Care Co-ordination Team for Older People Description of Service The Care Co-ordination Team is a multi-disciplinary / multi-agency (PCT/Social Services) service providing a pro-active preventative service to older people who have multiple complex health and social care needs. Core activities include:

z Person centred holistic assessment using the Single Assessment Process. Through early intervention prevent ill health or deterioration of health by the mobilisation and co-ordination of appropriate resources from statutory and voluntary agencies,including provision of information and advice to individuals and their carers on all aspects of care. z To act as a resource working collaboratively with statutory and voluntary agencies. z To offer health and social care Needs Assessments for the informal carers of people with dementia, and support in group settings. z To work with Ethnic Minority Elders providing assessment of health and social care needs, as well as health promotion sessions in outreach settings. z To develop evidenced based interventions, in line with national and local requirements that will improve the quality of life for older people and their carers. z To have an in depth knowledge of Adult Protection. Care Home Liaison Team Description of Service The service provides support to the residents and staff of all Bromley borough Care Homes with nursing. The support may be patient specific and involve hands on input or advice or support of a more general nature. All members of the team are involved in supporting the ongoing professional development of nurses working in this area, providing information, advice, informal and formal teaching sessions within the care homes with nursing, as well as facilitating training through the Trust's training department. Who can refer The Nursing Team accepts referrals from any clinician within the Trust as well as from Social Services, Bromley Hospitals Trust and Care Homes with Nursing themselves. Child and Adolescent Mental Health Service - Tier 2 Psychology and Behaviour Management Service Description of Service The Tier 2 service consists of individual specialist workers who help children, adolescents and their families with emotional, behavioural and psychological problems. The service is a multi-agency initiative and was set up as an additional specialist resource in response to the Joint CAMHS Strategy developed in Bromley in 1999. The service provides

z Tier 1 liaison, consultation and training z Evidence based work such as problem-solving skills and cognitive behavioural therapy z Services to vulnerable children z Psychology services for children aged up to 18 years and their families with regard to mental health and behaviour management issues. Children's Short Break Service Description of Service Hollybank offers a range of short term break services for children with disabilities aged 0-19. The services offered include a home-based respite service for children aged 0-5, a term-time only creche facility for children aged 2-rising 5 and overnight short term breaks for children aged 5 - 19. Chiropody and Podiatry Service Description of Service Provides a broad range of foot treatments and advice to those with a podiatric or medical need. Specialist services are offered in the following areas : wound management, diabetic management, podopaediatrics, minor surgery, biomechanics, orthotics. Eligible groups are : school children, people with a learning disability, people with physical/mental disability, diabetics, pregnant and nursing mothers. Community Heart Failure Service Description of Service This is a home visiting service for patients with confirmed heart failure on echocardiogram. The team monitor and review a client's symptoms, their medication and the care they require including health education and support.

Community Matrons Description of Service Community Matrons provide a pro-active preventative service to predominantly older people who have multiple and complex health and social care needs in their home setting. The Team works collaboratively with all professionals (statutory and voluntary agencies), the patient, carers and relatives to understand all aspects of the patient's physical, emotional and social situation providing information and advice on all aspects of care. The aim is to:

z develop a personalised care plan with the patient, carers, relatives and health and social care professionals, based on a full assessment of medical, nursing and care needs. The plan includes preventative measures and anticipates any future needs the patient may have. z Teach carers and relatives to recognise subtle changes in the patient's condition that could lead to an acute exacerbation or deterioration in their health, and to seek the appropriate help. Community Paediatricians Description of Service Community Paediatricians provide a wide range of specialist medical services in community clinics, schools, nurseries, and the Phoenix Children's Resource Centre. Our aim is to provide a child centred and easily accessible service and to promote the health of children within the context of the family, school and society. We work in close collaboration with health visitors, school nurses, general practitioners, and other members of the child development team, CAMHS and colleagues in hospital paediatric departments. We also work closely with social services in child in need/child protection enquiries and with education with regards to children with special educational needs. Most of the team is based at the Phoenix Children’s Resource Centre. The main aspects of a community paediatrician’s role are: z Providing assessment and care of children where there are neurodevelopmental concerns/disability. z Providing paediatric input into the assessment of children with special educational needs. z Contributing to the assessment and management of "children in need" including children in need of protection as part of a multi-agency process. z Medical Care for Looked after Children and providing medical support to Bromley Adoption Panel. z Working with Child Mental Health Services in providing assessment and care for children with emotional and behavioural difficulties. z Providing a paediatric opinion whenever requested. z Coordinating care within the community setting as a member of the multidisciplinary team. Community Physiotherapy for Children Description of Service A specialist service for those registered with a Bromley GP, aged 0-16 and 0-19 (if attending a special school within Bromley). It provides seamless support from leaving hospital to pre-school, school through to young adulthood.It aims to be responsive to urgency of need and flexible providing an assessment and treatment/advice as required. The aim is to enable, rehabilitate, maintain or support the child and their families/carers to meet his/her need and to assist finally with integration into society as a whole. Areas covered are orthopaedic, neurological, medical, neurodevelopmental, post surgery care, wheelchair and orthotics management. Who can refer The service operates an open referrals policy which includes all Community Health professionals, GPs and hospitals. In the case of self referral GPs will always be notified. How to make a routine referral Written referrals are accepted, parental permission should be obtained. There are generic forms available from the Phoenix CRC.Letters or reports are accepted provided that they contain the approriate information. They should include a medical history,main concerns and full contact details including telephone numbers. It would be useful to know if any other professionals are involved. Community Physiotherapy Service for Adults - Rapid Response Description of Service The Adult Community Physiotherapy service currently offers physiotherapy for adults whose health and circumstances means that they are unable to use hospital services, and whose problems are of mobility and physical management in the home environment, for example, older people who are too ill or too vulnerable to access the hospital out-patient physiotherapy services or those living with long-standing illness, disease or recovering from stroke or surgery. Clients referred to the Physiotherapy Service should meet the following criteria: • Registered with a GP within Bromley • Inability to access hospital services because of disability, illness or frailty • Physical management problems in home environment • Clients 18 years and over who are no longer in statutory education Who can refer GP or other Health Service/Social Service professionals e.g. District Nurse, Care Manager or Health Visitor. Complex Communication Service Description of Service Multi-disciplinary service whose aim is to:

Continence Service (Bladder and Bowel Management) Description of Service The Continence Team :

z provides a specialist assessment of bladder and bowel symptoms for children and adults z provides specialist treatment and intervention z educates health professionals in the promotion of continence z supports health professionals in continence management , identifying groups at risk, promoting awareness and prevention. Who can refer clients, carers, relatives, health professionals. Contraception and Reproductive Health Service Description of Service This service provides contraceptive, cervical cytology and sexual health advice and is available to all regardless of age or gender. The service is provided in clinics across the borough with three clinics dedicated to under 25s. Those under 25 can however be seen in all clinics. There is also a vasectomy service and a psycho-sexual counselling clinic. Who can refer For most services self-referral. A GP referral is required for vasectomy or psycho-sexual counselling. There is a domiciliary family planning service provided for those who are unable or unwilling, for whatever reason, to attend a clinic. Diabetes Specialist Nursing Description of Service Diabetes Service Orpington Hospital: Provides advice and education and support to people with diabetes and other professionals and carers involved with people with diabetes. The team work as part of the multi disciplinary Diabetes Service at the Diabetes Centre at Orpington Hospital. The Community Diabetic Service consists of a Diabetes Specialist Nurse, Dietitian and Podiatrist who provide advice and support to primary care and see patients at community clinics and in their own homes if necessary. The service provides structured group education for patients newly diagnosed with Type 2 Diabetes.ennedy House. Intermediate Care Services. Community Assessment and Rehabilitation Teams Description of Service C.A.R.T is a multidisciplinary team, jointly funded with London Borough of Bromley.The team comprises a Doctor, Nurses, Physiotherapists, Occupational Therapists, Rehabilitation Technicians, Care Managers, Nursing Rehabilitation Assistants,and support from Community Psychiatric Nurses, with direct access to a consultant as needed. There are two C.A.R.T's, one based in the East at St Pauls Cray clinic and the other in the West at Yeoman House in Penge. The Teams offer a service to people who would otherwise face unnecessarily prolonged hospital stays or inappropriate admission to acute inpatient care or long term residential care. Provision of the service is based on a comprehensive assessment, resulting in a structured individual care plan that involves active therapy, treatment or opportunity to recover. The service is time limited, normally no longer than 6 weeks but sometimes as little as 1-2 weeks. The service is provided to people over the age of 18 who are resident in the London Borough of Bromley or are registered with a GP within the Borough of Bromley and are medically stable and predicted to remain so. Occupational Therapy - Children Description of Service The Occupational Therapy (OT) service is provided to children with physical difficulties that impact on their ability to carry out functional activities required of them, such as dressing, handwriting, organising themselves, feeding themselves and playing. The service works in conjunction with education staff, parents/carers and other professionals, providing training and advice to develop an individual child’s skills. Children aged 0 – 18 are seen by the service and children will be seen if: a) They have a Bromley GP or b) They attend a special school in Bromley Borough or: c) If they attend a mainstream school in the Bromley borough and it has been agreed with their local therapist. Children should also meet the following criteria : a) Have a motor co-ordination difficulty where their motor skills are below those expected given the child’s chronological age and other areas of their development. b) The Child has Sensory/Motor problems that severely impact on their ability to perform daily living activities. c) Children with a chronic physical difficulty requiring intervention to develop/maintain their motor skills or the provision of equipment. Assessment is generally carried out initially within a clinic environment and advice and recommendations would be provided to both home and school. This process may also include assessment for specialist equipment and the recommendation of the provision from the most appropriate authority. Once a child’s case has been closed to the service either parents or education staff can re-refer the child for further advice or review via the advice clinic service. Primary Care Mental Health Team Description of Service The Primary Care Mental Health Team provides services to people with mental health problems who do not require specialist mental health services provided by Oxleas NHS Trust. The team works with GP practices across the borough.

School Nursing Service Description of Service The school health service provides health surveillance for children of school age and advice on management of any medical problem which affects a child in school. As part of the core health surveillance programme the school health service aims to promote healthy lifestyles amongst school aged children. The school health service liaises with the local education authority in the provision of health care to those children identified as having a health problem which affects the access to the normal school curriculum. This service is for patients over 18 years of age who have acquired communication and/or swallowing problems. Patients may have problems as a result of Stroke, Parkinson's disease, Multiple Sclerosis, Motor Neurone disease, cancer or have voice or fluency problems.
