Holt , M.D. 2 Abstract We can admire and learn from physicians with acute clinical acumen and superb approaches to translational research. The observations and studies of Dr. Willem Dicke, a Dutch pediatrician, on the toxic effects of a protein component of wheat and rye demonstrate the highest quality of such investigations. From a clinical observation of one child with celiac disease, through years of historical questioning and empirical dietary suggestions of patient families, he concluded that such foods were toxic. When research became possible after the second world war and fecal fat measures as a hard end point became available he studied 5 children in detail to establish the validity of his clinical clues. Analissa Drummond PA-C Department of Pediatrics Division of Pediatric Gastroenterology HISTORY OF CELIAC DISEASE Also called gluten-sensitive enteropathy and nontropical spue First described by Dr. Samuel Gee in a 1888 report entitled “On the Coeliac Affection” Term “coeliac” derived from Greek word koiliakaos-abdominal Similar description of a chronic, malabsorptive disorder by Aretaeus from Cappadochia ( now Turkey) reaches as far back as the second century AD HISTORY CONTINUED The cause of celiac disease was unexplained until 1950 when the Dutch pediatrician Willem K Dicke recognized an association between the consumption of bread, cereals and relapsing diarrhea. This observation was corroborated when, during periods of food shortage in the Second World War, the symptoms of patients improved once bread was replaced by unconventional, non cereal containing foods. This finding confirmed the usefulness of earlier empirical diets that used pure fruit, potatoes, banana, milk, or meat. HISTORY CONTINUED After the war bread was reintroduced. Dicke and Van de Kamer began controlled experiments by exposing children with celiac disease to defined diets. They then determined fecal weight and fecal fat as a measure of malabsorption. They found that wheat, rye, barley and to a lesser degree oats, triggered malabsorption, which could be reversed after exclusion of the “toxic” cereals from the diet. Shortly after, the toxic agents were found to be present in gluten, the alcohol-soluble fraction of wheat protein. PATHOPHYSIOLOGY Celiac disease is a multifactorial, autoimmune disorder that occurs in genetically susceptible individuals. Trigger is an environmental agent-gliadin component of gluten. -

(Tech.) Jinze Dou, Ph.D. Kavindra Kesari This study aimed to clarify the feasibility of utilizing spruce inner bark extract as a sun protection agent in sunscreens. Ultrasound-assisted extraction with 60 v-% ethanol was applied to isolate the extract in 25-30 % yield, that was almost independent of the temperature (45-75oC) and time (5-60 min) of the treatment. However, the yield of stilbene glucosides, measured by UV absorption spectroscopy, was highest after ca. 20 min extraction. Nuclear magnetic resonance spectroscopy of the extract showed that it consisted mainly of three stilbene glucosides, astringin, isorhapontin and polydatin (piceid). The maximum overall yield of the stilbene glucosides was > 20 %. Extraction with water gave a much lower yield of the stilbene glucosides. Sunscreens composed of a mixture of vegetable oils, surfactants (fatty acids), glycerin, water and the bark extract were prepared with the low-energy emulsification method. -
