Current 2017-2022 Alliance to Improve Equity by Advancing Patient-Centered Cancer Care for Underserved Populations in Northeastern Massachusetts Merck Foundation, Alliance for Patient-Centered Care (Percac-Lima) Co-investigator Goals of project: To develop and evaluate patient navigation for patients from community health centers with a new cancer diagnosis and improve the integration of psychosocial care. 2017-2019 Proactive Psychiatry Consultation for Individuals with Severe Mental Illness and Cancer: A Collaborative Care Intervention Harvard Risk Management Foundation (CRICO) Principal Investigator ($193,072) Goals of project: 1. To establish a population-based system to identify patients with serious mental illness at cancer diagnosis. 2. To assess the impact of proactive psychiatry consultation vs. enhanced usual care on cancer care for patients with serious mental 5 illness including disruptions in cancer care, patient and caregiver-reported outcomes and healthcare utilization

2017-2019 Bridging the Divide: Engaging Patients and Stakeholders to Promote Equity in Cancer Care for Patients with Serious Mental Illness Patient Centered Outcome Research Institute Engagement Award Principal Investigator ($250,000) Goals of project: 1. To develop a community network to engage patients, caregivers, and stakeholders with the shared purpose of reducing disparities in cancer mortality for patients with SMI 2. 2015- present Director and Founder, Collaborative Care and Community Engagement Program Center for Psychiatric Oncology and Behavioral Sciences, MGH Cancer Center Created clinical and research initiative dedicated to achieving equity in cancer care and outcomes for patients with serious mental illness with the following aims: 1) To increase access to clinical care and develop innovative models of care delivery for an underserved population, 2) To conduct research and decrease barriers to clinical trial participation for patients with serious mental illness, 3) To educate clinicians across disciplines and levels of training (oncology, psychiatry, social work, nursing) and 4) To forge community partnerships to increase awareness of cancer health disparities and create mechanisms to disseminate new models of care delivery I developed a model of integrated psychiatry and oncology care targeted to patients with serious mental illness and cancer which incorporates proactive psychiatry consultation and case management. This model has been highlighted in an editorial in the New England Journal of Medicine, presented at national conferences, grand rounds, and shared via national webinars. In 2017, I organized a symposium attended by 150 participants representing 30 institutions. We completed the first pilot study targeting this population and have received funding for a randomized trial of the intervention. Our initiative has been supported by seed funding from the MGH Cancer Center, MGH Department of Psychiatry, and the Massachusetts Department of Mental Health.

2016- present Consultant, Massachusetts Department of Mental Health Director, Second Opinion Service: Increasing Access to Collaborative Care 13 I developed a Second Opinion Service in partnership with the leadership of the MGH Cancer Center with the goal of increasing access to cancer care for individuals with serious mental illness in the community. Mental health clinicians and community members can reach out to our psychiatry team who will connect with an oncologist from the disease center and provide guidance on the best next steps. Approaches include teleconsultation, joint psychiatry and oncology visits, and community-based visits. The Massachusetts Department of Mental Health has recognized the promise of this model by contracting our team as consultants for patients with serious mental illness and cancer.

2017-present Founder and Director, BRIDGE: Inpatient collaborative care for serious mental illness and cancer We responded to feedback from oncology clinicians, patients, and families regarding challenges transitioning from the inpatient setting to the community (e.g. rehab, home, or community mental health setting). We piloted an approach where patients are connected to a social work case manager and psychiatrist during the inpatient stay with the goal of building trust and continuity while helping patients be cared for in the community, aiming to decrease length of stay and prevent unnecessary acute care.

Introduction I am a psychiatrist specializing in the care of adults with cancer and serious mental illness (SMI), particularly schizophrenia. After completing internal medicine internship and adult psychiatry residency at Massachusetts General Hospital and McLean Hospital in 2012, I joined the faculty at MGH and HMS, where I provide clinical care, supervise and teach trainees, and conduct research on disparities in cancer care and mortality for patients with SMI. 16

Area of Excellence – Clinical Expertise and Innovation My clinical interests and expertise center on cancer and SMI. I receive referrals from colleagues locally and regionally, with a focus on patients with SMI and cancer, clozapine, and challenges accessing care. In 2015, I founded the Collaborative Care and Community Engagement Program, a clinical and research initiative dedicated to achieving equity in cancer care for patients with SMI. I developed an innovative model of integrated mental health and cancer care delivery, successfully piloted this model, and received funding for the first randomized trial of an intervention for patients with SMI at cancer diagnosis. Our work has been highlighted in the New England Journal of Medicine and led to seed funding from the MGH Department of Psychiatry and Cancer Center. I have experience integrating mental health and cancer care in academic and community settings and consult to the Department of Mental Health. I have chaired symposia nationally and internationally and given grand rounds on cancer care and SMI. Teaching I have taught and supervised medical students, residents, and fellows since joining the HMS faculty. I teach a schizophrenia seminar monthly for medical students, supervise a social worker and nurse practitioner, and teach psycho-oncology to oncology fellows and psychiatry residents. Each week, I supervise a PGY-2 resident in community psychiatry to increase expertise with clozapine and public mental health and provide psychotherapy supervision to a PGY-3 resident. I also teach my research team including a research coordinator, social worker, and interns. I have mentored my research coordinators as they have produced independent research and transitioned to medical school.

Supporting Activity – Investigation Since medical school, I have conducted clinical research on psycho-oncology. Since 2012, I have focused my research on disparities in cancer care for people with SMI. I have received funding from the HMS Dupont-Warren Fellowship, the American Cancer Society, the Harvard Risk Management Foundation, and the Department of Mental Health. We are currently conducting the first randomized trial of an intervention targeting patients with SMI at cancer diagnosis. Additionally, I received a Patient-Centered Outcomes Research Institute Engagement Award to create a cancer and mental health collaborative to build capacity for patient-centered research at the intersection of cancer and mental health. I was funded by the NIH/NCI Program in Cancer Outcomes Research Training to complete a master’s degree at the Harvard School of Public Health to expand my knowledge of quantitative research methods. I recently received an NCI-supported Career Development K08 Award to support conducting the first randomized trial for people with serious mental illness at the time of cancer diagnosis. My research has been recognized by the NIH Loan Repayment Award, the MGH Cancer Center’s One Hundred Award, and the Academy of Psychosomatic Medicine. I have presented my research locally, regionally, and nationally and published in high impact journals including the Journal of Clinical Oncology.

Supporting Activity – Education of Patients and Service to the Community For 6 years, I was the medical director of the Clozapine Program and continue to participate in the Public and Community Psychiatry steering committee and committees focused on behavioral health integration. I founded and lead an annual symposium that brings together 200 patients, caregivers, and advocates with clinicians and researchers committed to decreasing disparities in cancer care for people affected by mental illness. Additionally, with the support of the Trefler Foundation, I lead an initiative to increase access to cancer care for patients with SMI in the community and conduct educational workshops for community mental health staff.

Summary I have dedicated myself to delivering excellent clinical care while conducting health services research, teaching, and engaging in clinical innovation. Through these activities at the local, regional, and national levels, service to the community and involvement with academic and professional societies and the public
