Sites vaccinating this age group include the Florida Dept. of Health Broward, CVS, Walgreens, Holy Cross Hospital, Walmart, independent pharmacies and Jackson Health Systems. • Children 16 and 17 can only receive the Pfizer vaccine and those sites require a parent or guardian to be present. They will need a consent form at state sites. Moderna and Johnson & Johnson are only approved for age 18 and over. • If the location is marked with an asterisk *, then it means there are open vaccine appts • Florida has eliminated the Proof of Residency requirement at State and Federal supported vaccine sites. Check with cities, hospitals and pharmacies to see their vaccine policies.
