We conducted occupational cohort studies to find out causes of ill health at work. The work of Sackett and colleagues raised our interest in other applications of epidemiology: how to use scientific evidence in professional practice. Their work showed that epidemiology was much more widely applicable in practical daily occupational health care than in occupational cohort studies only. Methods of clinical epidemiology can be usefully applied. After attending a course on Evidence-based Medicine (ebm) we became enthusiastic about the possibilities of applying ebm to our field. It turned out that there was a lot of research-based knowledge, information or evidence, whatever you like to call it, available in the scientific literature that has practical value. We judged this as a most interesting extension of our traditional sources of information such as text books or experts’ opinion. For us, it has been much fun to do and very instructive to learn. Since much of the information is available free of charge through the Internet, everyone with Internet access can use it. With this book we hope that we can convey our optimistic experience so far to other occupational health professionals. ;dgl]d^hi]^h\j^YZ^ciZcYZY4 The guide is intended for occupational and public health professionals interested in trying to improve the quality of their work. Retrieving information is not difficult nowadays. However, you need a more systematic approach if you want to be sure that you can rely on the coverage and quality of the information found. Unfortunately, there is not one fixed pathway for the approach. You have to have some interest in experimenting and finding out how the methods of evidence-based medicine suit you best. There are many textbooks available on evidence- based medicine, but none of them is written from the point of view of occupational health. We hope that this book will fill up this gap at least for some part. EG:;68: - L]Vieg^dg`cdlaZY\Z^hcZZYZYidldg`i]gdj\]i]^h\j^YZ4 There is some discussion about the prior knowledge needed to be able to use research infor- mation in occupational health practice. In our view, you need a basic background in health science and some knowledge of epidemiology. You need to be familiar with terms as prognosis and diagnosis and you need some understanding of bias that can distort the results of scien- tific research.

First published in 1997, the Handbook underwent a major update in July, 2000. The ILO Occupational Health Services Convention (No. 161) defines “occupational health services” as services entrusted with essentially preventive functions and responsible for advising the employer, the workers and their representatives in the undertaking on the requirements for establishing and maintaining a safe and healthy working environment which will facilitate optimal physical and mental health in relation to work and the adaptation of work to the capabilities of workers in the light of their state of physical and mental health. In reaching toward these objectives, NIOSH and WHO have been able to collaborate on several projects . One project has now resulted in this text, A Joint Publication on Teaching Epidemiology in Occupational Safety and Health. This volume focuses on the need to train occupational epidemiologists in the recognition and evaluation of occupational diseases and injuries. It is a training tool that uses the case approach to instruct epidemiolgists. A Select Committee of the House of Lords reviewing the provision of health and hygiene services in the UK has advocated a much greater involvement of the general practitioner in occupational health surveillance at a primary level.' The Asbestos Regulations that came into force in the UK last month will require new medical resources for their implementation. Health and safety legislation coupled with pressure from employees' organizations and the European Community will result in a marked increase in statutory medical examinations. A proposal for a second programme of action on health and safety at work has been made to the Council of the European Economic Communities. The implications of all these developments for general practitioners with an interest in occupational medicine are obvious. IN the absence of a generally accepted model for occupa- tional health and hygiene services in the UK such as exists for example in France, there are opportunities for experi- ment. Dr. Moline continues to maintain a clinical practice, focusing on patients with occupational exposures, and has had a particular interest in asbestos exposed individuals. Since 2001, many of Dr. Moline’s endeavors have been centered on the medical evaluation and treatment of World Trade Center (WTC) responders.
