Chalazion and Stye www.healthinfo.org.nz Chalazion and stye Chalazia (the plural of chalazion) and styes are lumps that form on your eyelids. Chalazia are formed by blocked oil glands, while styes are formed by an infection at the base of an eyelash. They can happen at any age, but are more common in adults. Chalazia A chalazion is a lump on one of your eyelids. It forms when a tiny oil gland on the inside of your eyelid (also called a meibomian gland) gets blocked and swollen. Oil builds up inside the gland and forms a lump. Eventually the gland may break open and release the oil into the surrounding tissue. This can cause redness and swelling of your eyelid. At first a chalazion is a red, tender, and swollen area but in a few days it changes to a painless, slow-growing lump. Usually chalazia start out very small, but they can grow to the size of a pea. They can make your eye water more, and large chalazia that press against your eye can cause blurred vision and discomfort. Often chalazia will go away by themselves after about a month, without any treatment. A chalazion might also be called a meibomian cyst. Styes Chalazia are often confused with styes. Stye is the name for a lump or swelling that happens when an infection forms around the base of eyelashes, on the edge of your eyelid. A stye causes a painful, red lump or swelling. Styes are caused by bacterial infection of the gland at the base of an eyelash. Styes are usually smaller, painful and closer to the surface of the eyelid than chalazia. They can look like a tiny yellow pus-filled spot. A chalazion can happen as an after-effect of a stye. Who gets them? Chalazia and styes can happen at any age. You are more likely to get one if you: . have had one before . touch your eyelids when your hands aren't clean . have chronically inflamed eyelids and eyelashes, also called blepharitis . have a skin condition like acne, rosacea or seborrhoeic dermatitis. To prevent chalazia or styes, it may help to gently clean the eyelash edge of your eyelids each night. Use eye cleansing pads or diluted baby shampoo. You can also put a warm compress on your eye lids for a few minutes each night before bed. And remember to keep your hands clean. Written by Canterbury optometrists.     Chalazion Treatment This material will help you understand treatments for chalazion. What is a chalazion? A chalazion is a red, tender lump in the eyelid. It is also known as a stye. The swelling occurs because one of the oil glands that is next to each eyelash can get backed up and become inflamed. This is very similar to a pimple. How is a chalazion treated? In many cases, chalazia resolve on their own without treatment. Applying a warm compress over your eye for 5- 10 minutes two to four times a day can soften the oil that is backed up. This helps the chalazion heal. If the chalazion does not heal after one month of using warm compresses, your doctor may suggest surgical removal or injection with medications to help it heal faster. How is a chalazion surgically removed? Surgical removal of a chalazion is an outpatient procedure. Before the procedure, your doctor will give you a local anesthetic to numb the area around the chalazion. Next, your doctor will place a clamp to help hold your eyelid in place for the procedure. That way, you will not need to worry about keeping your eyelid open for the procedure. The doctor will then make a small incision in the eyelid and remove the chalazion with a special instrument. The location of the incision (front or back of the eyelid) depends on the size of the chalazion. Small chalazia can be removed by making an incision on the inside of the eyelid. If your chalazion is large, the doctor may make an incision on the front of the eyelid and close it with dissolvable stitches.

Furthermore, an enlarged lymph node is seen in the right laterocervical site. On objective examination of the anterior segment we find bilaterally: reduced lacrimal meniscus, posterior blepharitis, obstruction of all the Meibomian glands of the upper and lower eyelids, conjunctival hyperemia with dry spots, transparent cornea, transparent crystalline. The no contact tonometry is 15 mmHg in RE, 16 mmHg in LE. The OCT of the macula does not show changes in both eyes. The BUT is 4.9 seconds in RE, and 15.6 seconds in LE.

The exact Tcause may be difficult to determine and may cause some concern that a serious ocular condition has been missed. Thorough history and clinical examination will help delineate the final diagnosis. When there are doubts, prompt referral to an oph- thalmologist can prevent serious consequences. Often, the most likely diagnosis of an acutely red eye is acute conjunctivitis. In the first day, an acute bacterial infection may be hard to differentiate from viral, chlamydial and noninfectious conjunctivitis and from episcleritis or scleritis. Below is a review of the most commonly seen forms of eye infections and treat- ments. Failure to improve after three to five days should lead to a re-evaluation of the patient and appropriate referral where necessary. CHRONIC BLEPHARITIS Clinical: Gritty burning sensation, mattering, lid margin swelling and/or scaly, flaky debris, mild hyperemia of conjunctiva; may have acne rosacea or hyperkeratotic dermatitis (Figure 1). Anterior: Staphylococcus aureus (follicles, accessory glands); posterior (meibomian glands). Treatment: • Lid scrubs (baby shampoo, lid-care towellettes, warm compresses). Figure 1. There may be localized sensitivity to the shampoo or the components of the solution in the towellettes (e.g., benzyl alcohol). • Hygiene is important for the treatment and management of chronic blepharitis. Topical antibiotic-corticosteroid combinations (e.g., tobramycin drops, tobramycin/dexamethasone or sulfacetamide sodium-prednisolone acetate). Usage of these medications is effective in providing symptomatic relief, as the inflammatory component of the problem is more effectively dealt with.

A 45 years old lady came to us 15 days after the chalazion surgery who, on examination revealed a pale optic disc, retinal hemorrhage and a perforation site following chalazion surgery in left eye. Care should be taken while giving block and further injection of anaesthetic agent should be withheld if resistance is encountered. What are the meibomian glands and what do they do? • The normal eyelid is made of muscle and skin on the front and a rigid cartilage-like structure on the back. This cartilage-like structure gives the eyelid its rigidity and is known as the tarsal plate. Located within the tarsal plate are the meibomian glands- 20-30 tiny specialised oil glands which secrete a fine film of oil on to the surface of the eye. If one carefully looks at the edge of the eyelid, one can see tiny holes which represent the openings of the ducts through which the meibomian glands secrete their oil. • This oil which is known as mebum has an important job in contributing to the tear film on the surface of the eye.
