Annex 1 4 August 2021 Details of Compulsory Testing Notice A. Places visited by a local preliminary positive case which is suspected to be re-positive 1. Any person who had been present at the construction sites for contract number RWCN 21003802 located at 248 Queen's Road East, Wan Chai, Hong Kong for more than one hour at any time during the period from July 12 to August 3, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 2. Any person who had been present at the construction sites for contract number RWCN 21003380 located at Shiu Kin Lane, Wan Chai, Hong Kong for more than one hour at any time during the period from July 12 to August 3, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 3. Any person who had been present at Ngan Lung Restaurant, Shop 112B-C, 1/F, Kwai Fong Shopping Centre, Kwai Fong Estate, 177 Hing Fong Road, Kwai Chung, New Territories, Hong Kong for more than one hour at any time during the period from 7:00 a.m. to 12:00 noon on July 12, July 16 to 17, July 19 to 20, July 23 to 24, July 26, July 29 to 31 or August 2 to 3, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 4. Any person who had been present at Kwai Yan House of Kwai Fong Estate, 177 Hing Fong Road, Kwai Chung, New Territories, Hong Kong for more than one hour at any time during the period from 7:00 a.m. to 9:00 a.m. on July 12, July 16 to 17, July 19 to 20, July 23 to 24, July 26, July 29 to 31 or August 2 to 3, 2021 (including but not limited to residents, visitors and workers), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the reQuirements set out in the compulsory testing notice. 5. Any person who had been present at 醉上海, G/F, 43 Shek Kip Mei Street, Sham Shui Po, Kowloon, Hong Kong for more than one hour at any time during the period from 7:00 p.m. to 12:00 midnight on July 29, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 6. Any person who had been present at Kwai Fong Playershop Game Centre, Shop D2, G/F, Kwai Fong Terrace, 15 Kwai Yi Road, Kwai Chung, New Territories, Hong Kong for more than one hour at any time during the period from 3:00 p.m. to 8:00 p.m. on July 25, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the reQuirements set out in the compulsory testing notice. 7. Any person who had been present at Tsui Yuen Dessert, G/F, Pao Woo Mansion, 177-179 Wan Chai Road, Wan Chai, Hong Kong for more than one hour at any time during the period from 3:00 p.m. to 8:00 p.m. on July 31, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 8. Any person who had been present at KC. CITY, Shops E, F and I, G/F, Kwai Fong Terrace, 15 Kwai Yi Road, Kwai Chung, New Territories, Hong Kong for more than one hour at any time during the period from 7:00 p.m. to 12:00 midnight on July 31, 2021 (including but not limited to full-time, part- time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the reQuirements set out in the compulsory testing notice. 9. Any person who had been present at Kevin Chan CLM HK Healthcare Centre Ltd., Unit 4104, 41/F, Metroplaza Tower 2, 223 Hing Fong Road, Kwai Chung, New Territories, Hong Kong for more than one hour at any time during the period from 6:00 p.m. to 8:00 p.m. on July 20, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the reQuirements set out in the compulsory testing notice. 10. Any person who had been present at 小雨天, Shop C33A-C37, 2/F, Kwai Chung Plaza, 7 Kwai Foo Road, Kwai Chung, New Territories, Hong Kong for more than one hour at any time during the period from 7:00 p.m. to 12:00 midnight on July 20, 2021 (including but not limited to full-time, part- time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the reQuirements set out in the compulsory testing notice. 11. Any person who had been present at Fairwood, Shops 9-10, G/F, Block 42 of Shek Kip Mei Estate, Shek Kip Mei, Kowloon, Hong Kong for more than one hour at any time during the period from 7:00 p.m. to 12:00 midnight on July 19, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 12. Any person who had been present at Dragon Palace Restaurant, G/F, Block 2, Kwai Fong Terrace, 15 Kwan Yi Road, Kwai Chung, New Territories, Hong Kong for more than one hour at any time during the period from 10:00 a.m. to 3:00 p.m. on July 18, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 13. Any person who had been present at Delicious Café, G/F, 243 Yu Chau Street, Sham Shui Po, Kowloon, Hong Kong for more than one hour at any time during the period from 3:00 p.m. to 8:00 p.m. on July 15, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 14. Any person who had been present at Fairwood, Shop 01 (Restaurant B), 2/F, Wu Chung House, 213 Queen’s Road East, Wan Chai, Hong Kong for more than one hour at any time during the period from 11:00 a.m. to 4:00 p.m. on July 15, 2021 (including but not limited to full-time, part-time and relief staff and visitors), has to undergo testing by August 6, 2021. If persons subject to compulsory testing have previously undergone testing between August 2 and August 4, 2021, they would be taken to have complied with the requirements set out in the compulsory testing notice. 15. Any person who had been present at Cheung Chau Columbarium (Phase 4), Cheung Chau Peak Road West, Sai Wan, Cheung Chau, New Territories, Hong Kong for more than one hour at any time during the period from 8:00 a.m.
