For more than 40 years, AHCH has been the trusted source for in-home care and hospice services for the communities of central, western and mid-coast Maine. Throughout your lifetime, as your health care needs change, AHCH is here to help. Each year, our Annual Report is an opportunity to highlight noteworthy accomplishments, communicate the ways in which AHCH has left its imprint on our communities, and acknowledge the philanthropic support from more than 2,000 community donors, event sponsors and foundation grantors. Here are some of our highlights from 2009: • We celebrated the successes of our home care clinicians in achieving outstanding patient care outcomes. Our “best practice” focus has put us ahead of national standards in five key areas that affect mobility, wound healing and the ability of patients to manage their care needs at home following our services. The documented effectiveness of our home care services reducing the need for re-hospitalization of patients has opened the door to new collaboration with our community hospitals and physicians. By establishing a specific care coordination process for patients dealing with heart conditions and those undergoing joint replacement surgeries, we are able to assure a safer transition home after a hospital stay. These successes have application to other health conditions and we will be exploring these in the coming year. • Our expansion of services to the mid-coast and Augusta areas was well received by referring physicians and long-term care facilities. With recently added staff capacity, we look forward to increased utilization of our services. • Our fully integrated electronic medical record system took an important step forward with significant technology updates and systems planning projects. Upgraded laptop computers with wireless data connections for our field- based nursing, therapy, social work and chaplain staff has made patient care documentation easier and more efficient. The implementation of a new telephone feature gives us the ability to electronically document patient care notes phoned in by supportive care staff and home care and hospice aides. In addition, our volunteer program can now utilize our electronic medical record system to make patient/volunteer matches and track the support services volunteers are providing to patients and families. • We completed a major philanthropy assessment in June 2009. Feedback from a cross section of Board, community and employees resulted in a work plan to guide future engagement with community donors and foundations around our funding needs for mission-driven services and strategic program investments. The recent passage of national health care reform legislation will challenge us in the delivery and funding of services. Sustaining our focus on continuous quality improvement and technology innovations will be critical.
