Section 01 22 00.00 28 MEASUREMENT AND PAYMENT will be applied to Item Nos. 0007 and 0008. 00 10 00 page 4 Statement of Work Construction Specifications Audubon Bend Road Repairs 1.0 Scope of Work. The contractor shall be responsible for all labor, equipment, and materials (except those explicitly provided by the Government) required to repair approximately 0.87 miles of gravel road damaged by severe flooding. Road repairs include blading the road surface to smooth out scour holes and rebuild the crown, resetting an existing culvert, reconstructing an eroded area that covered the existing culvert, installing a new culvert (provided by USACE) into an eroded area, reconstructing the road surface over the new culvert, and bringing in gravel to place along the road surface. The Contractor will coordinate with the COR on the commencement date of the project. The Contractor will have 120 days to complete the project upon receiving a Notice to Proceed (NTP). Offerors or quoters are urged to inspect the site where the work will be performed. 2.0 General 2.1 Standards and Regulations. The Contractor shall perform to the standards in the contract and comply with all Local, State, and Federal Regulations during the services performed. The Contractor is responsible for ascertaining the extent to which these regulations affect the operations resulting from this contract. The Contractor shall comply will all applicable regulations. Applicable regulations include, but are not limited to the following: a. Engineering Manual (EM) 385-1-1 (USACE – Safety and Health Requirements Manual), dated November 2014; http://www.publications.usace.army.mil/Portals/76/Publications/EngineerManuals/EM_385-1-1.pdf b. Occupational Safety and Health Standards (29 CFR 1910); 2.1.1 Activity Hazard Analysis (AHA). The Contractor shall prepare the AHA in accordance with Paragraph 01.A.14 or 01.A.15 of the EM 385-1-1. If using Paragraph 01.A.15, prepare the AHA IAW Figure 1-2. Note: Contractors and other individual employer’s typically use Job Safety Analyses (JSAs), Job Hazard Analyses (JHAs), or similar Risk Management assessment tools. The government considers equivalent to, and acceptable substitutes for, the USACE’s AHA provided the data collected is the same as that required by the AHA, commensurate with the type of services the contractor is performing. 2.1.2 Abbreviated Accident Prevention Plan (APP). EM 385-1-1 in its entirety may be too complex for the type of work performed under these contracts; contractors may reference Appendix A, for abbreviated Accident Prevention Plan (APP). The Contractor shall only need to submit items in Appendix A that are pertinent to the contract. The contractor shall use a qualified person to prepare the site-specific abbreviated Accident Prevention Plan (APP). The contractor shall prepare the APP in accordance with the format and requirements of the EM 385-1- 1 and supplemented herein. The Contractor shall sign and submit the APP for approval prior to the start of work onsite. 2.2 Measurement and Payment. The government will make payment based on each Unit cost per the Pricing schedule. Refer to the Pricing Schedule and Section 01 22 00.00 2 MEASUREMENT AND PAYMENT. 2.3 Location. Work will occur at Audubon Bend, located approximately 2 miles north of Wynot, Nebraska (See Figures 1 & 2 of attachments). The site entrance is located off of 893 Rd, east of the intersection of 568 Avenue and 893 Road, in Cedar County. 2.4 Staging Area. A public parking area is located at the entrance to the site and is available for use as a staging area. Limited space within the site is also available for the Contractor to stage equipment and/or material. The contractor shall contact the Contracting Officer’s Representative (COR) for staging area needs. This staging area is not secured. Page 1 2.5 Delivery, Storage, and Handling. The Contractor will be responsible to obtain all necessary state and/or local permits to utilize roads and bridges on their route. 2.6 Work Hours and Work Limits. The Contractor is responsible for conducting business between sunrise and sunset, Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor will notify the COR of expected work hours to assist in coordinating other activities at the site. The road is temporarily closed to the public, but USACE, USACE contractors, and agricultural lessees may access the road during repair operations. The Contractor is to make sure gate is closed and locked every night when work is completed. 2.7 Facilities. No public utilities are available on site. The contractor is responsible for portable toilets. 2.8 Environmental Conditions. The Contractor shall follow all applicable federal, state, and local laws and regulations concerning environmental pollution control and abatement. The site is located off of a county road that receives minimal maintenance. Conditions along 893 Rd may be variable depending on weather and level of use. 2.9 Antiterrorism/Operation Security Requirements. N/A 2.9.2 Pre-screen candidates using E-Verify Program. The Contractor must pre‐screen candidates using the E‐verify Program (http://www.dhs.gov/E‐Verify) website to meet the established employment eligibility requirements. An initial list of verified/eligible candidates must be provided to the COR no later than three (3) business days after the initial contract award. 2.10 Submittals. Prior to commencement of work, the Contractor shall submit the following documents to the COR for review and approval: a. Activity Hazard Analysis (AHA); b. Abbreviated Accident Prevention Plan (APP); c. Project schedule with individual types of work durations and total project’s duration; d. List of owned or rented equipment that will be used on this contract; e. List of who will perform work on this contract; f. Proof of Insurance for Contractors and Subcontractors; g. Specific materials specified in the SOW; h. Road/bridge permits; i. Borrow agreement. 3.0 Materials. The Contractor will meet the following specifications for earth fill materials (soils) to reconstruct eroded areas, gravel, topsoil, and grass seed: • Earth fill materials (soils) must be free of petroleum or other /hazardous materials. Satisfactory materials comprise any materials classified by ASTM D 2487 as GW, GP, GM, GP-GM, GW- GM, GC, GP-GC, GM-GC, SW, SP, SM, SW-SM, SC, SW-SC, SP-SM, SP-SC, CL or CH. Satisfactory materials for grading comprise stones less than 3 inches. Unsatisfactory materials include man-made fills; trash; refuse; backfills from previous construction; and material classified as satisfactory which contains root and other organic matter or frozen material. Joel Sprague TRI/Environmental, Inc., Greenville, SC ABSTRACT Much of the development of geosynthetics technology in environmental applications has been in response to government regulations. This is certainly true for geosynthetics used in erosion and sediment control. Geosynthetics continue to replace traditional materials such as soil and stone in performing important engineering functions in erosion and sediment control applications while simultaneously introducing greater versatility and cost-effectiveness. Geosynthetics are widely used as a “carrier” for degradable materials to the enhancement of vegetative establishment; as nondegradable materials to extend the erosion control limits of vegetation or soil; as primary slope or channel linings; as components in silt fences and turbidity curtains; and as a component in an ever growing array of sediment retention devices. Along with the introduction of geosynthetics into this wide range of applications has come the need for industry-wide initiatives to promote their correct use and new test methods to characterize them. All of which, are a “work in progress”. THE NEED FOR EROSION AND SEDIMENT CONTROLS Much of the development of geosynthetics technology related to erosion and sediment control applications has been in response to government regulations.     Erosion Prevention and Sediment Control Plan Checklist Page 1 of 4 What is an Erosion Prevention and Sediment Control Plan? Erosion and sediment control is much more than silt fence and hay bales. Prior to developing an Erosion Prevention and Sediment Control Plan (EPSCP), it is important to have minimized the areas of disturbed soils and the duration of exposure. It is also imperative to control water at up- slope site perimeters, control water on-site, control sediment on-site, and control sediment at the downslope site perimeters. An EPSCP is the final element in the erosion and sediment control planning process and a necessary component of an Act 250 permit application. The EPSCP ensures that sediment transport is addressed in one of the most crucial stages of the project: the planning stage. A good erosion prevention and sediment control plan first minimizes the extent of disturbance by focusing on erosion control (minimizing disturbed areas, seeding, mulching, matting) by controlling the amount of soil that can run off and by stabilizing exposed soil. Sediment control measures (i.e. stabilized construction entrances) then focus on any sediment that has escaped your erosion control measures. Erosion prevention measures are far more effective than sediment control measures (such as silt fence) and should be the primary focus of any EPSCP. An EPSCP has five primary components: 1. Location map (USGS and other) 2. Existing conditions site plan 3. Grading plan and construction timetable 4. Erosion prevention and sediment control site plan and timetable 5. Narrative briefly describing the four plans The location map shows the proximity of the site to any surface water bodies, roads, etc.

    Shelbyville, KY January 2013 Stormwater Best Management Practices 3.2 SEDIMENT CONTROL PRACTICES FACT SHEETS (SMP) Sediment Control Practices SMP-01 Silt Fence Symbol SF Description To detain sediment-laden water, silt fences are used to promote silt deposition behind the fence. These fences are made of filter fabric that has been entrenched, attached to support poles and occasionally supported by a wire fence. Silt fence is intended as a temporary sediment barrier and requires routine maintenance. Application Silt fence should be used in area accepting sheet flow conditions. Silt fence should not be used in ditch lines, streams, or other areas of concentrated flows. Silt fencing can be used along the downstream perimeter, below the toe of a cleared slope, upstream of sediment traps or basins, along streams and channels and around temporary spoil areas. Design The design criteria for silt fence is as follows: Silt fencing should be installed along the contour. It should not be installed up and down slopes or around the perimeter of large construction sites unless accompanied by measures such as “J” Hooks or other methods. The length of silt fence is determined by the amount of run-off area. The minimum area should not exceed 0.25 acre per 100 linear feet of silt fence. Spacing of silt fence is variable depending on the slope of land draining to the fence.
