• Effective Thursday, May 13, 2021, children ages 12 to 15 can now receive the Pfizer vaccine. Sites vaccinating this age group include the Dept. of Health Broward, CVS, Walgreens, Holy Cross Hospital, Walmart, independent pharmacies and Jackson Health Systems. • Children 16 and 17 can only receive the Pfizer vaccine and those sites require a parent or guardian to be present. They will need a consent form at state sites. Moderna and Johnson & Johnson are only approved for age 18 and over. • If the location is marked with an asterisk *, then it means there are open vaccine appts • Florida has eliminated the Proof of Residency requirement at State and Federal supported vaccine sites. Check with cities, hospitals and pharmacies to see their vaccine policies.

Homebound Seniors:

*Homebound Seniors – Broward County Residents: The State has launch a new way for homebound seniors to sign up to have the vaccine come directly to them. Email: [email protected] or call 866.779.6121

*Homebound Seniors – Miami Dade Residents: In-home vaccinations will be available for Miami- Dade residents, with no out-pocket-costs or insurance required, beginning April 11. The program is designed to assist county residents who are without transportation, elderly, persons with disabilities or not otherwise able to easily travel to receive a vaccine. County residents 18 years of age and older who are unable to travel to a vaccination site can register online and will receive a call back to schedule their in-home appointment. http://mymobilevax.com
