For all of these protocols you will need the written consent of the client beforehand. 1. Make a written request to a mental health team of the documents you would like copied and send to them with the signed consent form. They usually fax these copies to you. The one exception is the outpatient clinic at Lion’s Gate Hospital, where you will need to phone ahead to let staff know you are coming in. Show medical records the signed consent and then view the record and make your own copies. .
