    Preoperative Opioids Influence on Outcomes After Total Knee Arthroplasty by Craig Michael Goplen A thesis submitted in partial fulfillment of the requirements for the degree of Master of Science Department of Surgery University of Alberta © Craig Michael Goplen, 2019 Abstract Background: Up to 40% of patients are prescribed opioids prior to total knee arthroplasty (TKA) in the USA. These patients prescribed preoperative opioids have increased complications and worse outcomes after surgery. But, the impact of preoperative opioid use on patient-reported outcome (PRO) scores after TKA has not been studied extensively. To our knowledge, the number of patients prescribed opioids before and after TKA in Canada also has never been reported. The purpose of this thesis is to estimate the prevalence of opioid use before and after TKA in Alberta, Canada, and to determine the impact of preoperative opioid use on PRO scores 12-months after TKA. Methods: A systematic review evaluated the impact of preoperative opioid use on PRO pain and function scores after TKA. We then described three methods that can detect consistent opioid use with administrative health data. Applying this methodology to a cohort of patients that underwent primary, elective TKA between 2013 and 2015 in Alberta, Canada, we estimated the rate of opioid use before and after TKA, along with the dose, duration and most common opioid formulations dispensed. We also analyzed the relationship between preoperative opioid use and patient reported outcomes 12-months after TKA, adjusting for potentially confounding variables with multivariable linear regression. Potentially confounding variables included patient’s age, sex, comorbidities including depression and preoperative score.

Ramírez-Puerta Abstract The fascinating history of local anesthetics (LAs) began in South America with the herbal and traditional use of cocaine leaves by the indigenous peoples of Peru and Bolivia, the sacred plant of the Incas Erythroxylum coca. The use for anesthetic purposes dates back to 1884. Since then, the evolution of LAs has been closely related to research motivated by its efficacy and safety versus toxicity. According to their chemical structure, these drugs are classified into two main groups: esters and amino amides; however, there are three LAs with different characteristics: articaine, sameridine, and centbucridine. The pharmacological and toxic mode of action is primarily in the voltage-dependent sodium channels located in the cell membrane, which clinically produces analgesia, anesthesia, seizures, arrhythmias, and cardiac arrest. The quality of anesthesia and analgesia depends on the type of LA, dose, and application technique, while the deleterious effects are secondary to its plasma concentration. Nonanesthetic properties of LAs such as their antimicrobial, antineoplastic, antiarrhythmics, antitussive, and antiasthmatics effects have been described and are briefly reviewed. Its contents reflect, but do not present, WHO policies and activities and they embrace socioeconomic as well as technical matters. The objective is to bring issues that are of primary concern to drug regulators and pharmaceutical manufacturers to the attention of a wide audience of health professionals and policy-makers concerned with the rational use of drugs. In effect, the journal seeks to relate regulatory activity to therapeutic practice. It also aims to provide an open forum for debate. Bohm-Starke N, Hilliges M, Blomgren B, Falconer C, Rylander E Obstet Gynecol 2001 Dec;98(6):1067-74 OBJECTIVE: To evaluate vascular changes as a possible underlying cause of mucosal erythema in women with vulvar vestibulitis. METHODS: Laser Doppler perfusion imaging was used to map the superficial blood flow in the vestibular mucosa in 20 women with vestibulitis and in 21 healthy control subjects. A possible correlation between perfusion values and graded erythema (1-5) around the vaginal introitus was analyzed. Changes in microvascular density in the posterior part of the mucosa were investigated in sections from ten patients and ten controls by a computer-assisted image-processing program. Induced vasoconstriction of terminal arterioles in the same posterior area was also studied. RESULTS: Significant increases in perfusion values were registered in the posterior parts of the vestibular mucosa in patients compared with controls. The highest blood flow was registered in the posterior fourchette. The most pronounced erythema was also located in the posterior vestibule in the patients. However, there was no significant correlation between perfusion values and degree of erythema in the same individual.
