PEOPLE’S DEVELOPMENT INITIATIVES (PDI)

THE MISSION:

To recognize the inherent capacities existing among the marginalized and supporting and building on them to herald the beginning of a new life where people can make their own choices.

THE VISION:

To overcome economic, legal, social and gender inequalities and facilitate the process of “People Gaining Control over their Lives”.

MAIN OBJECTIVES:

 To uplift the weaker-sections of the society in rural and urban areas, through comprehensive Community Development programmes.

 To enhance the literacy status of the women through the functional literacy programmes and also to undertake programmes to improve the school

 To develop and implement the community based health care programmes for women, supported by a referral service and to provide medical relief to the poor, distressed, mentally and physically challenged persons.

 To undertake basic Research programmes on problems related to the development of the downtrodden of the society.

 To enhance income levels of women and unemployed youths through income generation programme.

 To encourage the youth groups to take initiatives on their own for the development of their community.

 To create environmental awareness to protect, preserve and promote Eco- environment balance for harmonious living with nature.

WOMEN EMPOWERMENT PROGRAMME – II APRIL 2016 TO MARCH 2017

Programme Context

PDI has implemented a Women Empowerment Programme in selected five villages of Tiruchirapalli District and in selected five slums in the city corporation of Tiruchirapalli (Tiruchy), during Aug. 2008 to Dec. 2014. Earlier, it also implemented an Urban Development Programme in selected ten slums of Tiruchy City during Aug. 2003 to July 2008. Both the projects aimed at empowerment of women in Tiruchirapalli District of with emphasis on the interactions between rural and urban poverty and also on the need to overcome gender discrimination as the first step towards development. PDI with the support of IMPLICADAS had implemented In order to serve the needy with gender based development approach and to replicate the successful initiatives taken in Tiruchy Slums, PDI had proposed to implement the programme in new target villages/slums of Tiruchy.

The staff team facilitated the Participatory Rapid Appraisal in the identified new villages and slums and developed a proposal for the implementation of a project, namely, Women Empowerment Programme (WEP). This project has been planned ford a five-year period from Jan. 2014 to Dec. 2018, in the following five slums and five villages in Tiruchy District with a total population of 16514 inhabitants (8150 men & 8364 women).

EDUCATION

Supplementary Education

During the reporting period 263 Children (Boys: 111 Girls: 152) attended the supplementary classes in both the urban slums (Boys: 53 Girls: 68) and rural villages (Boys: 59 Girls: 83). Supplementary Education Class at Parent – Teachers Meeting Andakondan

During the reporting period, 410 Parents-Teachers Meeting were organized in the field for the supplementary children in all the areas, both rural and urban. In total, 405 (75 men & 330 women) Urban 176 (1 man; 175 women); Rural 229 (74 men: 155 women) attended the meeting and the progress of the children was shared with their parents. The parents were motivated to send their children to the supplementary classes regularly and all parents were requested to attend the meeting to know their progress.

People’s Development Initiatives (PDI) 2 Annual Report 2016-2017 Non-Formal Education Programme During the year 24 adults (3 men; 21 women) got educated under this programme. The non- formal beneficiaries were given education to write their names and their family member’s names, to read simple words; they were taught with help of the books.

Summer Camp 2016 During the reporting period a three-day residential summer camp programme was organized to the supplementary children from our slums and villages at Periyar Memorial Girls Higher Secondary School, Children Displaying their creating at the Tiruchy. 87 children from slums and villages summer cam participated in the summer camp. The following classes were conducted to the children in the summer camp - yoga, drawing, hand work, greeting card making, key chain in fur cloth, handicraft class, hand writing class, menstrual hygienic training and gender sensitization programme to

Adolescents. The summer camp was Drawing Class in Progress at Summer Camp inaugurated by Ms. Usha, District Social Welfare Officer; Mr.Mangaleswaran, Assistant Professor, Department of Social Work, Bharathidasan University, Tiruchy was the special invitee of the programme. The highest scorers in the 12th standard board exams were honoured.

People’s Development Initiatives (PDI) 3 Annual Report 2016-2017 Observance of Yoga Day

The International Yoga day was observed by PDI on 21st June 2016 at Kallakampatty Higher Secondary School, Kallakampatty. 317 students participated in the yoga programme. Mr. Kanagu, Yoga Master, delivered the special address and explained the importance of yoga, Aasanas and Meditation.

Community Programme

During the reporting period, one community development programme to the supplementary students was organized by the students of Holy Cross College, Tiruchy. Totally 38 women from Samathuvanagar participated in the awareness programme on “ De-addiction”; Mr. Muthukumar, Counsellor in SOC-SEAD facilitated the programme.

II. HEALTH INITIATIVES

ANC & PNC Care Initiatives

During the reporting period, 184 ANC women and 229 PNC mothers were interacted by the staff and 184 ANC and 229 PNC mothers were immunized

Medical Camps:

During the reporting period, four medical camps were organised and the details are as follows:

 One General Medical Camp was organized with Child Jesus Hospital, Tiruchy at Pudhupatty. Totally 149 people were the beneficiaries of the camp (62 men; 87 women). Out of these 18 people were referred for various treatments to the Annal Gandhi Memorial Government Hospital. Dr. Sakthivel and Dr. Karthik facilitated the camp.

 One Special camp was organized to the women at Andakondan to diagnose the bone density. Totally 100 women were screened and 67

 One eye camp was organized with Rotary Club Hindu Mission Hospital, Tiruchy. Totally one man and 17 women benefitted, of Community Medical Camp in Progress which 13 were referred to hospital for further treatment. Two people underwent cataract operation and two people were referred for wearing spectacles.

 One hearing camp was organized with the support of GVN Hospital, Tiruchy. Totally 15 persons – 6 men and 9 women - attended the camp; Of these, eight people were referred for hearing aids; these were also supplied free of

cost, by the Rotary Club of Bone Density Diagnoses Camp at Shakthi, Tiruchy. Andakondan

Cancer Awareness Programme

During the reporting period, three cancer awareness programmes were organized at Andakondan. Totally 65 women attended and learnt on the symptoms of cancer, types, causes and treatments available for cancer.

Gender Sensitization to women

During the reporting period, five gender sensitization meetings were organized at Adaikampatty, Adaikampatty Annanagar, Andakondan, Karumalai Pudhupatty, and Karaipatty. Totally 92 SHG women attended and

People’s Development Initiatives (PDI) 5 Annual Report 2016-2017 discussed the gender equality. They were given sensitization on gender, gender discrimination and gender equality.

Community Health Volunteer Training:

During the reporting period, community health volunteer training programme was organised to the Community Health Volunteers (CHVs). The session continued for two days. The training was conducted at PDI Cancer Awarness in association with Office in Thuvarankurichi. Ms.Jeyalakshmi Harshamithra Cancer Foundation in Progress facilitated the training programme and educated on communicable and non- communicable diseases, about ANC & PNC referral and vaccination. They play the role of Change Agents in the community. They work along with the animators and refer the sick people to the nearby health centers. They help the team in organizing health campaigns and camps. Thirty health volunteers are working in the target slums and villages.

Women Empowerment Initiatives

Promotion of Self Help Group

During the reporting period, totally twelve Self Help Groups were formed (Urban-4 Rural-8) with a total membership of 216 women ([Urban: 59; Rural 157) members and Savings Bank Accounts for the groups were opened in TDCC Bank Tiruchy, Indian Bank, Microsate Branch (satellite branch for micro loans), Tiruchy and TDCC Bank, Thuvarankurichi for all the SHGs. The total amount of savings amounts to Rs.4,72,400/-

SHG Orientation:

Eleven SHG Orientation Programmes were organized at urban slums and rural villages. Totally 209 women attended and gained knowledge on SHG formation, number of members, roles & responsibilities, how to prepare and write

People’s Development Initiatives (PDI) 6 Annual Report 2016-2017 resolutions, age limit, procedure for internal lending, process of getting bank loans and repayment procedure, procedure for organizing the SHG meetings, maintenance of accounts book and importance of auditing of accounts.

International Working Women’s Day 2017

PDI organized a signature campaign on March 25, 2017 at Thuvarankurichi on the observance of International Working Women’s Day. The theme of the day is to create awareness on violence against women and around 215 people supported and signed. The campaign was organised in two Signature Campaign at Thuvarankurichi in Progress main places of Thuvarankurichi - bus stand and weekly market.

My Name is Nagarathinam aged 46 years residing at Samathuvanagar. I am a member of Nakshatram Self Help Group functioning in my area. I am selling idlies during morning hours and idly flour also. I have one son and daughter. My husband is a daily wage earner. During evening hours, I make and sell garland with flowers. I have very poor hearing capacity. I was identified by the PDI team and referred for hearing test at GVN Hospital, Tiruchy and after that I got free hearing aid machine. Now I have good hearing, thanks to PDI.

Voices

I am Muthumani residing at Kuppankulam. I am a member in the Self Help Group functioning in my area. I am selling old iron scraps and tools as my business. I got Rs.3000/- as internal loan for my business development. I have vision problem and was identified by PDI team and they referred me for eye test and the doctor advised me to wear spectacles. I am now wearing it. Part of the amount only was paid by me for the spectacles. Thanks to the PDI team to motivate us to take care of our health.

My name is Gowtham aged 12 years residing at Kuppankulam. My parents are daily wage earners and I am deaf from my birth. My mother is in the Self Help group. I am going to special school. I was taken to Hindu Mission hospital by PDI staff and now I got hearing aid. I am happy to hear and I am much thankful to PDI.

Voices

I am Mahalakshmi aged 41 years residing at Karumalai. I am an Illiterate, I don’t know to even write my name; wherever necessary I use thumb impression. I am a member in the Self Help Group functioning in my area. I always used thumb impression in the registers maintained by SHG. I was identified by PDI staff and now am learning through Non- Formal education. I can now write my name; read and write some basic words; all this was possible after attending non-formal class taken by PDI animator in our area. I am very thankful to PDI for this.

People’s Development Initiatives (PDI) 8 Annual Report 2016-2017 WOMEN SUPPORT CENTRE - TIRUCHIRAPALLI APRIL 2016 TO MARCH 2017

During Jan. 2010, Implicadas has awarded PDI a cash award of 2750 Euros to appreciate the services of PDI with the marginalized communities. Implicadas and PDI have jointly decided to use this amount to establish and operate a Women Support Centre in Tiruchirapalli. Implicadas has transferred the amount to PDI on 9th February 2010 and PDI has started its activities through Women Support Centre from 8th March 2010. The major goal of the Women Support Centre is “To assist the women under crisis and guide, educate, and Counsel and empower the women to cope up with the issues and to face the problem with the help of legal system.”

The women approached the PDI - Women Support Centre for matrimonial issues & Domestic violence [violent behavior by mother-in-law and harassment by husband]. During the reporting period, the number of women who approached the Women Support Centre is as follows.

During the reporting period, thirty-eight clients approached the Women Support Centre through various sources; one client was assisted to approach police station; sixteen clients were referred to the Free Legal AID to settle their legal issues; and two clients were under follow up.

Free Legal Camps

During the reporting period, four free legal education camps were organized with the support of District Legal Services Authority, Tiruchy at Kuppankulam, Jailpettai, Beemanagar and at Project Office of PDI for MSM/

People’s Development Initiatives (PDI) 9 Annual Report 2016-2017 Transgender. 197-women from different slums attended the camps. Advocates Ms. Shanthi, Mr. John Britto, Mr. Navaneedha Krishnan, Ms. Sudha, Mr. Elangeeran and Ms. Geetha, Secretary/Sub-Judge, District Legal Services Authority and Mr. Subramani, District Judge, Permanent LOK Adalat, Tiruchy facilitated the camp and elaborated the basic rights for women, Importance of birth & death certificates, Importance of registering marriages, and on the legal measures available for the women to combat against domestic violence.

PDI-Women Support Centre observed “International Day for the Elimination of Violence against women - 2016” in collaboration with District Legal Services Authority, Tiruchy at Kanaga Middle School, Thennur, Tiruchy.

Jesintha Martin B.L., District Sessions Judge, Mahila Court Tiruchy Thirumigu C.Karthika, M.A.B.L., 3rd Additional Sub-Judge, Tiruchy was the special invitee of the day. Ms. Geetha B.L., Secretary/ Sub-Judge, District

Legal Services Authority, Tiruchy, welcomed the gathering.

Ms. Jaibaby, Coordinator, briefed the activities of the Women Support

Centre and the importance of observing the day.

While speaking, Thirumigu Kumaraguru emphasized the importance of educating the girl children - the mother should frankly discuss the social issues so that the girl child would realize exactly what is violence and also every woman must be self-protective and self-defensive to safeguard themselves. It is important for every woman to know the laws which will protect them. Some case studies were also shared with the gathering.

People’s Development Initiatives (PDI) 11 Annual Report 2016-2017 FEMALE INFANTICIDE PREVENTION PROGRAMME - SALEM APRIL 2016 TO MARCH 2017

Staff Team Female Infanticide Prevention Programme

Programme Background

PDI with the support of IMPLICADAS has been working in four prioritized Blocks namely , Omallur, Konganapuram and Idapadi of Salem District since Sept.2007. It is a programme aim to eliminate the practice of killing the female child and to bring about attitudinal change among the parents to be gender-sensitized to bring up their girl child with due recognition. The staff team comprises of eight Field Officers and Two Counsellors. The goal of the project is to sensitize the whole of the population in the identified four blocks to be gender-sensitized and to protect the girl child from infanticide. The project takes efforts to sensitize the community in general and the family members of ante-natal women in particular on the importance of girl-children. The project counselors counsel the ante-natal mothers in solving their issues so as to protect their female children from killing by another family member. It brings attitudinal change among the spouses and the in-laws of the mothers at risk to protect the female infant.

People’s Development Initiatives (PDI) 12 Annual Report 2016-2017 The field level staff educate the women in Self Help Groups on gender issues. The youth population in the target blocks is trained on gender issues and on life skills. The project organizes skills training programmes on various trades to the women and motivate them to become self-employed. The major activities carried out in the project during the reporting period are as follows.

Collecting the List of Ante-Natal Mothers

The project has been collecting the list of anti-natal mothers in the selected four blocks, since start of the first year. A database has been created with the information collected and the counselors also meet them regularly till the delivery of the child. In case of female children, they follow the family for eighteen months period even after the delivery. During the reporting period, 2407 ante-natal mothers were enrolled and are under follow up.

Inter-Personal Communication

The field workers used to meet the entire ante-natal mother as per the list and educate them on the need for proper medical care from pre-natal to post-natal period. To start with, the field workers educate the women on nutritional status, One to One Communication by Out Reach Worker continue on hygiene and end the interaction with gender equality, particularly with greater emphasis against the female infanticide. 19,476 interaction sessions had been held during the reporting period.

Counseling Services to the Women at Risk

During the interpersonal communication sessions, the Field staff would take efforts to Interaction with Post Natal Mother

People’s Development Initiatives (PDI) 13 Annual Report 2016-2017 identify the women with the problems and refer them for counseling. The counselors would provide counseling services to the ante-natal mothers and for their husbands on the identified issues. 449 New counselling and 1877 follow-up counselling sessions had taken place with mothers at risk.

Sensitization of General Population through Street plays

The project has formed a Street Play Troupe; they performed plays in the villages on gender issues, importance of education to women and against the practice of female infanticide. The general population was sensitized on the gender issues. During the reported period, street plays were performed in twelve villages and nearly 5350 people were reached with the messages.

Life Skills Education

The trained staff conducted life skills education among 599 adolescents from the villages of the four blocks. Among them 307 were boys and 292 girls. The adolescents in the age group of 13 to 20 were motivated to attend the training. Gender issues were also included in the life skills programme and the adolescents were sensitized on key gender issues. The programme is conducted during the evening hours for 2 days and lasts for 4 hours.

Self Help Groups for Mothers at Risk

The project has identified women with girl children and formed 151 Self Help Groups with a membership of 1812 women. Mothers with girl children were given priority while forming the Self Help Group. Their group savings is Rs.77,00,228/- and during the reported period 12 SHGs have been started.

Family Dialogue

The mothers who were identified as risk were often under crisis and they feel hopeless as there is no one to support them. The problem can be from their family members, in such cases in order to support the mother at risk, family dialogues are conducted by the staff team. During the family dialogue sessions following issues were addressed:

 Preference of male baby by husband and mother-in-law

 Restrictions for family planning

 Poor health due to repeated deliveries

 Advising the husband to undergo family planning

The counselors along with the staff had met 187 families and conducted family dialogues to support the desperate women.

Women Support Centre

During June 2009, the Women Support Centre was formed in order to assist the women under crisis. The Women Support Centre consists of an

People’s Development Initiatives (PDI) 15 Annual Report 2016-2017 advocate, a feminist journalist, and the project leader of Female Infanticide Prevention programme. The advocate used to sensitize the women on Domestic Violence Act 2005, rights of women to inherit property, and on gender issues. Three legal education sessions were organised in the village itself. One case was taken up by the team and addressed. The women support centre provides legal guidance and counselling to the victims. The women support centre addressed the following types of issues: a) Family dispute b) Land dispute c) Guidance for educational loan d) Violence by men to SHG women e) Illegal sale of alcohol and subsequent problems f) Guidance for getting government welfare for spastic child g) Guidance to get compensation for accidental death

Gender Clubs – SAMAM

Adolescents who hail from the target blocks in the age group of thirteen to twenty were trained on life skills and were grouped into Gender Clubs. PDI has formed forty Gender Clubs during this reporting period; these clubs are named as “SAMAM” which means Equal in Tamil language. The objective of forming Gender Clubs is that these groups will serve as volunteers in the target area and the Gender Clubs are to raise the awareness level of youths on gender issues and to bring up them as gender-sensitized citizens. The project also plans to use them in preventing the practice of female infanticide.

International Working Women’s Day – March 18th 2017

The International Working Women’s Day was observed on 18th

Signature Campaign Dispaly March 2017, at Vellandivalasu of Idapadi Block. PDI and the women of Self Help Groups promoted by PDI joined hands in Signature Campaign on Working Women’s day 2017 organizing the programme. A signature campaign was also conducted where the public signed the banner displayed to endorse their solidarity in fighting against gender based violence.

Field officer met her during her third pregnancy. She showed interest to have one more male baby. Staff sensitized her on gender issues and referred her for counseling. Easwari and her husband both were counseled. She delivered a girl baby; motivated by the staff, she has undergone family planning.

Mariyammal, aged 34, lives in Chettimankurichi of Idapadi block. Her husband is Kandhaiyar and both work as agricultural labour in sugarcane field. They already have two girl children, ten and six. Staff came to know about them through their neighbours and when she was asked about it, she got angry and refused to talk with the outreach worker. During the second visit, the field staff along with the counsellor met her and spoke to her. During this interaction Mariyammal talked with the staff and it was found that she has not gone for check up and now at her sixth month of pregnancy, she had not even registered it in Anganwadi. When counsellor questioned about it, she stated that first they want to know the sex of the baby and only then they will decide whether to continue with the pregnancy or not. She was counselled and advised to get registered in PHC and also motivated to get vaccination and nutritious food. During the third visit, she cried when she saw the staff, and said that they have undergone scanning at nearby Namakkal and now they knew it is a girl baby. Her husband was also at home during that visit and hence both were counselled and sensitized on gender. They were explained on the risk of abortion; staff accompanied them to Primary Health Centre. She was regularly followed up. She delivered and was motivated for family planning. During the next visit, she was found to be in a happy mood - she had undergone family planning. The baby is now four months old and they will be under our follow up for another fourteen months.

Krishnaveni is 26, and her husband is Sakthivel. They live in Chinnakadampatti village of Omallur Block. Both are daily wage labourers. They already have a 6-year old daughter. Staff met her under the impression that it is her second pregnancy. During the interaction,

Next one is the 6-year old girl; after that, she had aborted the third one at 5-months when she came to know that it is a girl. So, now it is her fourth pregnancy. She came out with her past when the staff informed her about the government scheme for those who have two girl children. She questioned whether she is eligible for the scheme. She also said that her family is not interested in the government scheme for girl children and they would rather have a boy baby. She as well as her husband were kept under counselling; the family members were also met by the counsellor and addressed on gender through family dialogue. Krishnaveni delivered a girl baby. Staff motivated them for family planning which she did; they are now under our follow up.

Govindammal. 24, lives in Chinnappampatti of Tharamangalam block. Her husband is Murugan, 34. Both are employed in handloom. They already have two girl children, 7 and 4. The staff met Govindammal at her third pregnancy and kept her under regular follow up. All the family members were expecting her to deliver a boy baby. They were all addressed through family dialogue. Right from the beginning the couple was motivated for family planning irrespective of the sex of the third baby. Govindammal delivered a girl baby and they were planning to give the baby for adoption. This was informed to the staff by a neighbor. The outreach worker and counselor rushed there and talked with the family. The mother in law of Govindammal stated that they took that decision because their son, father of the baby has got paralyzed fifteen days before and they have no other way rather than giving for adoption. When they were questioned what they would have done if it was a male baby they could not answer. Govindammal was counseled, the entire family was addressed and as they were stubborn the staff explained them about legal adoption and with their consent informed Life Line. The next day staff met them once again counseled them and also informed them about the procedures. The following day, again the staff went there and met Govindammal, she was very happy then and stated the staff that their effort has changed the attitude of her family members and they have decided not to give up the baby for adoption. Now Govindammal is under follow up and been motivated for family planning.

Staff Team - Targeted Intervention Programme, Salem

Background of the Project

PDI has been implementing the Targeted Intervention Programme among the MSMs since July 2004 in Salem District with the support of Tamilnadu AIDS Initiative (TAI) and Voluntary Health Services (VHS) Chennai. The Programme is being implemented among 2000 Male Sex Workers in Salem District. The MSMs include Transgender, Kothi, and Double Decker. On 1st July 2012, the project got transitioned to Tamil Nadu State AIDS control Society (TANSACS). After transition TANSACS re- allocated to PDI the working area of the three TIs at Salem during Sep. 2012, and PDI started working in four blocks and Corporation of Salem with a population of 1400 FSWs and MSMs/TG. Till March 2016, the project works with 1536 Core Group Population (MSM -110, FSW – 435).

The staff team comprises of a Programme Manager, Six Outreach workers, one M&E cum Accountant, one ANM/Counsellor. The intervention has one stand-alone clinic with a Medical Officer. There are 24 trained Peer Educators who do out-reach services such as educating their peers on STI/HIV/AIDS, distribute condom and lubricant and refer their peers to STI Clinics.

Activities carried out during 2016-2017

Peer Education

Peer Educators plays a vital role in taking messages through outreach. They held responsible for educating a minimum of sixty of their own peers on STI/HIV/AIDS and the need for medical checkups. They share the messages informally to their friends. The peer Micro Level Planning by Peer Educators educators introduce their peers to the project and vice versa. The peer educators are expected to document all their activities including their referrals to STI Clinics & ICTCs and the number of condoms distributed. They meet the project staff every week to share their progress. The project team reviews their activities and mentors them for effective functioning. Cluster level review of Peer Educators and overall review-cum-training is being conducted at regular intervals. Twenty-four Peer Educators have been identified and fifty review and planning meetings were conducted. During the reporting period, four Peer Educators were newly recruited and three induction trainings were conducted. The project team facilitates in-house training programmes for the peer educators periodically.

The Demand Generation Meetings are helpful in identifying new core group members and to motivate the lost to follow up members to access the services regularly. The Peer Educators are capacitated in such a way to conduct group interactions at hotspot level with the support of ORWs. Fifteen to twenty Demand Generation Meeting in Progress community members used to participate in the meeting. Sixty-two meetings were conducted in the hotspots and 847 communities participated in it.

STI Care Services

One of the important strategies of the project is STI care. Under this strategy, the outreach workers and the peer educators educate the core group population on symptoms of Sexually Transmitted Infections, need for the treatment and its details, and on the availability of the treatment services. Followed by the interactions, if they found anyone saying that they have problems, they will refer them to the STI clinics either attached to the Government Hospitals or to the project owned clinic. Even otherwise, if the community says that they don’t have any problem, when a new member is identified they will be referred to the clinics for presumptive treatment, as they are vulnerable to the infections. After this presumptive treatment, the core group population is expected to undergo regular medical checkups once in thee months.

The female sex workers are encouraged to access the services in NCD facility of government centres where they could also access the screening facility for cervical cancer. During the reporting period, seventy-three individuals enrolled as First visitors to the clinic and all the 1536 members of the community have availed one time STI Care. 1534 individual core group members came for regular medical check ups (RMC) and the quarterly breakups are as follows. Motivating the core group members for HIV testing and taking care of the PLHIVs are the most important activities of the project. The project motivates every individual to undergo HIV testing once in a six months period. The individuals found HIV-positive are encouraged to access the counseling services at the project. The counselor counsels the PLHIV, motivates them to enroll for pre-ART, guides them for regular CD4 Tests, and supports them for emotional ventilation. The counselor and the respective outreach workers do follow-up activities and provide social support to the PLHIVs. During the reporting period, six MSMs have been tested positives and they have got enrolled with ART. In total, fifty eight PLHIVs are being followed up by the project team right from the beginning of the project.

Community Mobilization

The community mobilization towards making the programme sustainable is an important activity of the targeted intervention project. The project takes efforts to mobilize the community of core group members to take up the programme on their own even after the withdrawal of the project activities. It has formed committees for Induction Training to Peer Educators in progress accomplishing different tasks, and also to capacitate the community to take up the initiatives on their own. Project Management Committee, DIC (Drop-in-Centre) Management Committee, Condom Monitoring Committee, Crisis Response Committee andSTI Management Committeehave been formed by involving community as members in the committees. These committees bring the community closer to the programme activities and the project team handholds them to learn the day to day activities of the project team. The committee members meet once in three months to help the project team for troubleshooting and also to give guidance for effective implementation of the programme activities. Four such meetings were conducted during the reported period.

The project regularly organizes meetings twice in a month at the Drop In Centre. Thirty-two meetings were conducted during the reported period. This activity is also helpful in making the Drop in Centre lively, in mobilizing the community together, in increasing the inflow to the clinic, and in identifying the hidden talents of the HRG.

People’s Development Initiatives (PDI) 23 Annual Report 2016-2017 As part of the project initiatives towards mobilizing the community, the project organizes community events atleast twice in a year.

Community event for the year 2016 – 2017 was conducted for FSW and MSM/TG on 25th Sept. 2016 at Kalaingar Kalyana Mandapam, Kottai, Salem. It was a mass gathering of HRGs as a “Community Get Together - 2016.” Mr. Selvam, District Programme Manager, DAPCU-Salem, Mr.Johnpal, TB Unit-Salem, Mrs.Jeeva-amma, Mrs.Vijaya-amma were the guests of honour. Nearly one hundred and twentymembers of core Community Event for MSW (TG) RSW group population participated in the event. Various competitions were conducted and community culturals also held. Mr. Johnpal explained to the community Tuberculosis with Awareness messages on symptomsand the testing process.

The second community event for the year 2016-2017 was conducted for FSW & MSM/TG on 28th March 2017 at Semmandapatti Panchayat, Kaadaiyampatty block.It was a mass gathering of Core Group Population. The event is important in the way that it observed World Tuberculosis Day too. Mr.Chokalingam, Block Development Officer, Kaadaiyampatty, Community Event for FSW MSM / TG and Mr. Gopalakrishnan, District Programme Manager -DAPCU Salem, were the guests of honour. Nearly one-hundred and three members participated in the event. In the both events, the Mobile ICTC is accessed by the community members who are due to test. In Kaadaiyampatty, an eye camp was also organised with the support of Aravind Eye Hospital, Salem. Games and other talent shows have also been organised where the members actively participated.

The project takes regular efforts to involve all the stakeholders who could play effective roles in the project implementation. It organizes advocacy meetings with different stakeholders to introduce the programme components to them and also to secure their support for the programme activities. It provides a platform for both the project team and other stakeholders to exchange their views over the programme. Particularly, the project staff share the challenges faced by them in programme implementation. On 16th September 2016, an advocacy meeting was conducted with NCD (Non-Communicable Diseases) Staff at Dhadhagapatti PHC. The Staff of the NCD departments of and Dhadhagapatti Primary Health Centres attended the meeting. The project team sought the guidance of the participantsto make all the core group population to undergo HPV testing.The staff also assured that they will refer the community members for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke (NPCDCS);there were thirty-three participants.

On 17th Sept. 2016, an advocacy meeting with the NCD staff of Sarakkupillaiyur and Kaadaiyampatty Primary Health Centres has been organised at Omallur Government Hospital; Twenty staff of NCD participated.

On 14th March 2017, an advocacy meeting was held with retailers owning the petty shops, pan shops, and fancy stores located in Bharatha Rathna M.G.R. Central Bus Stand(New Bus Stand). It is to persuade the retailers to promote priced condoms through their retail outlets. New Bus Stand is one of the important Hotspots of the project, as significant number of core group population gathers there to solicit their partners. Selling condoms in these shops would increase the accessibility to get condoms by the core group. Hence, an advocacy meeting with retailers assumes importance. Nineteen shop-keepers participated in the meeting and agreed to promote condoms in their outlets.

An overview about the project was given and the activities of Targeted Intervention Programme were briefed. The discussions focused on the issues being faced by female sex workers, MSM/Transgender communities in the hotspots through various power structures, and how it affects the disease prevention activities. The Assistant Commissioner of Policebriefed on the available services of Police Advocacy Support Services (PASS). The serviceincludes individual, couple, and family counseling, critical incident stress management, education and training, support/therapy groups, and mental health response for major disasters. Two-hundred and twenty-fivepeople participated.

Advocacy with Auto Drivers

On 30th March 2017, an advocacy meeting with auto drivers was conducted for Bharatha Rathna M.G.R. Central Bus Stand, which is popularly known as New Bus Stand. The auto drivers at the bus stand are familiar with many of the community members. The project team organised this meeting with auto drivers to orient them on programme activities and to secure their support for effective implementation. TARGETED INTERVENTION PROGRAMME - THANJAVUR DISTRICT APRIL 2016 TO MARCH 2017

Staff Team - Targeted Intervention, Thanjavur

Background of the Project

PDI has been implementing a community STI/HIV/AIDS Prevention program among the High Risk Group including Males Having Sex With Males (MSM) and Transgender, since April 2007in Thanjavur District with the support of Tamilnadu AIDS Initiatives (TAI)and Voluntary Health Services (VHS), Chennai. As per the policy of the Government, the project was transitioned to the government and now the project is being supported by Tamilnadu State AIDS Control Society(TANSAC) from April 2012.Present report details the activities carried out during April 2016 to March 2017.

The project covers 1313 (155 Transgender / 1158 MSM) High Risk Population. The staff team includes one Programme Manager, One Physician, One Monitoring and Evaluation Office-cum-Accountant Assistant, one ANM/Counsellor, five Outreach Workers, and, twentyPeer Educators from the core group population support the project team for effective community outreach. They provide services such as educating their peers on STI/HIV/AIDS, distribute condom and lubricant, and refer their peers to STI Clinics and motivating them to undergo for HIV Test in ICTCs (Integrated Counseling And Testing Centers) once in six months.

The major objectives of the programme are to bring behavior change among the MSMs and transgender, particularly with focus on disease prevention and quality of life,

The project also works to increase the health-seeking behavior of the key population and to reduce the burden of sexually transmitted infections. The project has been implemented in fourteen blocks of Thanjavur district, namely, Thanjavur, Thiruvaiyaru, Budalur, Papanasam, Ammapettai, Kumbakonam, Pattukottai, Peravurani, Orathanadu, Thiruvonam, Thirupananthal, Thiruvidaimaruthur, Sethubavachatiram, and Madhukur.

STI Treatment Services

The vulnerability to sexually transmitted infections is relatively higher to the core group population as they are very active in both casual and commercial sexual relationships.Even though majority of them get educated on consistent and correct condom usage, they are motivated to undergo for STI test once in three months. The project team has good rapport STI Committee Meeting to Promote Quality STI Services with the STI Clinics in all government centers and refers the identified persons to the STI clinics. During this period 4481 people have undergone regular medical check-ups. They have been motivated for regular medical checkups also. The peer educators take the responsibility of motivating their peers to undergo regular medical checkups. Each community member is expected to visit STI clinic once in three months in the specific time. The peer educators take effort to ensure the periodical visit of community members to STI Clinics on time. Besides they are also expected to visit ICTCs, once in six months to undergo HIV Test.The peer educators motivate their peers for their regular referral to ICTC Centers attached to government hospitals. During the reporting period,among 1250 community members 1173 visited ICTCs twice and they had undergone HIV testing through ICTCs attached to government hospital. Efforts were taken to register the infected key population to ART centre and they are regularly being followed-up. During the reporting period, four community members living with HIV were registered in ART centre.

Peer education plays an important role in bringing behavior change among the core group members, through interpersonal communication. Peer Educators in this project are the change-agents of their own peers, particularly changing the unsafe behavior of key population into safer behavior. The peer educators are being selected by the peers themselves with the guidance of staff team. Peer Group Exercise during Peer Educator Training Educators play a vital role in outreaching the activities. Block Level, Drop in Centre Level and the Project Level review meetings were regularly conducted by the project team to review their activities and to motivate them for further strengthening of the activities. The Peer Educators were well capacitated on micro-level planning. The review meetings helped the project team to find out the gaps in their services, to identify the risk behavior and plan for their peers. For the period from April 2016 to March 2017, 20 Peer Educators were attached to the programme who attended 46 review meetings at Thanjavur Project office. In order to make the programmesustainable with greater involvement of the community members, the Project has formedProject Management Committee, DIC (Drop-in-Centre) Management Committee, Condom Monitoring Committee and Crisis Response Committee.The core members take the responsibility of managing committees andthe staff mentor the community to take up the initiatives.These committeeseventually bring the community closer to the programme activities andthe project team handholds them to learn the day to day activities of the project team. The committee members meet once in three months to help the project team for troubleshooting and also to give guidance for effective implementation of the programme activities. Besides, community events have

During the reporting period, the project has conducted community events programme in various places including Pattukottai and Thanjavur. The core group population is encouraged to show their talents in cultural activities through the community events. A minimum of 250 community members attended the event.

Advocacy meetings

The project takes efforts to persuade all the stakeholders who directly or indirectly influence the core group population. It lobbies with them at different levels to advocate the common issues of the programme. The advocacy meetings are being organised with them to make the programme more effective. The stakeholder meetings give them a clear picture on programme activities. The jhamadh leaders, pimps, lodge boys, auto drivers and other power structures have greater influence over the core group populations. They could support the programme initiatives, once they could understand its importance and the benefits. If we take the MSM and Transgender, the Jamadh leaders have greater influence on the decision made by the Advocacy Meeting with STI Counsellors individual target community. Such secondary stakeholders have greater role in creation of enabling environment and held responsible for the effective implementation of the programme. The project organized advocacy meeting with the Jamadh leaders to help the staff for mobilizing MSMs/Transgender. The other stakeholders including auto drivers, lodge boys, pimps, and brokers are also invited to the stakeholders meeting to be oriented on the different programme activities and its intended objectives. Help the project team to persuade their support. The service providers including the physicians and counselors also have a greater role in disease prevention and control of HIV. The meetings with the physicians and the ICTC counselors’ team are being organized for smooth referral services. The project motivates the stakeholders to get involved in the programme activities through organizing the meeting five times in a year.

During the reporting period,four advocacy meetings were conducted for the counselor of ICTC and STI, lab technician, Jamadhleader, retailers, and various government officials who attended the meetings indifferent places. In total, eighty-six

People’s Development Initiatives (PDI) 31 Annual Report 2016-2017 stakeholders attended the programme and discussed on improving STI access and ICTC, RPR screening and other programme activities. However, the advocacy meetings concentrate more on finding possible ways to get Social Security Schemes to the core group population.

Demand Generation Meeting

The key populations along with the peer educator meet at the hotspot to discuss their issues particularly on the basic facts of STI/HIV/AIDS.The outreach workers with the help of peer educators facilitate the meeting. Communication materials focusing on STI/HIV awareness, addressing violence and promoting quality of life were used during the demand generation meetings. The meetings are useful to make the community accessing the service of the project without fail. Ten to fifteen people will be gathered in every meeting. During the reporting period, 46 Demand Generation Meetings were facilitated in which 1250 key population attended.

Every year, Government of Tamilnadu organizes an Exhibition at Corporation Ground of Thanjavur City Corporation. This year also, they have organised the exhibition from 10th September. The authorities from District Social Welfare Board have invited PDI to run a food stall in the exhibition to sell the products by Transgender Community. PDI has informed the transgender and motivated them to take part actively. The stall received significant support from the public and other Government officials who participated in the inaugural function.

World AIDS Day

On 1st Dec. 2016, The World AIDS Day was observed by the project with Peer Educator and HRGs. A signature campaign has been organised at New Bus Stand, Thanjavur. The Staff team of PDI and Peer Educator distributed the IEC Material tothe common public. Five hundred people have participated in the signature campaign to express their solidarity in combating the stigma and discrimination against HIV/AIDS. Community Event on World AIDS Day

Background of the Project

PDI has been implementing Targeted Intervention Programme in selected seven blocks of Tiruchirapalli District, since Jan. 2009 which is supported by Tamilnadu State AIDS Control Society (TANSACS), Chennai. The project team currently works with 1268 female sex workers (FSWs) and 611 male MSMs. This project is being implemented in seven blocks Including Uppiliyapuram, Thuraiyur, Thottiyam, Thathaiyangarpettai, Musiri, Manachanallur, and Pullambadi. The major objective of this project is to educate and to bring behavior change towards practicing safe sex, among the core group population, namely, female sex workers and MSMs/Transgender, and motivate them to undertake treatment for STIs in the clinic attached to the project and in the Government Hospitals. Another important objective of the programme is to improve the quality of life of the Core Group Population and thereby reduce the risk of getting infected. Hence, the project works for disease prevention as well as takes effort to improve the quality of life of the target group. The staff team consists of one project manager, one counselor, one monitoring and evaluation officer, seven

Peer Educators in this project are the change-agents of their own peers, particularly changing the unsafe behavior of key population into safer behavior. The peer educators are being selected by the peers themselves with the guidance of staff team. Peer Educators play a vital role in outreaching the activities. Block Level, Drop in Peer Educator Meeting in Progress Centre Level and the Project Level review meetings were regularly conducted by the project team to review their activities and to motivate them for further strengthening of the activities. The peer educators were well capacitated on micro-level planning. The review meetings helped the project team to find out the gaps in their services, to identify the risk behavior and plan for their peers. For the period from April 2016 to March 201, 28 Peer Educators were attached to the programme who attended 30 review meetings at Tollgate Project office.

ICTC Referral Services and Care for the PLHIVs

The Project Team takes efforts to refer every member of core group population to undergo HIV test to know their status. In each block, there is one ICTC (Integrated Counseling and Testing Centre) attached to the Primary Health Centre and the staff team established good rapport with them. During the reporting Period, 4097 core

People’s Development Initiatives (PDI) 35 Annual Report 2016-2017 group members have been referred to the ICTC centers. The cumulative number of identified HIV positives among the target population is thirty-three. During the reporting period, only one new PLHIV/MSM got identified. The Project takes special care of the people living with HIV among the core group population. They are linked with ART Centres, and motivated to take health care in time. Theproject has greater emphasis on positive prevention to avoid any further transmission of HIV to their spouses or to their partners. Special messages on positive prevention are being sent to them through interpersonal communication sessions and through regular counseling sessions.Emotional support through counseling and nursing care through government hospital is also being provided by the Project.

STI-Related Services

An effective inter-personal communications session and group meeting usually ends with bringing about behavior change among the key population towards treatment- seeking behavior for their sexually transmitted Infections. The project team has good rapport with the Integrated Counseling and Testing Centres (ICTC) in all Government centers and refers the identified persons to the STI clinics. During this period FSW- 4834, MSM-2306 people have undergone regular medical checkups. The community members are expected to visit the hospitals, once in three months to undergo regular medical checkups for STIs. Nine female and three MSMs wereidentified with STIs and got treated. They have been motivated for regular medical checkups also. The peer educators take the responsibility of motivating their peers to undergo regular medical checkups. Besides regular referral to ICTC Centers, periodical screening for HIV was conducted by ICTCs at government hospitals. During the period 2515female sex workers, and 1158 MSMs have got tested for their HIV status through ICTCs attached to the government hospital.

Condom Promotion Activities

Condom promotion is an important strategy ensuring the safe sex practice among the core group. The core group members have been insisted on the need for correct and consistent usage of condoms during the interpersonal communication and group sessions. They were also trained on how to use condoms correctly. The project had distributed about 454,851 free condoms to the target community. A total number of

People’s Development Initiatives (PDI) 36 Annual Report 2016-2017 1890 condoms have been sold in different salesoutletspromoted by the project staff.During the reporting period, the project has promoted 15 sales outlets in the places accessible to the core group population.

Community Mobilization

The project takes efforts to mobilize the community of core group members to take up the programme on their own to sustain the programme activities, even after the withdrawal of the project activities. Community mobilization is being done to make the programme sustainable.The project has mobilized the community and formed two community based organizations, namely, Velicham Trust for female sex workers and Sesa Trust for MSM community. The Velicham Trust is very active in solving the issues of the community during a crisis. The board members of the registered trust were selected democratically by the members themselves. They do advocacy activities with the government officials, health care providers and other important stakeholders. Various committees like Drop-in-Centre Management Committee, Condom Management Committee, STI/ICTC Management Committee, and Crisis Response Committee have been formed by involving community as members in the committees. The committee members meet once in three months to help the project team for troubleshooting and also to give guidance for effective implementation of the programme activities. In order to encourage the community members, mass community events are being organised by the project team.

Health Camps and Community Events

On 24th Sept. 2016, a community event was organised at Chidambaram Pillai College for Women, Manachanallur. Dr. S.Savithri, Deputy Director, RNTCP, Tiruchirapalli, inaugurated the programme in the presence of Ms. Priya Govindaraj, Secretary, Rotary Club of Tiruchy Butterflies. Dr. Vijayalakshmi, Medical Officer, Manachanallur has spoken on Dr. Savithri, DD, RNTCP Addressing on Community Event

People’s Development Initiatives (PDI) 37 Annual Report 2016-2017 health issues affecting women. 182 Core Group Population have participated in the event and they accessed eye testing facilities, screening for tuberculosis, and ICTC services.

On 25th March 2017, a health camp was conducted at Utthamar Kovil Marriage Hall, Toll Gate. Ms.Subha Prabu, Manager, Joseph Eye Hospital Community Event on KTC Service inaugurated the camp. Dr. Yamini, Medical Officer, Harshamithra Medical Centre was the chief guest. 224 Core Group Population have participated in the event and they accessed eye testing facilities, screening for tuberculosis, and ICTC services.

Advocacy Meeting

The core group population has social contacts which are supposed to be very influential can positively or negatively influence the project activities with the primary target audience, namely, core group population. They are the vital stakeholders of the project who are responsible for the effective implementation of targeted intervention programme for the core Advocacy meeting with Stake holders in Progress group population. If we take the MSM, and transgender, the Jamadh Leaders have greater influence to take control over the community made by the individual target community. As they are elderly people of the community, they have the ascribed status of controlling the peer group members. Such secondary stakeholders have greater role in creation of enabling environment and held responsible for the effective implementation of the programme. The project organized advocacy meetings with the broker,Jamadh leaders, to help the staff for mobilizing FSW, MSMs, and transgender.

People’s Development Initiatives (PDI) 38 Annual Report 2016-2017 The other stakeholders including auto drivers, lodge boys, and pimps are also invited to the stakeholders meeting to be oriented on the different programme activities and its intended objectives. The stakeholder meetings give them a clear picture on programme activities and help the project team to persuade their support. The service providers including the various govt. department Priced Condom sales in Fancy Stores at Thuraiyur officials, physicians, and counselors also have a greater role in disease prevention and control of HIV. The meetings with the physicians and the ICTC counselors’ team are being organized for smooth referral services. The project motivates the stakeholders to get involved in the programme activities through organizing the three meetings in a year.

Demand Generation Meetings

The key populations along with the peer educators meet at the hotspot to discuss their issues particularly on the basic facts of HIV. The peer educators with the help of outreach facilitate the meeting. Communication materials focusing on STI/HIV awareness, addressing violence and promoting quality of life Dr. I.Ambalavanan, Project Director, Interacting with community at Manachanallur were used during the hotspot level meetings. The meetings are useful to make the community accessing the service of the project without fail. Ten to fifteen community members will be gathered in every demand generation meetings. The major objective of this activity is to introduce the programme components to the newly identified community members. Hence, preference is given to new members to attend the meeting. However, the community members who already accessed the services do attend the programme too. During the reporting period, 95 demand generation meetings were facilitated in different hotspots.

Catholic Health Association of India (CHAI) is an association of Catholic health care institutions ranging from small health centers to large hospitals, located in different parts of India. The organization promotes community health through its member institutions in the form of orientation and training programme.CHAI has successfully implemented a couple of TB projects, namely, ACTION PROJECT (Financial support from Results Fund USA), USAID TB Project (Through Union – Delhi)and in addition to this the majority of its member institutions are involved in implementing

RNTCP Schemes representing all over the country towards preventing and control ofTB for more than a decade. From October 2010 onwards CHAI implements The Project

Axshaya in Salem district through 4 Mother NGOS. PDI is one among the Mother

NGOs and implemented the CHAI Project Axshaya in Salem Corporation, Omallur and

Tharamangalam Block.

Axshaya Villages – Rangampalayam, Dasanayakanpatti

During the reporting year PDI implemented the programme in Konganapuram &

Panamarathupatti Block. Rangampalayam & Dasanayakanpatti villages are identified as vulnerable and it was prioritized as Axshaya Villages and in-depth outreach activities were carried out in the Axshaya village.

Goal:

To decrease morbidity and mortality due to drug resistant TB (DR-TB) in India and improve access to quality TB care and control services through enhanced civil society participation.

Target Group/Beneficiaries:

· Women, men and children

· Population living in poor & backward districts

· Population living in districts identified as predominantly tribal

· Population living in urban slums

Sensitization Meeting - GKS

During the reported period eighteen sensitization meetings were conducted in the rural slums of

Axshaya village – Rangampalayam &

Dasanayakanpatti, slum groups. 378 villagers are covered through GKS meetings.In the meetings, messages on TB awareness, information on testing, centres and treatment were given. TB Awareness Meeting in Progress

Intensive Outreach Activity (IOA)

IOA is a new activity carried out during the reported year. It is a door to door outreach activity for 1316 people to meet at their homes and were given messages on

TB. The staff did verbal screening for TB to them. Suspects were identified for testing and were followed up.

WORLD TB DAY - 2017

PDI in collaboration with CHAI - AXSHAYA conducted a mass awareness programme on 28/03/2017 at Semmandapatti of

Salem.

The programme started at about 12 noon. Then Ms. Subha, Programme

Manager - PDI, welcomed the participants and gave a brief on the project activities of

PDI and the CHAI-Axshaya programme. Then Mr. Chokkalingam gave the presidential address. He appreciated the project team and CHAI-Axshaya for adopting

Semmandapatti for their awareness initiatives by PDI on tuberculosis. Then Mr. Arul in his speech talked about the vulnerability of TB to the slum dwellers and he talked about symptoms of TB, methods of transmission and treatment. After that

Mr.Gopalakrishnan appreciated the efforts of the project and talked about world TB day.
