David W. Beall Memorial Scholarship: The David W. Beall Memorial Scholarship was established to memorialize one of Maconaquah’s most beloved teachers. The “Students of Beall” learned many lessons from Mr. Beall and not only in social studies and government. They learned to become patriotic, responsible citizens with good common sense. Eligibility Residents of Miami County and graduating Seniors of who attended the school for at least 4 years. Must be planning to attend a college, university or trade school in the State of Indiana and show a financial need. A grade point average of a C or above is required and applicant must not be in the top 10% of the graduating class. Those applying should also be actively involved in school, work and the community. Claude S. and Marie M. Berger Foundation, Inc. Scholarship: Marie was born in Akron Indiana and worked at the Akron and Rochester Telephone Company. Claude was born in the Gilead area, where he farmed until retiring in 1972. Their dedication to the education of students continues on through this scholarship. This scholarship is eligible for renewal. Eligibility Must be current graduates of North Miami High School or Tippecanoe Valley High School and have a “B” average or above. Applicants must also be deserving of financial assistance and show good character and citizenship. Additional Required Information  One letter of recommendation from an educator -  list three (3) references who are not family members. They must be as follows: o Educator – Please list their name, home phone and work phone. o Personal – Please list their names, home phone and work phone. o Professional – Please list their names, home phone and work phone.  Claude S. and Marie M. Lyman H. Bond Scholarship: Lyman was a leader determined to be successful. Leadership, a strong work ethic and his love of the game of football are pillars of this scholarship that bears his name. Eligibility Students must have been a member of the Peru High School football team or other athletic program, demonstrate a history of leadership in school, work, community groups and activities; two letters of recommendation Additional Required Information  Two letters of recommendation (One from a coach or teacher and one from any non-related adult)  Lyman H. Elaine Cultice Bright Memorial Scholarship: Elaine was a graduate of Peru High School and co-valedictorian of the class of 1971. She graduated with honors from Indiana State University and received a degree in accounting. Elaine was diagnosed with multiple sclerosis in 1987 and succumbed to the disease in 2004. Her parents have chosen to honor her memory by providing an annual scholarship to a Peru High School female graduate who has been a good student and has been actively involved in school, work, and community. Eligibility Female students who are residents of Miami County and graduates or past graduates of Peru High School. Must have attended for two years prior to graduating. Must be full time or part time students at an accredited Indiana college, university, trade or vocational school. Involved in school, work and community, display good citizenship and academic accomplishments. Student who are Lilly Endowment Community Scholars, 21st Century Scholars or those who have a scholarship that pays for their entire tuition are not eligible to apply for this scholarship. The scholarship is eligible for renewal for one year.

2017 Miami County Community Foundation Scholarship Application Page 3 The Class Of ’64 John McElheny Memorial Scholarship: This scholarship was created by the first class to graduate from Maconaquah High School and memorializes John McElheny, a beloved teacher known as “Mac”, who inspired and motivated their class to become the “First and Best”. Eligibility Graduating senior of Maconaquah High School who has attended Maconaquah High School for one full year before graduation. Must also be pursuing a degree in Elementary or Secondary education; have at least a 2.5 GPA based upon a 4.0 scale; graduating in the top 50% of class; active and involved in school, work, and community and display good citizenship in school, work, and community. Additional Required Information  Essay Question D. Marilyn Coppernoll Memorial Scholarship: Graduating seniors of Peru, Maconaquah or North Miami High Schools. Marilyn was a former reporter for the Peru Tribune and community volunteer. She was a widely published free lance writer and AP correspondent who contributed to many publications and authored the Pictorial History of Miami County. Eligibility For students enrolled in the study of writing or publishing. Must have a “C” average or above to apply. Additional Required Information  Letter of recommendation  Marilyn Coppernoll Essay Question. James R. and Bette J. Davis Scholarship: James and Bette Davis were born and raised in Miami County and were firm advocates of higher education. Eligibility Must be a resident of Miami County and pursuing a degree in any form of health care and be accepted into and attend a college, university or vocational school in the state of Indiana. Must be active in school, work and the community; must have a 3.0 or higher GPA and attended high school in Miami County for at least three years prior to graduation

LaVonne M. Eddy Nursing Scholarship: Thomas Eddy established this scholarship in honor of his wife LaVonne in 2006 to benefit students planning a career in nursing. Eligibility Must be residents of Miami County, who have attended North Miami, Maconaquah, Peru, or Oak Hill High School for at least 2 years prior to graduation, and pursuing a 4 year nursing degree at an accredited college or university. Must have a C or higher GPA and be in the top, 50% of their graduating class. Involved in school, work and community. Additional Required Information  Essay Question C. Eugene K. and Martha E. Fike Memorial Scholarship: Created by Eugene and Martha Fike to benefit Peru High School Students. Eligibility Graduates of Peru High School and likely to succeed in college in the curriculum or discipline of their choice.

Friend Family Scholarship: The Friend family has been in the Macy, Indiana area for generations and they are making a commitment to the future of the community by supporting students who have attended North Miami High School. Eligibility Must be resident of Miami County, and graduate of North Miami High School. Must be pursuing a degree in education, nursing, agriculture, foreign language, pharmacy, business or medicine. Must have a minimum 2.25 GPA. Must demonstrate meaningful involvement in his/her school and or community. Additional Required Information  Essay Question C.

James U. and Betsy Guthrie Health Scholarship: Created by the children of James and Betsy, to honor their parents. Jim devoted himself to the practice of general surgery, moving to Peru in 1959. Eligibility Residents of Miami County and pursuing a degree in any form of health care; accepted into and attend a college, university or vocational school in the state of Indiana. Active in school, work and their community. Must have a minimum 3.0 GPA and attend high school in Miami County for at least 3 years prior to graduation.

2017 Miami County Community Foundation Scholarship Application Page 4 Tony Hare Photojournalism Scholarship: Tony was a member of the National Press Photographer's Assoc. and the Indiana Press Photographer's Association. He was Outstanding Photographer of the Year and President of the National Press Photographer's Student Association. Eligibility Graduating senior or past graduate from any Cass, Miami or White County High School, studying photojournalism at an Indiana college, university or trade school. Seniors in the top 5 of their class are not eligible. Must maintain a C or higher GPA. Additional Required Information  Essay Question C. Note* Must be a 500 word essay for the Tony Hare Photojournalism Scholarship essay. Ray E. and Marie Rupp Hoffman Memorial Scholarship Fund Ray and Marie were both born and raised in Miami County. Ray, on a farm south of Peru and Marie in Peru. They both graduated from Peru High School. Ray graduated from Purdue and Marie from IU. This scholarship is to celebrate and honor their lives. Eligibility Seniors or past graduates from Maconaquah or Peru High School who are residents of Miami County and pursuing a degree in a technical or business field. The applicant must display good academic accomplishments and deserve financial assistance. Additional Required Information  Essay Question A  FAFSA’s Student Aid Report (SAR). EFC (Expected Family Contribution) number must be included. Ryan McCarthy Holmes Scholarship: Ryan graduated from Peru High School and was a teacher and coach at Triton Central, Westfield High School and Center Grove High School. Students of all the schools listed including Peru High School are eligible to apply for this scholarship. Eligibility Graduating seniors with a GPA of 2.8 or higher; accepted to a college, university, trade or vocational school. Financial aid will be important in the selection of recipients; member of a church and actively involved; community service; may hold a job during high school. Additional Required Information  Ryan McCarthy Holmes Scholarship Essay Question.  Two (2) letters of recommendation. o One letter from a staff member of your High School. o One letter from your High School Guidance Counselor  FAFSA’s Student Aid Report (SAR). EFC (Expected Family Contribution) number must be included. Hoppes Memorial Fund Founded in memory of Marion Hoppes by his wife Rhea Hoppes. Rhea was the speech team coach for Maconaquah High School. This scholarship is to award scholarships to Maconaquah HS seniors who participated on the speech team. Eligibility Maconaquah High School Seniors who participated on the Maconaquah speech team. Applicants must have a GPA of a 3.0 (4.0 scale) or above and must submit a letter of recommendation from a Maconaquah staff member. Preference given to students who have received awards for their participation on the speech team and/or number of years they participated. Additional Required Information  Letter of recommendation from a Maconaquah staff member  Hoppes Memorial Fund Essay Question Randy Hughes Memorial Scholarship: Awarded to the Valedictorian of North Miami High School. There is no application.

J & J Scholarship: Established by anonymous donors to help and assist students in obtaining a post-high school education. Eligibility For those students who received the J & J Character Award at Blair Pointe Elementary School in Peru, Indiana and the Joseph Mauck Character Award from York Elementary School in Middlebury, Indiana. Additional Required Information  Please contact the NICF Scholarship Coordinator if you are eligible for this scholarship.

Glen Jones Honor Scholarship: Honors Glen Jones, North Miami High School teacher and mentor. Eligibility Graduating senior of North Miami High School, active in NMHS FFA for at least 3 years of their high school, including their senior year. Applicants must submit FFA Supervised Agricultural Experience (SAE) records, brought to date at the time of submission of this scholarship during their senior year. Additional Required Information  Essay Question C  Letter of Recommendation with verification of SAE records from the current FFA/Ag teacher Pat Keady Memorial Scholarship: The scholarship was founded in memory of former Purdue University coach, Gene Keady’s wife, Pat. Eligibility Graduating senior of Maconaquah High School, attending any Indiana college. Two scholarships given, one each to a male and female student. Consideration to students with good athletic accomplishments and those attending Purdue. Special consideration also given to members of the Maconaquah High School Kiwanis Key Club. Active and involved in school, work and community; deserving of financial assistance; Additional Required Information  FAFSA’s Student Aid Report (SAR). EFC (Expected Family Contribution) number must be included.  Pat Keady Memorial Scholarship Essay Question Leonora E. Kolb Scholarship: Created by family of Leonora E. Kolb who passed away in 2000. The first scholarships were awarded in 2003. Eligibility Graduates of Peru High School and likely to succeed in college in the curriculum or discipline of their choice. Harrie and Marty Lanning Memorial Scholarship: This scholarship was established by Harrie and Marty’s family. They resided on a farm west of Mexico, Indiana for 35 years and were long standing members of First Christian Church in Peru. The Family would like for Harrie and Marty to be remembered as a generous and giving couple. Eligibility Resident of Howard or Miami County in Indiana who is graduating or graduated within the previous 5 years from any Howard or Miami County High School in Indiana. A GPA between 2.7 and 3.5 (4.0 scale); class rank of 87% or higher; good citizenship in school, work, community; deserving of financial assistance. Additional Required Information  FAFSA’s Student Aid Report (SAR). EFC (Expected Family Contribution) number must be included.  Essay Question A. Robert M. Larrison – First Farmers Bank & Trust Scholarship: Robert was a beloved Miami County resident and involved in his community. He was a retired CEO and board member of First Farmers Bank & Trust. His friends at First Farmers Bank & Trust created this scholarship to celebrate his life and love of community. Eligibility Current resident and graduating senior from any Grant, Howard or Miami County High School likely to succeed as college, university or trade school students. Student’s family income must be low to moderate as defined by Federal Regulation. GPA of 3.0 or higher based on a 4.0 scale, participated in athletics during high school, involved in community service, employed during high school career. Additional Required Information  2 letters of recommendation from school officials.  FAFSA’s Student Aid Report (SAR). EFC (Expected Family Contribution) number must be included.  Copy of 2014 or most recent tax form (Form 1040-pages 1 & 2; Form 1040A-pages 1 & 2; Form 1040EZ-page 1). This must be your parents’, (the family you live with) tax form not yours unless you are deemed a non-dependent student. Mary Lynn’s Nursing Scholarship: As a young woman, Mary Lynn Houser desired to help others as a nurse. She achieved this dream at 48, going back to school. She received her degree in 1980 and worked in critical care for 17 years until she retired at the age of 65. Eligibility Current female resident of Miami County who are at least 25 years old, pursuing a degree in nursing and currently accepted into a 2 or 4 year nursing program at an accredited Indiana college, university or vocational school. Must have completed one year of college or required prerequisites, must display good academic accomplishments, and must show financial need. Karla Hattery Meyer Nursing Scholarship: FOR CURRENT COLLEGE STUDENTS: Established in 2015 by Ron Meyer and Max Hattery to honor the memory of Karla, who served as a registered nurse in Cincinnati, OH, Peru, IN and Rochester IN. This scholarship is to help students who also share a desire to serve others in the field of nursing. Preference is given to graduates of NMHS. Eligibility Miami County high school graduates who are already accepted into a nursing program at an accredited college, university, trade or vocational school. Must be active and involved in school, work and community. Must display good academic accomplishments. Additional Required Information  Essay Question E Mae Merrell Music Education and Dwight Stout Elementary Education Scholarship: Honoring Dwight Stout and Mae Merrell, Maconaquah educators who enhanced the lives of children through music and education. Eligibility Graduates of Maconaquah High School, accepted into and attend an accredited two or four year college or university pursuing a degree in education. Preference will be given to those seeking a degree in elementary education or music education. Students must be active in school and participate in community activities; Have a 7.5 on a 12 point GPA scale or a 2.5 on a 4.0 GPA scale; Display good musical accomplishments; and attended Maconaquah High School for at least 2 years prior to graduation. Additional Required Information  Essay Question A Miami County Historical Society Richardson Scholarship: Honoring Walt and Betty Richardson and Norma and Bill Richardson. Supporting those pursing a degree in the field of history related subjects. Eligibility Miami County residents and graduating seniors accepted into a 2 or 4 year educational institution and enrolled in a history related subject, museum studies, or non-profit administration with a history-related goal. Good citizenship, involved in extracurricular activities. Must have a “C” average. Special consideration will be given to 1st Generation College Students. Additional Required Information  2 Letters of Recommendation from the following: o 1 letter from a teacher, preferably a history teacher, coach, principal, athletic director, or faculty member. o 1 letter from any other community leader.  Miami County Historical Society Essay Question Miami County Retired Teachers Scholarship: Assists students who are pursuing a degree in Education.

Eligibility Graduating senior from any Miami County High School accepted into and attending an accredited 4 year college or university pursuing a degree in the field of education. Must have a 5.0 GPA on a 12.0 scale. Additional Required Information  Miami County Retired Teachers Essay Question Celia Miller Scholarship: Created to assist those students who are pursing a degree in Education.

Eligibility Graduating senior from Peru High School accepted into and attending an accredited 4 year college or university pursuing a degree in the field of education, except music education. Ranked in the upper 10% of the graduating class; have talent; display good attitude; deserving of financial assistance; who is a good prospect for completion of a college or university course of study. Additional Required Information  Essay question D  FAFSA’s Student Aid Report (SAR). EFC (Expected Family Contribution) number must be included. Alice Moon Scholarship: Alice Moon was a Guidance Counselor at Peru High School. She was a mentor to many students and this scholarship was established by her son George to continue to help the students at PHS.

The alumni of this class established this scholarship as a legacy to their time at North Miami High School. Eligibility Graduating senior who is a resident of Miami County and who graduated from North Miami High School in Indiana. Have at least a 3.0 GPA (4.0 scale); active and involved in school, work and community; display good citizenship and good academic accomplishments. Additional Required Information  Essay Question A

Janice Nowling Memorial Scholarship: Created by family and loved ones to honor this beloved teacher, friend, mother and wife. Jan was involved in the community and a mentor to her students at PHS. Eligibility Graduating seniors of Peru Senior High School, who have an 8.0 on a 12.0 GPA or the equivalent of a B or higher; show good citizenship in work and community. Additional Required Information  Janice Nowling Memorial Scholarship Essay Question.  Letter of Recommendation from a teacher. Monroe W. Smith Band Scholarship: Established to honor Monroe W. Smith and his love of band music by his widow Emma Lou Koch and his daughter Karen Smith Filler. Mr. Smith was a founding member of the Peru Circus Festival Band. Eligibility Graduating seniors of Peru High School with a 2.0 (“C”) or higher GPA. Active and involved in the Peru High School Band program for 4 years. Additional Required Information  Monroe W. Randy Steg Memorial Fund: Created by family and friends to honor his memory and to benefit local charities in Miami County. Randy graduated from Peru High School and Indiana State University. Randy spent his short career as an officer of the Indianapolis Police Department. Eligibility Graduating seniors of Miami County High Schools and residents of Miami County. Special preference will be given to any student with a relative that graduated from Peru High School class of 1984. The applicant must be pursuing a degree in one of the following fields of study: criminology, criminal science, criminal justice or law enforcement. Applicant must display good citizenship in school, work and community. Deserving of financial assistance is preferred but not required. Mary Lee Temple Memorial Scholarship: Mary taught elementary school and was a woman of great character, integrity, and enthusiasm as evidenced through her professional, personal, and charitable achievements. Eligibility Female, graduating seniors of Peru, Maconaquah, or North Miami High Schools and residents of Miami County accepted and plan attend an accredited college or university pursuing a degree in the field of elementary education. Involved in school, work, and community and have outstanding character. Additional Required Information  Letter of recommendation from someone who can comment on your school, work and community involvement.  Mary Lee Temple Essay Question

Lynn Smith Tharp Memorial Nursing Scholarship: The family of Linda “Lynn” Tharp wishes to honor her love for geriatric nursing. They would like Lynn’s life to serve as a model for those individuals who seek to further their education in the field of nursing. Eligibility Must be a resident of Miami County in Indiana and graduating from North Miami High School pursuing a degree in the field of nursing. Involvement in school, work and community; display good citizenship and good academic accomplishments are important. Additional Required Information  Essay Question A.

Unger Memorial Scholarship: Students of Muncie Community Schools, Angola Public Schools and Peru Community Schools are eligible to apply for this scholarship. Eligibility Must be a graduating senior of Peru Community Schools, Muncie Community Schools or Angola Public Schools and pursuing college and graduate training in string music. GPA of 2.8 or higher; Additional Required Information

Samuel T. Waltz Health and Science Scholarship: This scholarship is to promote the art of medical science for the treatment and research of disease and the wellbeing of society. Eligibility Must be a Miami County, Indiana resident and graduate of any Miami County High School. Traditional or non-traditional students are eligible to apply. Students must be full or part time studying in the following health-related or medical research course of study: medicine, dentistry, anesthesiology, nursing, radiology or any medical specialty. Also those students who are planning to study primary and or secondary medical research. Additional Required Information  Essay Question C. Essay Questions Part D: Respond with insight, good grammar and English usage, and clarity of thought.
