Swissair MD 11 SR 111 Investigation Report How This Report Is Organized This report was prepared in accordance with International Civil Aviation Organization standards and recommended practices,[*] and with Transportation Safety Board (TSB) standards for investigation reports. In keeping with these standards, the report is organized into the following main parts: § Part 1, Factual Information: Provides objective information that is pertinent to the understanding of the circumstances surrounding the occurrence. § Part 2, Analysis: Discusses and evaluates the factual information presented in Part 1 that the Board considered when formulating its conclusions and safety actions. § Part 3, Conclusions: Based on the analyses of the factual information, presents three categories of findings: findings as to causes and contributing factors to the occurrence; findings that expose risks that have the potential to degrade aviation safety, but that could not be shown to have played a direct role in the occurrence; and "other" findings that have the potential to enhance safety, or clarify issues of unresolved ambiguity or controversy. § Part 4, Safety Action: Based on the findings of the investigation, recommends safety actions required to be taken to eliminate or mitigate safety deficiencies, and records the main actions already taken or being taken by the stakeholders involved. Note: Owing to the scope of the Swissair 111 investigation, various supporting technical information (STI) materials are referenced throughout the report. STI materials are peripheral to the report and are not required to develop a complete understanding of the facts, analyses, conclusions, or recommended safety actions. Rather, the STI materials expand, in technical detail, on the information provided in the report. A superscript "STIx-yyy" is inserted into the report wherever such a reference exists. In the hard-copy version, the number "x" identifies the part of the report, and "yyy" identifies the reference within the part, as indicated in Appendix E – List of Supporting Technical Information References. In the electronic version of the report, such references are hyperlinked directly to the applicable location in the electronic version of the STI. Appendix E is not included in the electronic version.     CANADIAN MARITIMES 2016 19 June - 17 August 2016 SMART Canadian Maritimes Caravan 2016 19 June - 17 August 2016 Wagon Masters: Carl and Gwen Hopper Assistant Wagon Masters: Mark and Linda Avey The 2016 Canadian Maritimes Caravan started and ended in Hermon, Maine, and covered over 3,000 miles in the Maritime Provinces of New Brunswick, Nova Scotia, Newfoundland, and Prince Edward Island. We started the trip with 19 rigs but unfortunately lost one in Monc- ton, New Brunswick ,due to an accident. No one was seriously injured, but we had to continue on with only 18 rigs. Some of the highlights of this trip included the Bay of Fundy with 25-foot tides, the Royal Nova Scotia International Tattoo, rides on the Bluenose II and Amoeba sailing vessels, whale watching tours, and some of the most beautiful and breathtaking scenery in the world. Some of our group even took a day trip to Labrador, while others sailed out of St. Anthony, Newfoundland, to view icebergs and whales. We enjoyed many caravan-sponsored dinners with lots of lobster and other seafood. This was an amazing trip which was made even more enjoyable by the outstanding people who traveled with us. Many thanks to all who contributed time and effort to make this a truly memorable trip. Introduction t 22:31 on 2 September 1998, Swissair Flight 111 from New York JFK to AGeneva crashed into the sea, five miles from the shores of St. Margaret’s Bay – near Peggy’s Cove, Nova Scotia, within the territorial waters of Canada. The cause of the crash was an electrical fire. The plane carried 229 passengers and crew from 16 countries, all of which were fatalities, as well as precious cargo. Rescue and recovery operations were started immediately during the night by local fishermen followed by the Canadian Coast Guard and the Navy. Over a period of two years more than two millions pieces of wreckage and body parts were recovered – brought to a Hangar at Shearwater for sorting, storage and processing. Lastly, a giant ship the Queen of the Netherlands was chartered to suck up a part of the ocean floor. Recovery tanks were used to sort out the silt. Aside from passengers and crew, Swissair flight 111 – which was dubbed “The UN Shuttle” – carried cargo including artworks, gold, diamonds and jewellery. Several years later the insurers applied for a treasure trove license because of missing valuables (diamonds). This operation came to the attention of the media and of the families. The treasure trove license was denied at that time by the Provincial Government of Nova Scotia1. 2. Photo: Kathy Willens / AP TWA Flight 800 families mark 15 years since crash NEW YORK — Families and friends of some of the 230 people killed when a Paris-bound jumbo jet mysteriously exploded in the sky off the Long Island coast are planning to mark the early evening moment with a quiet beach gathering Sunday. The destruction 15 years ago of TWA Flight 800 off east Moriches just 12 minutes after takeoff from Kennedy Inter- national Airport set off a mammoth FBI investigation amid fears that a bomb or a missile had downed the plane and introduced sky-based international terrorism to U.S. shores. The unity of family members after the July 17, 1996, crash — and the impact of an organization formed by one of them for those who lost loved ones in disastrous circumstances — are an enduring legacy of the crash. A 16-month federal inquiry relied on Navy divers to pull bodies and pieces of the plane off the ocean floor and rebuild a 96-foot section of the Boeing 747. For weeks, the crash drew heavy international interest in news confer- ences where details of the tragedy were spoon-fed to a worried public. Investigators eventually determined explosives were not to blame — and that a flaming object which people on land said flared across the sky and met the plane just before it exploded was not a missile.

    11 Working with the airlines in the Australasia-Pacific region With my tenure at the new Unisys on shaky ground, I asked former colleagues if they knew of a job opportunity for someone with my skill set. I certainly wanted to stay in the Asia-Pacific area. New Zealand, as a relatively small country, did not present any immediate opportunities other than working with a computer consultancy or setting up on my own, and I was not keen on the latter. Richard Hawkins had left Unisys in Hong Kong and joined the Société Internationale de Télécommunications Aéronautiques (SITA). He had already spoken to SITA on my behalf and advised me to call Ms Irene Legay in SITA’s Hong Kong office. The SITA opportunity was attractive. I knew the organisation and had overseen its Sperry installations in Bangkok and Jakarta. I knew SITA to be technically aggressive. It used Sperry second-level communication processors, without mainframes, for its communications network, and I had recently liaised with SITA when the New Zealand Defence Department had contemplated a similar application. After the end of World War II, international telephone lines had been hard to obtain, so the airlines decided it would be to their advantage to pool resources. SITA was set up to control those resources and provide additional services. In the 1980s, as SITA approached its 40th anniversary, director general Claude Lalanne had instructed McKinsey & Company, as consultants, to investigate the strategies that would keep SITA viable for the next 40 years. McKinsey had recommended that SITA set up 223 ALSO INNOVatoRS a sales and marketing operation — a group of 32 people — to optimise its potential.
