing this package. To earn recertification credit, learners must review the activity content and successfully complete the online assessments by the deadline. Only completed assessments will be eligible for credit; no partial or incomplete assessments will be processed. You are allowed only one attempt to successfully complete this assessment.

Recertification credit is earned in the calendar year in which you pass the recertification assessment.

Managing Sepsis in the Older Adult ACPE Number: 0204-9999-20-292-H01-P Learning Objectives At the conclusion of this application-based educational activity, participants should be able to do the following: • Describe physiologic changes in the older adult that affect early identification of sepsis. • Evaluate the current literature, including the Surviving Sepsis Campaign, regarding treatment recommendations for sepsis in older adults. • Design a treatment strategy for sepsis in the older adult. Not so FAST – Engaging in SLOW Decision Making for Secondary Stroke Prevention ACPE Number: 0204-9999-20-319-H01-P Learning Objectives At the conclusion of this application-based educational activity, participants should be able to do the following: • Evaluate literature that defines secondary stroke treatment goals, including prevention, to the care of older adults. • Identify clinical situations for secondary stroke prevention where life expectancy, functional status, patient preference or goals of care should override standard recommendations for screening/treatment. • Recommend secondary stroke prevention strategies for older adult patients with multiple conditions, including hypertension, dyslipidemia, atrial fibrillation, tobacco use disorder, diabetes, and hormone deficiency.
