Physical Exams

Now is not too early to begin scheduling important physical examination and immunization appointments for 2018-2019!

All students must be up-to-date with physical examination and immunization requirements. All forms must be turned into the School Health Office prior to August 1st. Students will not be able to attend school until all required health information is on file.

Important Health Requirements for 2018-2019

All students entering Early Childhood, Kindergarten, Sixth and Ninth grades are required to have a current physical examination completed on the approved Illinois Department of Public Health Form. A sport physical does not meet the requirement. A current physical is one that was completed within one year of August 2018. The parent portion on the upper back side of the form must be completed and signed. All students must be up-to-date with the State of Illinois immunization requirements. Immunization records can be viewed through the Infinite Campus Parent Portal. Physical examination forms are available in all School Health Offices and on the D300 Health Services website. Please turn in completed forms to the school nurse or you can email forms to [email protected]. Remember to keep a copy for your records.

Nurse Carrie

Dear 5th Grade Students,

As many of you have heard, we have a 6th Grade Choir at Hampshire Middle School! Choir is a fun place where you can be surrounded by your friends and make new ones. Choir happens during the school day and is a real class that gets a real grade! It doesn’t matter if you are a boy or a girl, choir is open to all HMS students and you don’t need any prior experience singing. We have kids that are involved in all sorts of activities in our choir, including sports, band, orchestra, theater, student council, student newspaper, yearbook committee, and the list could go on.

Even if you are already signed up for band or orchestra, you can still be in choir! Our schedule is made so that all students have equal opportunity to get involved in our choirs here. Choir is a class during the school day!

You might be wondering what is in store for the 2018/2019 school year in choir. We will learn a lot of music, perform on at least 3 concerts, go on at least 1 fieldtrip (maybe more), laugh, and so much more!

I really hope that you will join us for a great year of singing in the fall! What have you got to lose? You never know if you will like something until you give it a try! I hope that you will join choir and check it out.

By the way, did you know that kids involved in music generally have higher grades and score better on tests?

I hope to see you in the fall! Musically Yours,

Mrs. Truesdell [email protected] *Email is the best way to contact me over the summer! 847-792-3254

 Cut here and return to your general music or choir teacher  ------

How do you join? It’s simple! Just fill out this form, have it signed by a parent or guardian and turn it in to your general music or choir teacher. You can also turn it into the office at HMS.
