The objective of the FULLBEAUTY Brands Testing Program is to ensure that products meet or exceed our performance standards as well as all Federal and State Compliance Regulations.

FULLBEAUTY Brands understands that timing of production is crucial. Please read this document to ensure that you understand FULLBEAUTY Brands’ testing requirements.

TEST REQUIREMENTS: • FABRIC TESTING • GARMENT TESTING • TRIMS: Dyed to match trims are only required to be tested in the garment stage for any noticeable appearance and functional changes to the care instructions. Non-Dyed to match trims must be tested for all colorfastness properties (same as fabric testing). • ZIPPERS • TAGLESS LABELS • COMPONENT PART TESTING • ACCESSORIES: Hats, gloves, scarves, bags, belts, jewelry, etc. NOTE: See Appendix II for complete list of the lab testing required in each Test Protocol Package.

WHEN & WHAT TO LAB TEST: • Package Test Options Option 1: o Fabric Test Package: Test Shell and Lining (if applicable) First Colorway - If fabric is submitted for the initial lab test, then only fabric testing requirements are performed. Additional Colorway(s) Test Package - If there is more than 1 color or if new colors are added during the testing year, then additional colorways must be submitted for fabric and colorfastness testing. o Garment Testing: If fabric is submitted for testing separately, then garment testing is required after the fabric tests have passed and then only garment testing will be performed. Option 2: o Full Garment Test Package: Test Shell and Lining (if applicable) If a garment is submitted for the initial lab test, then all fabric and garment testing requirements must be lab tested. Additional fabric may be required if there is not enough fabric available to perform all of the lab tests in the garment sample.

FULLBEAUTY Brands www.fbbrands.com 3 Additional Colorway(s) Test Package - If there is more than 1 color or if new colors are added during the testing year, then additional colorways must be submitted for fabric and colorfastness testing. NOTE : o Please review the testing protocols carefully before you quote product costs and again before you submit lab test samples to ensure your products comply with all requirements! o Production should never start before receiving a Pass Report with Final Approval from the Product Testing Department. o If production has started/completed before a Pass Report is obtained, the supplier assumes responsibility and ownership if design or composition is modified or rejected due to failure.

• Sample Requirements Fabric: o Submit all colors for testing to the lab at the same time on the same Test Request Form. o All testing must be done on bulk fabric. o Only one color way will be tested for dimensional stability and appearance after laundering. o Fabric used in the production of multiple items can be tested together. Fabric Sample Quantity: o Submit 1/2 yard of fabric each color. 1 yard each color for flammability testing. o 5 or less color ways ordered Test all colors. o 6 or more color ways ordered: Test 1 light, 1 medium 1 dark & 1 bright color way (if applicable). All heather color ways. All Neon color ways. Red and Black color ways must always be tested. o Additional fabric yardage may be required for performing lab testing. Garment: o Samples must be a representative of the actual finished product from the factory producing bulk production. o Only one size is required to be submitted for testing in any size you choose. o Approved trims in production quality MUST be attached to the finished product submitted for testing. o Please test the most intricately constructed item using the same fabric. o Submit 2 garments (one garment for testing and one for control sample). Full Package Testing – if only the garment is submitted for both fabric & garment testing, then 1 additional garment sample or additional fabric is required if colorfastness testing is conducted at the garment phase.

• Re-Orders o All reorders that meet the following criteria will NOT need to be retested: Mill/factory/fabric content and construction have NOT changed. Valid lab report(s) that have been issued within one year. o New colors, patterns or prints added later in the testing year must be tested for colorfastness.

FULLBEAUTY Brands www.fbbrands.com 4 • Pre-verification or Development Testing o FULLBEAUTY Brands’ negotiated pricing will be applicable. o Lab reports will only be sent to the supplier requesting the lab testing. o These reports can also be used to obtain an approved care instruction as long as they are representative of production and forwarded to the FULLBEAUTY Brands product testing e-mail address ([email protected]).

WHERE TO TEST • Approved Testing Laboratories: FULLBEAUTY Brands has designated 4 Accredited Third Party Laboratories for Testing which are as follows: o Bureau Veritas (BV) o Intertek o UL Verification Services, Inc.

HOW TO SUBMIT PRODUCTS FOR TESTING 1. Review Testing Protocols : • Find the Lab Testing Requirements and Standards on FULLBEAUTY Brands’ B2B website: (www.b2b.fbbrands.com) Passcode: 1StopPlus [Note: Password is case-sensitive]. Internet Explorer web browser is not compatible with the FULLBEAUTY Brands B2B. • Review protocol for sample size and additional tests required. o Additional tests may be required after the samples are reviewed by the lab. o Additional samples/fabric may be necessary to perform additional lab tests which may incur an additional charge.

2. Complete the Test Request Form (TRF) : o The TRF can be found on FULLBEAUTY Brands’ B2B website. o All information must be completed on the TRF. Incomplete, missing or incorrect information may result in delay in testing or re-testing at the supplier’s expense. o If testing fabric used in the production of multiple items then every item number must be referenced on the TRF. o Petite and/or tall item numbers must be referenced on the TRF of the Avg. or Big item number. o The full package lab test report number for additional color ways must be referenced on the TRF. o If retesting is required due to a failure , a new TRF must be submitted and the previous lab test report number must be referenced.

3. Submitting Samples to the Labs o The TRF must be submitted with the samples. o Please attach the Bill of Materials from the Product Specification File (PSF) to the TRF. o The TRF must be attached to the samples in order to receive the FULLBEAUTY Brands negotiated pricing.

FULLBEAUTY Brands www.fbbrands.com 5 NOTE: o Samples must be submitted for testing in a timely manner in order to allow sufficient time to correct any problems that may arise during the testing procedure including time needed to retest. o If you submit fewer samples than the stated sample size on the protocol, please check with the lab to confirm the impact on your testing turn-around-time.

TEST REQUEST FORM (TRF) INSTRUCTIONS: • Fields denoted with an * are mandatory! • Please select the appropriate information as indicated in each field that says “Please Select”. • Missing information may result in a delay!

BRAND • Brand - Please select the brand(s). o More than one brand may be tested if the same fabric is used in multiple brands. o Collection Sharing for Europe Market – select yes only if the item is produced for the EU Market Section 1: Contact Information: • Vendor# & Name (as listed on the FULLBEAUTY Brands purchase order) • Factory name and submitter’s name & email address. • Agent/Sourcing office and Contact as appropriate. Section 2: Product Testing Information: • Lab Test Report Type – select Initial, Re-Test or Q.A. • Product/Fabric type – select knit, woven, sweater, leather, accessory, trims, or bags. • Test Protocol (end use) – select the protocol(s) in which the laboratory should use for testing. o There are 4 slots in case there are multiple test protocols to be lab tested. • Apparel Package Test Type - Fabric Test Package, Additional Colorway Package, Garment Test Package, Full Garment Test Package. o There are 3 slots in case there are multiple test types for the sample submit required. o If the Apparel Package Test Type is not applicable, then select the appropriate Test Protocol. • Product Category – Select the appropriate categories: Bags, Beads, Belts, Buttons, Elastic, Fabric, Garment, Hats/Gloves/Scarves, Snaps, Jewelry, Leather, Screen print Tee, Sequins, Sunglasses, Tagless Label, Trims, Zipper o There are 3 slots in case there are multiple product categories for the sample submit required. • Retest Reason(s) – select the appropriate reason. o There are 4 slots in case there are multiple retest reasons. • Previous Report Number(s) for Retesting– type the previous report number in this field. • Retest Corrective Action Taken – list the steps taken to improve the merchandise in order to meet the standards and requirements previously failed. Section 3: Trim Supplier Information: • Nominated Trim Supplier – o Zipper or Trim 1-6 – select the appropriate nominated trim supplier from the list. o If the trim supplier is not listed, then lab testing is required or the Component Part(s) Lab Testing process must be followed. • Component part(s) testing

FULLBEAUTY Brands www.fbbrands.com 6 o If the factory has the appropriate lab test report(s) for the trims, then they may submit the documents to FULLBEAUTY Brands’ approved laboratory in lieu of lab testing. o The Trim Type(s), Trim Materials, Lab Name and Lab Test Report number must be listed on the TRF in order for the trim(s) to be exempted from lab testing. Section 4: Product Information: • Description(s), dept., ALL item #(s), color name(s), style# & PO#. Section 5: Material Information: • Season – please list the season in which the purchase order has been issued. • Ship date(s) • Sweater wt. – based on a size Large (women) & 3XL (men) sample. • Fabric information of main body fabric 1, 2 and lining - Fiber Content, Fabric Structure, Fabric Wt., Thread Count, Yarn Number. • Claims – select from the list of stated claims. • Fabric or garment finish – select from the list of finishes. • Filling – enter the fiber content. • Select “Plasticol Print” if submitting a screen print product. • Print Process/Dye Type – enter the information if applicable. • Submitted Care Instructions and additional comments if applicable. Service Required: • Regular Service (4-5 working days) o Testing turnaround time will vary based on the product type tested. • Rush services (3 working days) o May or may not be available and would be subjected to a surcharge. o Are only available if sufficient samples are submitted to the lab. Factory is responsible for any delays due to insufficient sample size submitted to the lab. o Chemical analysis may take longer. • Input the date samples are sent and company’s approval to conduct testing.

TRIM SUPPLIER S.O.P • Nominated Trim Suppliers: o Chemical & Physical lab testing on trims are exempt for nominated trim suppliers. YKK & TALON are FULLBEAUTY Brands’ nominated trim suppliers. No documentation or document review is required by FULLBEAUTY Brands’ approved 3rd party laboratories. NOTE: o Nominated trim suppliers are solely responsible for the accuracy of documents presented to FULLBEAUTY Brands. o For any invalid or fraudulent documents, the trim supplier will be held accountable and liable.

• Component Part Testing: o Component part testing on trim(s) is exempt if the following procedure is followed: The factory must submit documentation of the required lab test report(s) for each applicable trim from an independent certified accredited testing laboratory to FULLBEAUTY Brands’ approved 3 rd Party Laboratory.

FULLBEAUTY Brands www.fbbrands.com 7 The lab report(s) of the applicable trim(s) is valid for one year. The approved 3 rd Party Laboratory will review the document(s) submitted to ensure all applicable required lab test(s) were conducted to the component parts testing protocol. The lab must denote “ satisfactory ” on the lab test report. If all testing was completed and denoted as “ satisfactory ”, then the trim(s) will be “Exempt” from component part lab testing on the garment. The document review is “FREE”, no additional cost to the vendor. NOTE: o If required documents for the trim(s) are NOT attached to the TRF and submitted with the garment, then a document review will NOT be conducted and the trim(s) will NOT be “EXEMPT” from lab testing. The laboratory will proceed with the applicable required testing of each trim.

TEST RESULT PROCEDURES:

• Lab Testing is Completed: o The lab will rate the results of the tests performed against our Product Performance Standards as either a “PASS” or “FAIL”. o The lab will email or fax the report to the vendor and upload into FULLBEAUTY Brands’ FlexPLM system. o FULLBEAUTY Brands only accepts lab reports that are uploaded into FULLBEAUTY Brands’ FlexPLM system or emailed directly from the lab to [email protected] .

••• FULLBEAUTY Brands Lab Report Review: o After the lab test report has been uploaded into FULLBEAUTY Brands’ FlexPLM system, the Product Testing team will review the required test results and advise the sourcing office/vendor of either: 1. Pass with Final Approval Approved Care Instructions (only after all required testing is completed) Required Disclaimers 2. Pass with Waiver Does not meet the standards, but is within an acceptable tolerance and production can proceed. Vendor must improve goods for future orders. 3. Fail Improvements and retest(s) required on any testing failures Missing required test results Additional recommendations NOTE: o Any care instructions prior to lab testing are only recommended care instructions and may be subject to modification. ••• Passing Samples o Remain at the lab for a period of 3 months. o If samples are returned to the vendor and FULLBEAUTY Brands requests the vendor to send to the FULLBEAUTY Brands Product Testing Team, please leave all tags from the lab intact.

FULLBEAUTY Brands www.fbbrands.com 8 ••• Timeline for Lab Test Results: o FULLBEAUTY Brands’ Product Testing Team will issue a pass, pass with waiver or fail report based on their assessment of the lab test results within 5 business days. o Only if you have not received the results within the 5 day period should you follow up with a telephone call or e-mail to the [email protected] email address.

••• Label Approval o Care Labels – Factories must produce the care label based on the approved care instructions listed on the Final Pass Report issued only by the Product Testing Team. Sourcing offices/suppliers must strictly adhere to the FULLBEAUTY Brands vendor compliance packaging manual for label instructions listed on the B2B website (www.b2b.fbbrands.com) Passcode: 1StopPlus Note: Password is case-sensitive]. It is the Sourcing office/vendor’s responsibility to review and approve all labels prior to shipping merchandise to the USA. NOTE: o Merchandise that does not meet the FULLBEAUTY Brands packaging or labeling requirements will be sent out for correction at a 3 rd party facility. o The factory is responsible for all fees incurred in transit costs and re-label costs at a 3 rd party facility.

PAYMENT TERMS: ••• All testing fees are the sole responsibility of the vendor/factory submitting the samples.

LAB TESTING COMPLIANCE:

o Vendors are required to provide all applicable required lab test results to the FULLBEAUTY Brands Product Testing department. o The lab test report "completion date" may not exceed more than 1 year. o All vendors producing FULLBEAUTY Brands’ private label merchandise must receive an FULLBEAUTY Brands final pass report with approved care before shipping goods. o Failure to comply with our FULLBEAUTY Brands product testing program will result in a $500 charge per occurrence. o All purchase orders are subject to random testing after they have arrived at the DC. o If random test results differ from original testing report on file, the factory will be responsible for all testing fees and all additional fees incurred at FULLBEAUTY Brands’ third party correction facility plus a $500 penalty for non-compliance will be assessed.

GENERAL CONFORMITY CERTIFICATE (GCC) • Merchandise which requires a General Conformity Certificate (GCC): o Wearing apparel required as well as exempt by Code of Federal Regulation (CFR) to be tested under 16 CFR 1610, FFA (Federal Flammability Act). • Requirements of the General Conformity Certificate (GCC): o The GCC must uniquely identify the product and the applicable CPSC Product Safety Regulation Test(s).

FULLBEAUTY Brands www.fbbrands.com 9 o A GCC can only be issued on items with PASSING lab reports or items considered exempt. o If a shipment contains multiple products, a GCC must be available for each item number. o The date, name and full address of the factory in which the product was manufactured, the importer and lab in which the product was tested as well as the contact of the individual maintaining the records. o GCC’s and test records should be retained and available upon request for 3 years from date of certification by the manufacturer and the importer. o Language written on GCC and test report must be in English. o Failure to abide by the GCC requirements will subject shipments to refusal of admission into the country and potential destruction of the merchandise. o The electronic GCC must have a means to verify the date of its creation or last modification. • GCC Process: o FULLBEAUTY Brands requests the factory to complete the GCC form and forward to the Product Testing Team . o The GCC Template may be found on the www.b2b.fbbrands.com Passcode: 1StopPlus o A valid lab test (performed within 1 year of the FCR date) must be on file in order for FULLBEAUTY Brands to approve a GCC. o The initial GCC may be reused by updating manufacturing date for repeat orders where there is no change in product or manufacturer/factory. o A new GCC will be issued with each applicable new passing lab test. o It is the sourcing office/agent and supplier’s responsibility to ensure the accuracy of all information including the factory name and address. o FULLBEAUTY Brands requires the following process for documentation in which the GCC is required: One copy of the Initial or New GCC to be submitted to FULLBEAUTY Brands’ Product Testing Team for approval prior to shipment departure (FCR date). Updated GCC’s for repeat orders do not need to be re-submitted to FULLBEAUTY Brands’ Product Testing Team for each shipment. One copy of the GCC must be submitted with the factory’s import shipping documents for Every Shipment.

CALIFORNIA PROPOSITION 65 • Lab Testing Requirements o Lab testing must be conducted on apparel & accessory items as follows: Apparel - constructed with/composed of any beads, sequins, plastic & metal trim components (i.e.; buttons, D-rings, toggles, etc.), belts, PU, PVC & other plastics materials. Apparel with detachable belts constructed with/composed of PU, PVC or other plastics & metal components. Apparel with non-detachable or detachable jewelry. Accessories - all jewelry, belts, shoes, handbags (including totes & fashion bags) Apparel products consisting of finishes, i.e.: wrinkle resistance, stain resistance, crinkle/pre- crinkle finishes NOTE: o Please refer to the FULLBEAUTY Brands Restricted Substance List (RSL) located on the www.b2b.fbbrands.com site and the testing protocols for detailed information.

REGULATORY COMPLIANCE REQUIREMENTS

All FULLBEAUTY Brands vendors are expected to comply with all laws including but not limited to International, Federal and State Compliance Regulations. Vendors must also comply with Corporate Requirements and the Terms and Conditions of FULLBEAUTY Brands’ domestic or import Purchase Orders. FULLBEAUTY Brands does not represent that all applicable laws appear on its B2B site. It is the sole responsibility of the vendor to identify and comply with all applicable legal requirements.
