Shaving can not only result in razor burn, ingrown hairs and razor bumps, but it can lead to increased sensitization and inflammation that results in premature aging. Unfortunately, as the average man’s beard grows two mm per day, there is ample opportunity to create an inflamed skin condition during shaving. As a matter of fact, if the average man starts shaving at age 13 and continues until he is 85 years old, and assuming he spends all of five minutes shaving each day, he will devote over six months of his life to just shaving his beard. As professional skin therapists, we may not be shaving our clients, but with the ever increasing number of men in skin treatment centers, salons and spas, we need to educate them about their specific skin care needs as it relates to shaving. Problems Associated with Shaving We know that the simple act of shaving imposes constant stress on the skin. Shaving is a form of physical exfoliation that can impact the health of the skin. Razor bumps, ingrown hairs, razor burn and inflammation are just some of the visible signs of trauma that the skin endures when a razor is used on the beard. Anyone who has ever shaved or waxed will no doubt be familiar with ingrown hairs and well aware of the pain, irritation and embarrassment they cause. As a therapist, I’ve seen many ingrown hairs and the most memorable ones are the ones that have been squeezed, picked, plucked, dug out and cut at by frustrated clients looking for a solution. More often than not, this leads to infection and potential scarring. No one wants to have ingrown hairs! They’re unsightly, sometimes painful and itchy causing much discomfort, sometimes so bad that the client gives up on waxing altogether. Why put a client through of all of this when solutions exist?!? I thought the best place to start this piece is to explain what an ingrown hair is and why they occur. To put it simply, an ingrown hair is a hair that does not emerge from the follicle but remains embedded in the skin, usually causing inflammation. Ingrown hairs can occur after hair removal when dead skin cells accumulate and form a papule as the skin heals. Hair growing in the area gets trapped under the papule and is prevented from exiting the skin, creating an ingrown hair. Irritating and unsightly ingrown hairs can occur on any part of the body however are most common under the arms, bikini line, legs, chest and on the face/ neck for men. Ingrown hairs and other problems associated with hair removal vary from person to person and can be more prominent depending on hair and skin types.

Pretty much anyone who shaves experiences this annoyance at some point, but people with thick or curly hair are especially prone. Black people tend to get more ingrown hairs than those of European or Asian descent, since their hair is often thicker and curlier, making the inward spiral and subsequent irritation more likely. The problem is that you need to go against the grain to get the closest shave, but that’s also the best way to make a hair begin growing inward. When a freshly-shaved hair-tip sits too close to the skin, it becomes more likely to catch and grow in a little curlicue inside the body rather than out of it. Once the hair starts growing inward, there’s not much you can do. The skin will become red and irritated, maybe even develop a little pustule, and eventually resolve itself. But in the meantime, you’re stuck with a gross bump on your skin. Luckily, you can take a few steps to prevent them from happening in the first place. Often referred to collo- quially as “chicken skin,” keratosis pilaris usually appears on the backs of the upper arms, the thighs, the buttocks, or the face as a rough patch of bumpy skin that does not itch. The bumps are hair fol- licles that are plugged by keratin, a protein found in skin cells. Often there is a coiled, ingrown hair inside the bump. Keratosis pilaris is not a serious condition; it requires no treatment, unless the patient is both- ered by the skin’s appearance. The use of topical moisturizers, salicylic acid, lactic acid, urea, vita- min D, or tretinoin may be of benefit, but the plugs usually reappear when treatment is discontin- ued. The condition has a connection to vitamin A deficiency, so supplementation with small amounts of vitamin A may help. Keratosis pilaris usually disappears eventually without treatment.     Pilonidal Cysts Have you ever had an ingrown hair that seemed to grow beneath the surface? Well, that is the same process that sometimes leads to a condition called a pilonidal (PIE-low-NI-dal) cyst. Pilonidal disease develops when a dead hair is pushed into tiny abrasions or scars in the skin. (In fact, the term pilonidal meaning "nest of hair" was coined in 1830.) As the hair grows into the skin, it forms a small one-ended tunnel beneath the surface. Germs can grow in this tunnel, causing it to become infected. If the opening to the skin becomes blocked, painful swelling occurs, much like a boil. Where does this happen? The typical location for a pilonidal cyst to form is the skin over the tailbone, just above the cleft of the buttocks. This embarrassing location is chosen not only because of the hair and bacteria there, but especially because of the friction and pressure on the area that can push hairs into the skin when the person is sitting. A pilonidal cyst does not turn to cancer, but may cause painful symptoms. Rarely, serious infection may result. Who gets pilonidal cysts? Pilonidal cysts tend to occur in young men since they usually have more body hair, although it can occur in anyone. In a population study of Minnesota college students, pilonidal cysts were found at routine physical exams in 1.1% of males and 0.11% of females. It usually occurs after puberty and is rarely seen after age 40. Incidence is highest in whites and less in Africans and Asians, perhaps due to differences in hair characteristics.
