5. NHS ENGLAND COMMISSIONING INTENTIONS

NHS England commissioning intentions are detailed below. All services are under review and may change in the future.

Local Scheme 4: Gluten Free Food scheme – pharmacies provide gluten free foods to those patients who have a written referral from either a GP or dietician (originally contracted to NHS Cumbria). Under this scheme pharmacies provide advice and support on the range and quantities of gluten free foods available, agreeing a standard monthly order with each customer according to local guidelines. This is currently in operation in 4 of 6 localities (NB. The CCG has plans in hand to roll out this scheme to the remaining two. They currently hold the budget for this)

Local Scheme 6: Medication Administration Record (MAR sheets) – pharmacies aim to support domiciliary care workers by preparing a MAR sheet when a patient’s prescription is presented, and to provide a medicines management service for these patients. This was a time limited pilot in Barrow in Furness. The CCG possibly have future plans to roll out a pilot in Carlisle in 2014 (there is no further detail on this).

Local Scheme 10: Effective Intervention Scheme - pharmacies identify prescription interventions and communicate this information to GP practices. Such interventions are designed to identify cost savings, waste reduction and quality medicine management issues over and above those traditionally highlighted during the dispensing process. This ended on 31 March 2014. The CCG has recommended this should not be re-commissioned.

Out of Hours – pharmacies are directed to open on particular bank holidays as deemed necessary.

The following schemes for the commissioning of IV antibiotics and palliative care drugs is not a clearly defined process at present and NHS England are currently reviewing these services.

Local Scheme 5: Intravenous Antibiotics - pharmacies supply intravenous antibiotics with the contractor stocking a locally agreed range of specialist medicines. Pharmacies also provide information and advice to the user, carer and clinician and may also refer to specialist centres, support groups or other health and social care professionals where appropriate.

Local Scheme 9: Palliative Care - pharmacies supply palliative care medicines, and carry a locally agreed range of specialist medicines. Pharmacies are also required to commit to ensuring service users have prompt access to medication during opening hours. There is also a requirement for pharmacies to provide information and advice to users, carers and clinicians, to make referrals to specialist centres, or other health and social care professionals, as appropriate.

7. OTHER RELEVANT SERVICES

The following pharmaceutical services have been considered as other relevant services:

• North Cumbria University Hospitals NHS Trust provides an inpatient and outpatient pharmacy dispensing service to the CCG population from two sites; West Cumberland hospital, Whitehaven and Cumberland Infirmary, Carlisle. The hospital pharmacy also supplies medicines to the CCG community hospitals and community clinics, Cumbria Health On Call (GP Out of hours service), Cumbria Partnership Foundation Trust and Eden Valley Hospice. This medicine supply service to other providers is due to terminated on 31st January 2015.

• University Hospital of NHS Trust provides an in-patient and out-patient pharmacy dispensing service to the CCG population from two sites; Furness General Hospital, Barrow-in-Furness and Westmoreland General Hospital, Kendal. The hospital pharmacy also supplies medicines to the CCG community beds and community clinics, Cumbria Health On Call (GP out of hours service), Cumbria Partnership Foundation Trust and the NHS Treatment Centre, Kendal. The medicine supply service to other providers is under review (no further details available).

9. PUBLIC SURVEY

Background/Method

As part of the development of the draft Pharmaceutical Needs Assessment (PNA) an early phase public consultation was carried out. The consultation ran from the 1 May to 30 May 2014. The consultation was conducted online using Cumbria County Council’s dedicated website. Awareness was raised by issuing press releases and making posters and leaflets available to pharmacies to inform their users.

140 responses were received. This report only summarises the data and opinions provided by respondents regarding services provided by their pharmacy. No attempt has been made to cross reference respondents opinions/perceptions against actual services commissioned/available.

Due to rounding some percentage totals in tables and charts may not add up to 100.

Results

Q1. How often do you use a pharmacy?

As can be seen from Figure 1, 60% of respondents stated they used a pharmacy at least monthly and 98% at least once in the last year.

Q2. How far is your pharmacy from you?

Figure 2 shows that 73% of respondents live less than 2 miles from their nearest pharmacy.

Q3. How do you normally travel to your pharmacy

The overwhelming majority of respondents indicated they travelled to their pharmacy either by car (52%) or by walking (44%). Only 2% stated they normally used a bus. 87% of those who live one mile or more away from their pharmacy use a car. 72% of those who live less than one mile walk. 23% of those who live less than one mile away use their car.

Q4. Access/travel options to the nearest pharmacy

Figure 4 shows that the significant majority stated there was available car parking nearby (117) with 90 stating there was a nearby bus stop, however, only 37 respondents indicated there was a bus route from their home. Only 22 felt there was a safe cycle route from their home and just over half (74) indicated their pharmacy has disabled access.

Respondents were asked to provide further comments regarding travel and access to their pharmacy. The main issues have been bullet pointed below.

 Car parking issues

 Health centres with car parking could disadvantage town centre pharmacies

 Perceived poor public transport especially in rural areas

 No disability access to a named pharmacy

 Home delivery services ‘removes’ the need to travel

 Claimed no home delivery service in Wigton

Q8. Pharmacy services that are used

Figure 5 shows that most respondents use pharmacies for prescription dispensing (124 respondents) with a high number prescription collection services (75) and minor aliment services (56). More than a third of respondents (46) also use the medicine disposal service. A small number of respondents indicated that they used the more specialist services: Weight management advice, emergency contraception, stop smoking advice, alcohol support services, screening services and needle and syringe exchange.

Q9. Services respondents feel are important

Respondents were asked how important or not important a number of services were to them. Figure 6 shows the numbers of respondents that indicated which of these services were either very important or somewhat important. Mirroring what services people use prescription dispensing with 123 respondents indicating this as being important more than any other service. Late opening times moves higher as the second most important aspect (82 respondents).

Those who stated they use a pharmacy monthly were almost twice as likely to indicate prescription collection services were very important compared to those who use pharmacies less than monthly (36%). The same group are also significantly more likely to indicate medicine disposal as important to some degree, 55% compared to 37% of all respondents.

Respondents were given the opportunity to provide further detail about services available from their pharmacy. There were a number of comments regarding what was considered to be poor prescription dispensing services, with comments relating to long delays in drug availability (72 hours and 7 days quoted) with one respondent stating that they had to drive to Lancaster as no out of ours pharmacy had prescription available. There were also some criticism of queues and waiting times within pharmacies. There was some strong support for some local pharmacy citing excellent service.

There was some commentary regarding tracking and tracing of prescriptions. Suggestion that online technologies should be used, requests that notifications to be sent out when prescriptions are ready and GP’s and pharmacies to improve communications so doctors know what medications are available.

Suggestions that services could be improved by extending the number of pharmacies that provide additional services such as treatment of minor ailments, improving waiting facilities, the availability of card payment facilities, more extended opening times and addressing concerns around privacy/confidentially within the pharmacy.

Some respondents raised concerns about the distance they had to travel to access an out of hours pharmacy.

One comment was received relating to language, from a respondent who’s first language is not English. They raised the issue that not all information is available in different languages.

Some felt that those extended services should be better promoted.

Q12. Factors in deciding which pharmacy to use

Being near to home (117) and quality of service are (107) were most frequently indicated as important factors in deciding which pharmacy to use. Weekend opening times (93), late opening times (81) and being near to GP (80) were also significant factors. Being on a bus route was lowest with 26 respondents stating tis was important.

Those respondents aged under 35 years are more likely to consider early and late opening times as important factors in choosing a pharmacy compared to those 35 and over (61% to 47% and 83% to 73% respectively).

When asked if they used their nearest pharmacy 77% of respondents said yes. However, when given a series of statements about which pharmacy they used, only 45% selected the statement ‘I normally use the pharmacy nearest my home’ (see Figure 8).

The Board discussed the documents and suggested that a guide as to where provision was needed be included.

The distances given to the nearest pharmacy provision in the document was also misleading, as it assumed everyone had a car and could get there in 5 minutes. There were a lot of people who relied on public transport to get to the nearest pharmacy and in some cases this could involve a 2 hour bus journey.

There was a request for a flowchart to be produced on the commissioning arrangements for PNA.

There is considered to be adequate provision therefore no provision needed statement required

Geographical barriers to services have been considered in section 4.5. For the assessment car travel was considered the most reliable way of assessing access due to the changing picture of public transport. The assessment has been amended to ensure the car travel time is reflected as a minimum travel time.

In the appendices, it quotes a customer comment I think saying that there is no delivery service in Wigton.

This is factually inaccurate as a delivery service is in operation twice a week from both pharmacies. This has been in operation now for 18 months.

I note that in the appendices, it states that the pharmacies were asked about delivery and collection services as a ‘non-NHS service’ but no-where in the PNA is there any information about the outcome of those questions apart from this comment. If this is so important (because the question is asked), why is the outcome not published? Also why is the comment included when it is obviously factually incorrect (should have been picked up from the response to the pharmacy question) without a note to that effect.
