+COVID-19 VACCINATION WEBSITES/PHONE NUMBERS • Effective Thursday, May 13, 2021, children ages 12 to 15 can now receive the Pfizer vaccine. Sites vaccinating this age group include the Florida Dept. of Health Broward, CVS, Walgreens, Holy Cross Hospital, Walmart, independent pharmacies and Jackson Health Systems. • Children 16 and 17 can only receive the Pfizer vaccine and those sites require a parent or guardian to be present. They will need a consent form at state sites. Moderna and Johnson & Johnson are only approved for age 18 and over. • If the location is marked with an asterisk *, then it means there are open vaccine appts • Florida has eliminated the Proof of Residency requirement at State and Federal supported vaccine sites. Check with cities, hospitals and pharmacies to see their vaccine policies.
