 “Dog Walk” Form ~ The form is only required if you wish to pay at the “Dog Walk” event by cash or check. Please note that payment for FHS merchandise and PTSA bulldog apparel are separate. If paying by check, two checks will be required. For Folsom High School merchandise (yearbooks, PE clothes, sports pass, student parking, and ASB sticker), please make checks payable to Folsom High School. For PTSA bulldog apparel (sweatshirts, t-shirts, pajamas, stickers, bulldogs, etc.), please make checks payable to FHS PTSA.

 Dress Code Policy

 FCUSD ~ Instructional Calendar ~ Please review instructional days. Note: First semester finals are scheduled on December 14th, 15th, and 16th. Second semester finals are on May 23rd, 24th, and 25th. These are minimum days for students ~ 8:00 a.m. – 12:23 p.m. Please do not schedule vacations during this time. Final examinations are “mandatory” at Folsom High School and will not be given early for any reason. Students who miss their final exams will be issued a grade of “incomplete” for the semester. Students are required to make- up their missed final exams during the first 2 weeks of the following semester to replace the “incomplete” grade with a letter grade. Failure to do so may result in a semester grade of “F” (fail).

 Folsom High School ~ Parent Teacher Student Association (PTSA) ~ Be a part of Folsom High School’s Parent Teacher Student Association by completing the PTSA Volunteer/Membership Form. The PTSA offers scholarships; however, membership is required for eligibility. Please turn your membership form in at the “Dog Walk” event or to the Folsom High School Administration Office.  Transportation (no attachment) ~ If your son/daughter will be using FCUSD transportation, please refer to the Transportation Department for an application and information. Applications are available through the Folsom Cordova Unified School District website: www.fcusd.org; select “Departments”’; select “Transportation”’; select “Forms”; then download the application. Applications will also be available at the “Dog Walk”. Applications must include a photo and a check or money order payable to FCUSD, and must be mailed to FCUSD Transportation Services, 11458 Elks Circle, Rancho Cordova, CA 95762. NEW for the 2016-17 school year:

1) The athletic registration process will now be completed online (no more paper packets) except for the Sports Physical form which will need to be signed by a physician. Please return the original signed Sports Physical form to the office, attention Lisa Finley.

2) The athletic registration process must be completed prior to any summer or offseason workouts, camps, games, conditioning and/or weightlifting.

3) Sport physicals are now valid for one calendar year. For example, if your physical form is signed on February 1, 2015, it will expire on February 1, 2016. The Sport Physical form must be signed by a medical doctor; chiropractors and physical therapists cannot sign a physical form. Every student-athlete must have a current sports physical on file before he/she can participate in try-outs, conditioning, practice or game/competition.

To access the NEW online registration and sports physical form, go to www.fcusd.org/fhs; then click on the “Athletic” tab to download the forms. Sports physical forms are also available in the FHS administration office.

For tryouts, please dress in workout clothes (shorts, t-shirt, athletic shoes), bring plenty of water to drink, and sunscreen is advisable.

There is a $125.00 transportation fee per sport for those students who make a team. Please make check payable to Folsom High School.

First Day of School Information The 2016-2017 school year begins on Tuesday, August 9, 2016, at 8:00 a.m. If your son/daughter is scheduled to have a zero period class, please note that there will be no zero period classes on the first day of school. Zero period classes start on the second day of school. Regular school hours are 8:00 a.m. – 3:03 p.m. Monday through Thursday (for students with a zero period 7:00 a.m. – 3:03 p.m.) and on Friday 8:00 a.m. – 1:26 p.m. (for students with a zero period 7:00 a.m. – 1:26 p.m.).

Dress Code Policy **Revised** Please remember that a successful school experience for students is largely dependent on regular school attendance and appropriate behavior. By working together, we can ensure a successful 2016-2017 school year. Please refer to the attached dress code policy flyer. During the first week of school, your child will be receiving a student PAWSPORT handbook and yearly planner that provides detailed school policy information.

Back-to-School Night Save the date for Back-to-School Night on Thursday, September 8th, starting at 6:00 p.m. in your son/daughter’s first period class. This will be a minimum day for students ~ school hours will be 8:00 a.m. – 12:23 p.m.

Homecoming Homecoming is scheduled for the week of September 19th through the 24th. The homecoming football game will be on Friday, September 23rd, and the dance will be on Saturday, September 24th.

Athletic/Extra Curricular Requirements

The Folsom Cordova Unified School District does NOT carry dental or medical insurance for students injured on school premises, while under school jurisdiction, or while participating in school district activities. You are responsible for the medical bills if your child gets hurt during school activities.

Insurance coverage may be arranged by parents through individual policies, group insurance or through a voluntary insurance program of the Student Insurance Company. The Student Insurance policy covers accidental injuries sustained by a pupil traveling to and from school and while participating in regular school activities including athletics; see chart on reverse side for rates.

I. ATHLETIC INSURANCE - Please Read Carefully California State Law (Education Code 32220-32224) requires every member of an athletic team to have accidental bodily injury insurance providing at least $1,500 of scheduled medical and hospital benefits. Insurance must be applicable to practicing for, participating in, and traveling to or from interschool contests. These requirements can be met by private insurance if the student already has such protection, or a plan can be purchased through Student Insurance at reasonable rates.

SPORTS OTHER THAN FOOTBALL AND SKIING - One of the following is required: Student Insurance - See chart on reverse side for rates. (1) School Time (2) 24- hour Accident Insurance

II. REQUIREMENT FOR EXTRAMURAL ATHLETICS A) Before a student may participate in any extramural athletic squad practice or contest, the student must present evidence of: (1) A physical examination by a California Licensed physician (2) A concussion and head injury information sheet (3) Accidental injury insurance that covers medical and hospital expenses (4) Parental permission

"Member of an athletic team" includes members of school bands or orchestras, cheerleaders and their assistants, pompom girls, team managers and their assistants, and any student or pupil selected by the school or student body organization to directly assist in the conduct of the athletic event including activities incidental thereto.

NOTE: All extracurricular participation forms must be completed and on file with the Vice-Principal or Athletic Director before a student is issued a uniform, allowed to practice, or participate/compete.

“Is the PTSA really relevant? Don’t you guys just run bake sales and sell magazines?”

Not only are we relevant, sometimes the PTSA is the difference that takes Folsom High from a good school to a great school.

It’s no secret that for years now, we have lacked adequate funding to provide some of the educational experiences our children deserve. What you may not know is that, with the generous support of parents and staff, the PTSA has been making a difference by sponsoring and funding programs that impact EVERY child in this school, throughout their entire high school experience.

Each and every one of your children - from a freshman to a senior – continues to benefit from PTSA programs. We sponsor programs starting with the Freshman Kickoff – a unique program designed to ease that difficult transition to High School - all the way up to Sober Grad Night, the final, memorable evening of your child’s high school experience. With limited counseling resources available at FHS, the PTSA has stepped in to offer SAT and ACT practice tests throughout the year as well as High School Transition and College Planning evenings for parents.

The PTSA is making a difference in the classroom too. From textbooks to Lab equipment, from the clubs your student joins to the crackers they get from the nurse’s office on that day they just didn’t feel well, the PTSA is there to help.

So, become a member. The $10 membership fee is one of our key sources of funding AND you will automatically be on our mailing list to receive relevant information and updates.

Volunteer – we always need help. There are opportunities from a couple of hours chaperoning a school dance or helping with Spirit Wear to major involvement as a Board member.

Support our fundraisers, including our Fall Buck$ for Bulldogs and our Spring eBash Online Auction. Take the time to attend our programs.

WHO: All students 12 to 25 years old enrolled in the Folsom Cordova Unified School District. Students enrolled in San Juan and Twin Rivers Unified School Districts have also been invited.

WHY: It is estimated 1 in 500 school-aged children suffer an undiagnosed heart defect, which can lead to sudden cardiac arrest and death. A typical annual physical does not screen for advanced heart abnormalities. This does not take the place of your annual or pre-participation sports physical.

WHAT: Screenings take 60-90 minutes (wait times may vary) and include a health history questionnaire review, an EKG test, and if warranted, an echocardiogram. The entire process is non-invasive—no needles or x-ray exposure—and we ensure each teen’s confidentiality, privacy and individual modesty is respected throughout all aspects of the screening.

HOW: Your child must be registered in advance. Please bring their completed Health History Form and student ID card to the event. Please register and access forms at viaheartproject.org/screenings/ . Registration opens August 1, 2016 to noon on September 16th, 2016. Registration is limited to 750 students.

Our research indicates that participants in these types of programs have earned higher grade point averages, been more likely to be involved in extracurricular opportunities, and have had fewer disciplinary situations than students who have not participated. There is no doubt that the experience will be informative and fun. We urge all incoming freshman to attend our 2016 LINK CREW/Freshman Orientation on August 3, 2016 from 7:30am-2:00pm. Freshman Orientation is a unique opportunity for our freshman to connect with fellow peers and our campus. From 12:30pm-2:00pm, LINK CREW will provide a BBQ for freshman. Orientation and the BBQ is a student only event. There will be a parent informational meeting from 1:00pm-2:00pm on the same day in the FHS Theatre. Parents will be expected to pick up their students at the end of the parent informational meeting at 2:00pm.

LINK CREW 2016 What is it? LINK CREW 2016 is the first step in your orientation to Folsom High School. It is a one-day orientation for incoming Freshmen that is led by the staff and volunteer upper-classmen of Folsom High School. The orientation from 7:30-2:00 pm is only for students, but there will also be a separate meeting for the parents in the Theatre from 1 – 2 pm, where you too can get oriented, meet some FHS staff and have an opportunity to ask questions. The orientation and parent mmeting will be done at 2 pm.

What will be accomplished?  You will have fun!  You will learn how to get around the Folsom High School campus.  You will learn about important issues from upper-classmen.  You will meet and hear from several teachers and staff members.  You will be made aware of the many activities available at Folsom High School.  You will meet incoming freshmen coming from other schools as well as your own middle school.  You will be introduced to your upper-class mentors who will meet with you multiple times throughout the school year.  You will make new friends.  Did we mention you will have fun?

When and where is it? WEDNESDAY, AUGUST 3, 2016 7:30 a.m. to 2:00 p.m. Folsom High School, 1655 Iron Pt. Road. Check-in begins at 7:30 a.m. at the Prairie City Gymnasium entrance.

Sober Grad Night—What is it?—A safe and fun, alcohol and drug free celebration for seniors on graduation night, organized by a group of dedicated volunteers and supported by our community. Seniors buy tickets for $62-80 each, but this in no way covers the actual cost of Sober Grad Night.

Cow Pie Bingo—What the heck is that??!!!--the primary fundraiser for Sober Grad Night. CPB raises $8,000 each year. Sober Grad typically carries forward $5,000 to start up the next year’s Sober Grad Night.

“Pay It Forward” Campaign-- Seniors are encouraged to buy 10 CPB tickets and then resell to Friends & Family. The “Pay It Forward” Campaign focuses on students supporting Sober Grad throughout their four years in smaller increments. We urge all freshmen, sophomores & juniors to buy at least 2 tickets each year, ultimately supporting their own Sober Grad Night. Two tickets cost $10 — a small price to pay for a chance to win $1,000 and support a great cause.

We are strongly encouraging parents of freshmen, sophomores, juniors and seniors to purchase your tickets at the Dog Walk. All senior parents will receive a phone call to purchase tickets.

How CPB works: A grid of squares is marked off on a field at Zittel Farms and the squares are numbered in random order; your CPB ticket number corresponds to a number on that field. A cow is turned loose on the grid to deposit their “PIE”. The square containing the “PIE” is the grand prize winner of $1,000, the four corner squares will each receive $100. Winner need not be present to win.
