Attendees introduced themselves. Apologies as above. Actions from previous meetings: • MCa discussed issues with SHINe website partition. S Howie suggested using a system called wiki and perhaps the SHINe website could be moved over. Issue MCa 1 remains ongoing. • MCa commented on tissue request form development. A form has been generated and used for the first 3 applications but further discussion on its Archive format would take place during the archive steering committee meeting being Steering held at the end of the SHINe meeting. Group 1 Presentation - Cytology Update given by Sheila Nicoll The talk will be available on the SHINe website. The history of cervical cytology service delivery in Scotland was discussed including the establishment of a cytology steering group. Current and potential future screening scenarios were presented. In addition, it was highlighted that HPV testing was now part of cervical screening with rollout of this component in Test of Cure now live. A potential new HPV prevalence study for other age ranges (other than 20/21y) was presented which could inform Scotland age range and screening frequency requirements. A report is awaited from the Cytology Steering Group regarding this matter. 2 Presentation - HPV QA and Validation for English CSP given by Catherine Moore The talk will be available on the SHINe website. The presentation described the role of the Scottish HPV Reference Laboratory in supporting QA of HPV testing on various approved platforms for up to 33 different laboratories in England. This role may continue to a lesser extent in future as the English labs become more established in molecular testing. 3 Presentation - Future Shape of SHPVRL and HRG given by Heather Cubie The various roles for each were presented, including how they interact and facilitate one another. Preliminary data on the STOCS-H study was also shown. 4 Presentation - HPV vaccine change and future surveillance given by Katy Sinka Data relating to the current HPV vaccination programme and potential consequential scenarios was presented. This talk also highlighted the vaccine switch from the bivalent Cervarix to the quadrivalent Guardasil. 5 Presentation Scottish Health Research Register SHARE given by Fergus Daly The background and current status of this project was summarised together with the public’s reaction to the register. 6 Presentation Update from BSCCP given by Margaret Cruickshank Issues regarding training in the context of HPV testing were presented together with the potential requirement for other biomarkers in relation to HPV positive/Cytology negative individuals as further triage before colposcopic referral. Afternoon Session: Research Update The afternoon session was chaired by Heather Cubie 7 Open Discussion - Scotland and Oropharyngeal Carcinoma where next for service and research? Discussion led by Liz Junor The session started with an open discussion on the topic ‘ Scotland and Oropharyngeal Carcinoma – where next for service and research?’ led by Liz Junor. HAC had prepared a number of questions and discussion was wide ranging. A summary of the discussion has been produced as a companion to these Minutes. In relation to HPV testing as part of the diagnostic assessment of potentially cancerous oropharyngeal lesions, a number of people present expressed interest in being associated with the development of a business case for funding of such HPV testing. Kevin Pollock of HPS suggested it would be reasonable to present such a case as a Reference Laboratory development proposal. A shorter discussion was held on the development of a protocol to study oral HPV prevalence in the population through oral rinse samples. Again a number of people expressed interest in being part of a protocol development team. HAC agreed to progress discussions by email, teleconference or meeting . HAC 8 Presentation - Update on HSR programme & early results from one of the HSR studies given by Seonaidh Cotton An overview of the Health Sciences Research programme grant was shown comprising of 5 study elements. The initial results of one study, COGHS, showed the positive predictive value of colposcopy relating to HPV type. It was envisaged the positive predictive value of colposcopy would be lower in a vaccinated 2 population. 9 Presentation - Vaccination and screening: knowledge and information needs of young women given by Christine Campbell CC explored the implications of HPV vaccinations on future cervical cancer prevention policy. The first part of this CSO project was a literature review which revealed a dearth of literature around the intersection of HPV vaccination and cervical screening behaviours. The second part will involve a survey of young women in Scotland to obtain a current understanding of young women’s attitudes towards HPV infection, cervical cancer, associated risks and cervical screening. A draft questionnaire has been subjected to cognitive interviewing to refine the questions making it suitable for girls below Higher Grade, at Higher Grade and Degree Level. The survey will be delivered with help from the Scottish Primary Care Research network to girls aged 18-22 yrs, over a 15m period during 2012-13. 10 Presentation - Investigations of novel HPV-16 E7 protein interactions given by Ramya Gundarao Ramya explained that her project aimed to identify genomewide HPV protein interactions and novel host binding partners of HPV, using yeast two-hybrid system. She described a new interaction between HPV 16 E7 and E1 utilising zinc binding CCKC domain of E7 and the N terminal 92 amino acids of E1. At this stage she has concluded that E7 might play an inhibitory role on E1 mediated HPV episomal replication. In the second part, Ramya has identified a number of interactions between cellular and HPV proteins using Lumier assays and has recently concentrated on the interaction of E7 with SPZ1, hypothesising that SPZ1 may play a role in E7 mediated tumourigenesis. 11 Presentation - HPV Archive: Numbers, QA and applications for use given by Maurice Canham MC highlighted that there were over 12,000 samples, mainly LBC from 20-21 year olds and most with 2 or 3 aliquots usually of 4 mls each. Much time has been spent recently on developing QA protocols to ensure that samples are stored adequately and validated for DNA, RNA or protein analyses. There had been 3 full applications for access to samples. 12 Presentation - Summary of SHINe networking activity given by Heather Cubie As time was running short, HAC rapidly summarised activity and stressed that there was a need to broaden our funding base which was to date, heavily reliant on CSO support.
