Middle School Winter Term Activities Sign-up • Participation in after-school activities during the winter term is optional. If you choose not to participate in one of the activities listed below, you must have your parents sign this form indicating that they approve of your decision not to participate in an after- school activity and that you can be picked up every day by 3:15 p.m. • You will make your selection for an after-school activity on Wednesday, September 27. (Advisers will go over this form with you and return the completed forms to Mrs. McDowell. Students electing not to participate in an after-school activity must bring this form home to be signed by a parent.) • You may only select one activity. If you want to participate in more than one activity, you need to get permission from Ms. Birdsall. (This exception does not include , which starts after most winter activities end and before spring competition begins.) • Winter activities begin on Tuesday, October 10. • One season of physical activity during the school year is required for all Middle School students.
