You may also call Customer Service for updated provider information. Y0083_PharmDirectory2020_C Introduction This booklet provides a list of Medi-Pak Rx’s network pharmacies. To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and Medi-Pak Rx’s formulary. When this pharmacy directory refers to “we,” “us”, or “our,” it means Arkansas Blue Cross and Blue Shield. When it refers to “plan” or “our plan,” it means Medi-Pak Rx. We call the pharmacies on this list our “network pharmacies” because we have made arrangements with them to provide prescription drugs to Plan members. In most cases, your prescriptions are covered under Medi-Pak Rx only if they are filled at a network pharmacy or through our mail order pharmacy service. Once you go to one pharmacy, you are not required to continue going to the same pharmacy to fill your prescription but can switch to any other of our network pharmacies. We will fill prescriptions at non- network pharmacies under certain circumstances as described in your Evidence of Coverage. All network pharmacies may not be listed in this directory. Pharmacies may have been added or removed from the list after this directory was printed. This means the pharmacies listed here may no longer be in our network, or there may be newer pharmacies in our network that are not listed. This list is current as of October 1, 2019. For the most current list, please contact us. Our contact information appears on the front and back cover pages. You can go to all the pharmacies on this list, but your costs for some drugs may be less at pharmacies in this list that offer preferred cost-sharing. We have marked these pharmacies with “SAVE” to distinguish them from other pharmacies in our network that offer standard cost-sharing. You can get prescription drugs shipped to your home through our network mail order delivery program. For more information, please contact us or see the mail order section of this pharmacy directory. If you have questions about any of the above, please see the first and last cover pages of this directory for information on how to contact Medi-Pak Rx. Pharmacies are listed alphabetically by city and by type of pharmacy. These features are identified on each page with the following key: • SAVE = Preferred cost-sharing. Drugs from these pharmacies may cost less than other pharmacies in our network. • 90 = 90-day supply. These are retail pharmacies that offer an extended (90-day) supply of medications. • 90+SAVE= 90-day supply with Preferred Cost Sharing.     Robert J. DeSalvo Papers Business Combinations in the Cosmetic and Pharmaceutical Industries 1944 - 1990 Collection #107 Abstract Robert J. DeSalvo’s research focused on business combinations (acquisitions, mergers, and joint ventures in the cosmetic and pharmaceutical industries. This topic was the basis for his master’s thesis in pharmacy administration at the University of Pittsburgh and continued as a life-long interest. This collection consists of two series of notebooks that Dr. DeSalvo developed to record relevant business combinations. The first series records acquisitions, proposed acquisitions, mergers, and joint ventures for the period of 1944 –1990 in an alphabetical arrangement. The information on these entries is cumulative so that the history of an organization is collected in one place. The second series of notebooks is arranged in chronological blocks. The information is arranged alphabetically by the name of the acquirer. The name of the acquired (merged), type of combination (acquisition, proposed acquisition, joint venture) and the date is also provided. The information is cross-referenced between the two series so that the researcher can approach the information by the name of the parent company or chronologically. Dr. DeSalvo used this resource for many of his publications as well as his master’s thesis. A copy of these publications and his thesis make up the remainder of the collection. Donor Gift of Barbara DeSalvo, 2000 Biography Robert James DeSalvo was born on July 20, 1933 in Toledo, OH. He died on January 23, 1993 in Cincinnati, OH. DeSalvo graduated from high school in Toledo and attended pharmacy school at the University of Toledo where he received his B.S.
