The Community Care Program (CCP) of Illinois Department on Aging offers a statewide network of services to persons age 60 and over. These services offer eligible adults of Illinois the opportunity to remain in their own homes and their own community. Adult Daycare Service is part of CCP. Xilin Adult Daycare Services (Xilin ADS) is certified by CCP program, and the services include monitoring, medication supervision, personal care, therapeutic activities and nutritious snacks and lunches.

In 2005, Xilin's Adult Daycare Service program established its first site in Naperville, co- existing with four other Adult Daycare Service programs in the DuPage County area. The program started with serving 4 seniors, 3 days a week and now operates average 65 clients and 5 days a week. The clients who attended Xilin's ADS are mostly from the DuPage County area but also include other counties, such as the Will, Kane, and Cook Counties. The diverse client base includes the following ethnicities: Chinese, Koreans, and Indians. In order to effectively provide the seniors' various necessities, Xilin Association began to consider opening a second site in early 2010. Now, the Schaumburg Adult Daycare Center, located at 1635 Wise Rd, is in the process of acquiring real estate and certification. 1. Business Description 1.1 Community Care Program (CCP) Overview and Adult Daycare Service The Community Care Program, administered by the Illinois Department on Aging (IDOA), offers a statewide network of services to persons age 60 and over. These services offer eligible adults of Illinois the opportunity to remain in their own homes as an alternative to inappropriate or premature institutionalization in a facility. Illinois certifies providers for services under the Community Care Program. There are three major programs under the CCP. They are: Adult Day Service, Emergency Home Response Service, and In-Home Service.

Adult Day Service is designed specifically for older people who want to remain in the community but cannot be home alone during the day due to a physical, social, and/or mental impairment. Adult day service also provides respite for family , especially those who are employed outside the home, and socialization for isolated adults. Services offered in adult day service centers include health monitoring, medication supervision, personal care and recreational/therapeutic activities. Nutritious lunches and snacks are served and special diets are provided. Xilin Association's Adult Daycare Program is certified by the CCP program of IDOA.

1.2 About Xilin Association Xilin Asian Community Center at 1163 E. Ogden Ave. Suite 301, Naperville, IL, is the headquarters of Xilin Association, a 501 C(3) non-profit organization. Xilin's mission is to promote a healthy transition of new immigrant families into mainstream American society through social and cultural services by enhancing the quality of life through provision of educational, cultural, social, and civic services to the Asian communities.

In 1989, Xilin Association was founded by several parents at the campus of UIC. The goal of the program was to teach Chinese language, culture, and performing arts to Chinese American children, and to provide social, legal, financial, and educational workshop to immigrant families. Now there are eight Xilin weekend Chinese school programs in UIC, Hoffman Estates, Lake Zurich, Grace Lake, Lake Forests, Evanston, Naperville, and Hyde Park. It enrolls over 2,000 students during the school year. In 1999, Xilin began to notice the presence of elderly citizens accompanying parents to the weekend schools. As a result, Xilin Association began providing services for senior citizens. Funded by DuPage County Human Service Grant and DuPage Community Foundation and partnered with College of DuPage, the first Xilin senior service center was established in DuPage County. Funded by Retirement Research Foundation and partnered with Xilin Northwest Chinese School in Palatine at that time, the second senior service center was established in the Suburban Cook County area in 2001. Funded by Age Options, Xilin started its Senior Congregate Meal program in two Chinese restaurants in 2002, one in Arlington Heights and one in Streamwood. At the same time, Xilin started its case management and caregiver support for Asian communities in Palatine, Wheeling, Hoffman Estates, Nile, and Schaumberg Townships. In 2003, Xilin Association built its 10,000 square foot Xilin Asian Community Center in Naperville. Now center programs include the Adult Daycare Service and the after-school program. It hosts Xilin Art Center's classes in its dance studio, and is the headquarters of community special culture events (Lantern Festival and Asian Pacific American Heritage Festival) such as social services, health education, and screening programs. Xilin also offers weekend classes. Based on this center location, Xilin Association works with the DuPage County Department of Public Health, DuPage County Senior Services, Illinois Department on Aging, Age Options, DuPage County Human Service Department, School District 203, School District 204, Cook County Health Department, etc. Xilin's role now is a bridge between public services and the Asian population, and is also a resource center for many service providers (hospitals, County Sheriffs office, City or County departments, public/private schools) in terms of their Asian clients, Asian culture events, etc. Xilin Asian Community Center currently employs 30 ethnically diverse staff members of Chinese, Korean, Indian, Filipino, Thai, Iranian, African American, and Caucasian races. Its Adult Daycare Service serves Indian, Korean and Chinese food for lunch every day. The funding sources of center operations are privately paid (children's programs) tuition, government, and foundation funding.

1.3 Xilin's Adult Daycare Service Program Xilin's Adult Daycare Service program (Xilin ADS) was certified in 2005. The first site of Xilin's ADS was in Naperville, located at Xilin Asian Community Center, along with Xilin's Case Management Social Services and Community Health program.

Xilin ADS targets Asian seniors who have limited English-speaking ability and low-income status. Starting with 4 clients and 3 days a week of operation, Xilin ADS now serves a daily average of 65 seniors and is open 5 days a week. Those Asian seniors need not only attention to their physical and mental situations throughout the day, but also their cultural and language barriers. Due to these barriers, they may not be able to drive, watch TV, make emergency phone calls.. .etc. They are isolated at home. These obstacles further deteriorate their mental and physical condition.

In order to serve clients with limited English, Xilin hires and trains bilingual staff members who are knowledgeable in a variety of cultures and languages. Xilin current clients in ADS are Chinese, Korean, and Indian. They are not only from DuPage County, but also the surrounding counties of Cook, Will, and Kane. The most distant site Xilin ADS transportation picks up is Green Castle Senior Housing in Barrington, with a one-way trip being about 35 miles. Seniors attending Xilin ADS enjoy the culturally sensitive environment and personal care, and have become much happier and healthier. The case worker from DuPage County has told us that she has seen a significant change in her clients' situations after they have participated in Xilin's ADS. 1.4 Purchasing Facility and Modification to the Building Responding to the need from numerous senior residents and a year's worth of research and discussion, Xilin Association's board has approved the opening of another Xilin ADS in the Schaumburg area. The plan is to purchase an 8,000 square feet office space located at 1635 Wise Rd, Schaumburg. After inspection of the facility, Xilin found that no major modifications were necessary, except the installation of a fence and gate in order to close the courtyard (to prevent clients from wandering away). Internally, besides installing a sprinkler system, only partition and furnishing will be necessary. 1.5 Goals and Objectives The goal is to provide personal care to fragile seniors and to host a daily maximum of 100 clients (40 square feet per client requirement by IDOA regulation). The objectives are to: • Create a culturally sensitive adult daycare environment • Transport clients safely • Provide therapeutic activities and personal care • Provide nutritionally balanced ethnic lunches and snacks • Monitor daily medicine intake by clients • Work collectively with caregivers, case managers of CCU, and physicians 1.6 Critical Success Factors The critical factor to overall success is financial success. This is decided by daily attendance rate. Adult Daycare Service only takes seniors in a certain health situations. The eligible attendees have to be mobile and must be able to sit for at least a couple hours (otherwise they belong in a ). But in this case, if a senior is mobile, he or she can most likely drive, isolating themselves in an ADS setting is unnecessary. So this resulted most clients are enrolled in Adult Daycare program are dementia clients.

Asian seniors (or the seniors with limited English) have a totally different attitude for that situation. Because those seniors can neither drive and nor understand English, they are completely isolated at home. They are yearning for socialization. Due to the fact that Xilin's Adult Daycare Service helps low-income Asian seniors out of their homes, motivates them to and socialize with other seniors. The Adult Daycare Service that targets seniors who speak little or no English has higher daily attendance rates. According to the experience of Xilin's ADS in Naperville, and the high percentage of the Asian population in the Schaumburg area, Schaumberg ADS will not only be financially viable, but also be able to accept more non-Asian clients as well.

1.7 Xilin Association's Governing Body Xilin Association is a 501 C (3) non-profit organization. The governing Body of the organization is our Board of Directors. 1.8 Xilin Association's Operating Chart Please see attachment. 1.9 Exit Strategy The Exit plan is to inform the CCU (Kenneth Young and Catholic Charity Arlington Heights) of the closing, and then sells the property. Many seniors will lose their benefits of the services.

2. Services 2.1 Adult Daycare Service Description Adult Day Service is designed specifically for older people who want to remain in the community but cannot be home alone during the day due to a physical, social, and/or mental impairment. Adult day service provides socialization for isolated seniors, and also provides respite for . The services offered in adult day service center include health monitoring, medication supervision, personal care and recreational/therapeutic activities. Nutritious snacks and lunches are served and special diets are provided. 2.2 Conducting Personal Care Partner With CCU, Caregiver and Physician For each client, Xilin will work with the client's caseworker from CCU (Kenneth Young, or Catholic Charity Arlington Heights, and or Aging Care Connections) and the client's giver and physician as a team. Detailed below is an outline of the process.

Throughout the course of the day, our staffs will kindly and diligently attend to each senior's wants and needs while constantly watching out for each safety. Should a situation arise, Xilin's staff will consult the senior's physician, report the incident(s) to the senior's caseworker, and also inform the senior's caregiver. In the case of an emergency, emergency services will be called immediately. Once the situation is resolved, follow up calls to all the above will be made. Caseworkers will also work with physicians to determine whether the senior should be admitted into a hospital or nursing home. This four-party team will guarantee the well being of the client.

3. Absence of Adult Daycare Service in the Area 3.1 Adult Daycare in the area close in 2010 Last year, the Northwest Community Hospital Healthy Aging / Senior Services that served the Barrington, Elk Grove, Hanover, Palatine, Schaumburg, and Wheeling Townships closed. That left Adult Daycare Service absent in those townships. The Lutheran Home for the Aged provides housing as its main service and runs a small scale Adult Daycare Service, but it is quite far from the Schaumburg area. 3.2 Adult Daycare Services targeted to Asian Elderly in Chicago Metropolitan Area There are several Adult Daycare Service programs targeting the Asian population in the Chicago metropolitan area. They all have high daily attendance rates. According to the Limited English Elderly survey in 2010, all of these agencies have more than 40 clients daily. However, none of them are in the Schaumburg area. 3.3 A New Service Model of Schaumburg Adult Daycare Center Schaumburg is a very diverse town. According to the Census Bureau 2005-2009 estimation, Asians are 16.9% of the total population, the highest percentage after the Caucasian population, which consists of 74% of the total. There are many more Asians than the Hispanic and Latino population, which consists of 7.6% of the total population. Due to the high Asian population and the general absence of Adult Daycare Services, Xilin's Schaumburg Center will take a mixed service model. This model will not only follow the service model in Naperville (mainly Asian clients) but also will accept non- Asian clients. This model is unnecessary in Naperville because of other ADS exist in the Naperville and DuPage County. This model will be covering the absent service for the general populace and will also work with culturally sensitive seniors.

4. Marketing and Outreach Plan 4.1 Outreach to Asian Community Elderly Funded by Age Options, Xilin Title III B (senior case management and social service programs) and Title III C I (senior congregate nutrition program) in the northwest Cook County has been developing since 2003. Xilin currently serves over 15,000 senior meals in two Chinese restaurants a year, one in Arlington Heights and one in Streamwood. Xilin also helps over 700 seniors apply for public benefits. Xilin's community health program reaches out to over 300 Indian, Korean, and Chinese seniors for educational workshops covering topics such as hepatitis B, osteoporosis, and breast cancer. It also arranges screenings for them. Xilin partners with Cook County Health Department, Asian Human Service Clinic, and St. Francis Hospital to set up 150 women for mammogram screenings. Xilin constantly reaches out to Indian temples, Korean and Chinese churches, and weekend Chinese Schools. Xilin advertises these social service programs in ethnic newspapers and the event partners (temples, churches.) advertise Xilin's services when they host joint events. These existing outreach channels, along with word of mouth, will greatly promote the newly established Schaumburg Adult Daycare Center. 4.2 General Marketing Plan Xilin will be working mainly with Kenneth Young and Catholic Charity of Arlington Heights for their referrals. Xilin will also post brochures and flyers in senior gathering places.

5. Program Development 5.1 Development of the program Looking back the developmental stage of Xilin's ADS in Naperville, we anticipate that the program will start with three days a week (most clients are approved for 1-3 days/week of service) of operation. We currently have 15 seniors (approved for 1-3 days of service) from Cook County going to the Naperville ADS. They will be transferred to the Schaumburg Center once it opens. We will start the outreach effort in January so it will have more clients approved by the time it opens (it takes 30 days to finish assessment cycle). More service dates will be added according to the clients' needs. In Naperville, we proceeded through milestones of 3 days, 4 days and now 5 days of service weekly. 5.2 Development timeline Xilin's Schaumburg Center is planning to open in February or March, depending on the financial approval, city permits, and ADS certificate. Xilin is planning to apply for a non-profit real estate tax waiver after the closing of the property.

6. Operations 7.1 Daily Activities See attached Daily Activity schedule sample 7.2 Transportations Xilin has been provided with a 15 passenger van by Age Options to be used for transportation of senior citizens, but will acquire more vans if necessary. Xilin will also work with the township senior transportation program for seniors in Schaumburg. Xilin will apply for an ADA approved van (with wheel chair lifting equipment) from IDOT in 2011. Xilin has already received one from IDOT in 2010 for Xilin's ADS in Naperville. Xilin anticipates purchasing a maximum of 3 additional vehicles. There are 35 spaces in the parking lot, which will provide ample room for parking of the vans as well as employee parking.

From the clients' side, CCU will decide if the individual will receive a ride according to each client's situation. If CCU approves transportation, the client will be approved for 2 units of transportation service a day (a ride in the morning and one in the afternoon). Most clients (anticipated 90%) will not be able to drive themselves for many reasons, so Xilin will provide transportation for them. Very few (around 10% will be transported by their caregivers or by driving on their own. 7.3 Food Currently, Xilin's Naperville ADS provides Chinese, Korean, and Indian lunches and two snacks every day. Much work is required for its flawless execution. For the Schaumburg site, we will arrange different types of food to start with. In order to determine how to make lunch arrangements, the program needs the demographics of the enrolled clients. The ethnic lunch is a big part of the culturally sensitive environment and much work is put into making it run smoothly. 7.4 Emergency Xilin will be working with the Schaumburg police department and Alexian Brother's Hospital. Xilin has worked cooperatively with emergency providers at Naperville Police Department and Edward Hospital for the case in may cases. Xilin's care workers will be licensed for CPR, and a nurse will be on site for 3 hours aday (IDOA regulation). Fire drills will be periodically conducted. 7.5 Book Keeping Xilin has a centralized accounting and control system. The off-site controller is paying bills, issuing salaries, and monitoring spending. There will be a bookkeeper in the Schaumburg Center. The controller issues petty cash to the bookkeeper and the bookkeeper reports back to the controller at least once per month. Most of the revenue will be coming from state reimbursement. A small portion is privately paid. The state reimbursement will be electronically sent to Xilin's account, and the privately paid will be deposited to a separate Schaumburg Center account by the bookkeeper. 7.6 Inventory and Supplies

The Schaumburg Center will need to purchase food, and activity and office supplies. It will keep its inventory. A weekly purchase request will be send to the Building Management Department, and the supplies that are bought and inventoried will be kept in the center. 8. Financials 8.1 Start-up Funds Total start-up funds required total at $200,000 ($50,000 down payment and closing cost of purchase, $100,000 remodeling, and $50,000 for the first three months of operation). $15,000 will be grant funding from the Chicago Community Trust, and $185,000 will be from Xilin's own reserve fund.

8.2 Lender — Illinois Financial Fund The purchase price of building is $485,000, 10% down payment being $48,500. Xilin will borrow $436,500. The lender is the Illinois Facility Fund, a financial institution specializing in lending to non-profit capital projects. More information can be found at http://www.iff.org . POLICY: The staff members of the Center shall attend continuing education programs as approved by the Director. All employees shall have a minimum of 13-16 hours of continuing education each year.

PURPOSE: , Since adult day care is changing at such a rapid pace, it is imperative that all staff keep current in the relevant aspects of this field.

SPECIAL INSTRUCTIONS:

1. Staff members are expected to keep abreast of current practices related to the care of elderly and the operations of adult day care in their respective professions. 2. Staff members are expected to attend at least one workshop annually which focuses on issues associated with adult day care for elderly. 3. Expenses for approved workshops and staff education will be paid by Xilin and staff will be compensated at regular salary for time spent in training. 4. All staff members shall be responsible for keeping their continuing education records up-to-date.

SUBJECT: Basic Life Support Classes EFFECTIVE: AUGUST, 2005

POLICY: Basic Life Support (CPR) certification will be required of all Xilin ADS staff.

PURPOSE: To maintain standards for quality client care.

SPECIAL INSTRUCTIONS:

1. Documentation of training must be written on continuing education records kept at the Center. 2. Classes must meet the American Heart Association requirements.

POLICY: Staff evaluations will be performed in accordance with Xilin policies.

PRUPOSE: To assist staff members develop, strengthen and expand their skills and abilities.

SPECIAL INSTRUCTIONS:

1. All evaluations will be based on Xilin's philosophy and method of performance assessment. 2. The ADS Administrator will annually provide input regarding job performance to Executive director. 3. Interim reviews may be conducted as necessary should job performance not meet minimum standards.
