Background: Opicapone is a catechol O-methyltransferase (COMT) inhibitor indicated for use as adjunct to levodopa therapy in patients with Parkinson’s disease (PD) and motor fluctuations. Objective: To characterize the safety and tolerability of adjunct opicapone (25 and 50 mg) in a pooled population of levodopa- treated PD patients who participated in the opicapone Phase-3 clinical program. Methods: Patient-level data (placebo, opicapone 25 mg and 50 mg) from the BIPARK-1 and BIPARK-2 double-blind and open-label studies were combined. Results: Pooled analyses included 766 patients from the double-blind studies and 848 patients from the open-label studies. In the double-blind studies, 63.3% of opicapone-treated patients reported treatment-emergent adverse events (TEAEs) versus 57.2% in the placebo group. The most common TEAEs reported in the opicapone group compared to placebo were dyskinesia, constipation and insomnia. The incidence of serious TEAEs was similar across opicapone and placebo groups (3.5% versus 4.3%, respectively). Overall, 71.3% patients treated with open-label opicapone reported at least one TEAE; most occurred within the first 2 months of the open-label studies, and then decreased thereafter. Throughout the Phase-3 clinical program, there were no serious AEs suggestive of hepatic toxicity, and the incidence of gastrointestinal disorders such as nausea and diarrhea remained low (<2%). There were no relevant changes in laboratory parameters including liver enzymes, vital signs, physical or neurological examinations, or ECG readings. Conclusions: Long-term use of opicapone once-daily over 1-year at doses of 25 mg or 50 mg was generally safe and well tolerated, supporting its clinical usefulness in the management of PD motor fluctuations. Thirty seven years later, what was initially envisioned as a minor category of drugs has become a major part of the drug development pipeline. The Orphan Drug Act was passed by the United States Congress in 1983 in order to spur drug development for rare conditions with high unmet need. The legislation provided financial incentives to manufacturers if they could demonstrate that the target population for their drug consisted of fewer than 200,000 persons in the United States, or that there was no reasonable expectation that commercial sales would be sufficient to recoup the developmental costs associated with the drug. These “Orphan Drug” approvals have become increasingly common over the last two decades. In 2000, two of the 27 (7%) new drugs approved by the FDA had Orphan Designation, whereas in 2019, 20 of the 48 new drugs (42%) approved by the FDA had Orphan Designation. Since the passage of the Orphan Drug Act, 37 years ago, additional regulations and FDA designations have been implemented in an attempt to further expedite drug development for certain serious and life threatening conditions. Drugs with a Fast Track designation can use Phase 2 clinical trials to support FDA approval. Drugs with Breakthrough Therapy designation can use alternative clinical trial designs instead of the traditional randomized, double-blind, placebo-controlled trial. PD is caused by progressive loss of dopamine producing neurons in the substantia nigra located in the midbrain. The cardinal motor features of PD are bradykinesia, tremor, rigidity, and postural instability. As PD progresses, it causes increasing motor disability. It should be taken once-daily at bedtime, at least one hour before or after levodopa combinations. In certain patients, this may enable a simplified drug regimen which may potentially improve compliance. Some patients taking a complicated PD drug regimen may find it easier to add a single tablet such as opicapone and keep their familiar levodopa doses over the day. In contrast, entacapone has to be taken with each levodopa dose, which may be up to 10 times daily.

Opicapone is a catechol-O-methyl transferase (COMT) inhibitor. It received a European marketing authorisation in June 2016 and was launched in the UK in October 2016. It is licensed for adjuvant therapy to preparations of levodopa/DOPA decarboxylase inhibitor (DDCI) in adults with Parkinson's disease who are experiencing end-of-dose motor fluctuations and cannot be stabilised on those combinations. Overview This evidence summary reviewed 1 randomised placebo- and active-controlled trial in people with Parkinson's disease of at least 3-year duration, who were taking a stable dose of levodopa and experiencing end-of-dose motor fluctuations. Most participants were also taking other arkinsonP 's disease medicines, most commonly a dopamine agonist. There are limited data on the use of opicapone as a first choice adjunct therapy to levodopa. Description Opicapone (Ongentys®) is a catechol-O-methyltransferase (COMT) inhibitor. FDA Approved Indication(s) Ongentys is indicated as adjunctive treatment to levodopa/carbidopa in patients with Parkinson’s disease (PD) experiencing “off” episodes. Policy/Criteria Provider must submit documentation (such as office chart notes, lab results or other clinical information) supporting that member has met all approval criteria. It is the policy of health plans affiliated with Centene Corporation® that Ongentys is medically necessary when the following criteria are met: I. Initial Approval Criteria A. Parkinson’s Disease (must meet all): 1. Diagnosis of PD; 2. Prescribed by or in consultation with an neurologist; 3. Age ≥ 18 years; 4. Member is experiencing “off” time (see Appendix D) on levodopa/carbidopa therapy; 5. Failure of two of the following adjunct drugs prescribed in combination with levodopa/carbidopa, each from different classes, unless contraindicated or clinically significant adverse effects are experienced:* a. MAO-B inhibitor: rasagiline; ® ® b. COMT inhibitor: entacapone (Comtan /Stalevo ), tolcapone; c.
