Non-Commercial Service Obligations and Liberalisation Non-Commercial Service Obligations 2003 The OECD Competition Committee debated non-commercial service obligations in October 2003. This document includes an executive summary and the documents from the meeting: an analytical note by Mr. Sean Ennis for the OECD, written submissions from Australia, Austria, Canada, Czech Republic, Denmark, the European Commission, Hungary, Japan, Norway, Portugal, Switzerland, Chinese Taipei, the United States, as well as an aide-memoire of the discussion. Universal service obligations are common in many of the infrastructure sectors. The obligations are often cited as a justification for limiting entry of new providers because the new providers would cherry-pick the high profit customers who provide the basis for subsidisation of another group of customers. When obligations are beneficial, there are a number of policy traps that can be encountered. Obligations are often not well-focused on the customers who are meant to receive help. Obligations are frequently defined narrowly in ways that disfavour new technologies and cause extensive waste; and financing for the non- commercial obligations can often be raised in more efficient ways than through cross-subsidies and can often be spent on multiple service providers rather than one preferred provider. Falling into these traps can create incentives for over-investment in certain infrastructure technology and under-investment in other technology. This document provides guidance on the competition problems that can arise from universal service obligations and on some ways to limit these problems. Failure to include the SIJ in the diagnostic evaluation of low back pain could result in unnecessary health care expenses. 1Department of Orthopedic Surgery, 2Department of Neurosurgery, Design: Decision analytic cost model. University of Minnesota, Minneapolis, Methods: A decision analytic model calculating 2-year direct health care costs in patients with MN, 3SI-BONE, Inc., San Jose, CA, USA chronic low back pain considering lumbar fusion surgery was used. Results: The strategy of including the SIJ in the preoperative diagnostic workup of chronic low back pain saves an expected US$3,100 per patient over 2 years. Cost savings were robust to reasonable ranges for costs and probabilities, such as the probability of diagnosis and the probability of successful surgical treatment. Conclusion: Including the SIJ as part of the diagnostic strategy in preoperative patients with chronic low back pain is likely to be cost saving in the short term.
