Rules: I agree to abide by the following rules: NO CHEATING. If you are caught cheating, i.e., using unauthorized scripts, configurations or other types of cheats you will be evicted from the FlyingTee event and permanently banned from all future FlyingTee E Sports Tournaments. Ask for help when you need it from the people in charge, never assume anything. Don't mess with anyone's equipment or belongings unless they ask you to. If you are caught stealing, you will be evicted. Don't bring illegal items (weapons, drugs, etc.) into our facility or onto our network. SMOKING IS PROHIBITED INSIDE OF OUR FACILITY. Software pirating and/or copying is strictly prohibited! No rough housing or damage to the gaming equipment or facility property. Follow all instructions and guidelines provided by the Tournament Team Leads and/or facility management. We reserve the right to amend the rules at any time without notice.

Administrators/Sponsors assume NO liability for any injury, loss, or damage of any kind arising from or in connection with any person's participation the Event or Tournament, including without limitation, participation in any real life activity, or injury, loss or damage sustained from use of any prize won. If for any reason, the Event or Tournament is not capable of running as planned, including infection by computer virus', bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Administrator's/Sponsors which corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Event or Tournament, Administrator's/Sponsors reserve the right at their sole discretion to cancel, terminate, modify of suspend the Event or Tournament. RELEASE: The individual signing below ("Registrant") does hereby RELEASE AND WAIVE any and all causes of action, claims, losses, and damages, whether such causes of action, claims, losses and damages are known or unknown, whether such causes of action, claims, losses and damages existing in law or in equity, and whether such causes of action, claims, losses and damages exist under contract, tort, strict liability or other theory, owned by Registrant or which may be owned by Registrant as against Sponsors or any Sponsor or any Sponsor's directors, officers, employees, agents and representatives, arising, out of, relating to or in connection with the Event and/or Tournament (collectively, the "CLAIMS"). Registrant FOREVER DISCHARGES Administrators/Sponsors and their respective directors, officers, employees, agents and representatives from all Claims.

INDEMNITY: Registrant does hereby agree to indemnify, defend and hold harmless each Administrator/Sponsor and each Sponsor's directors, officers, employees, agents and representatives from and against losses, lawsuits, causes of action, claims and damages which arise from the acts or omissions of Registrant in connection with the Event or Tournament.

We will eject anyone that doesn't follow the rules. If you are ejected for breaking the rules, then no money will be refunded, you will not pass go, and your tournament scores will be stricken from the record. SUPEPRare nStaMl WaAivSerH Fo rmBROS Underage Player Parental Waiver For your child to participate in the actives of the FLYINGTEE SUPER SMASH BROS E SPORTS TOURNAMENT, which includes console video game play, we require your consent for your child to participate. Every participant's/player's parent or legal guardian must read, sign and submit this waiver, IN PERSON, to Tournament organizers prior to the commencement of the tournament.

I am the parent or legal guardian of ______("registrant/participant/player") who is under 18 years of age. I hereby consent to this registrant's attendance and participation without restriction in the FLYINGTEE SUPER SMASH BROS E SPORTS TOURNAMENT. Activities consist primarily of console video game play and video game tournament in which there is an entry fee and prizes may be awarded to certain winners. I understand that the video games may have a rating of "M" for Mature assigned by the Entertainment Software Rating Board (ERSB). I am aware that this rating indicates the ESRB has determined that the content of the console video game may be unsuitable for children under 18 years old because of violence, blood, gore and language. I acknowledge that the registrant may choose to participate in non-video game tournament activities available at the facility.

In consideration of the right and opportunity to participate in the FLYINGTEE SUPER SMASH BROS E SPORTS TOURNAMENT conducted by FlyingTee and GameStop, I hereby acknowledge and agree as follows: consent to the use of registrant's name, picture, and/or likeness in publicity and advertising for the FLYINGTEE SUPER SMASH BROS E SPORTS TOURNAMENT in any and all media and/or promotion. In addition, I consent to no claims - past, present, future - against hosts of event for injuries or loss/damage of personal property during the event.
