3. Question: In order to appropriately address the County’s needs, could you provide sample assignments from the most recent Preliminary Engineering Services contract and/or the anticipated work orders for this contract?

Response: Appendix I provide the scope of Work. It can be full scale Preliminary Engineering Services or certain tasks normally associated with preliminary engineering services work. . Below are some example assignments which have been issued under the Department of Transportation and Highways' current Preliminary Engineering Services contract(s): Similar assignments can be expected under this contract.

a) Prepare a Phase I study for traffic signal modernization and low-cost safety improvements at an intersection which will utilize federal Highway Safety Improvement Program (HSIP) grant funds for construction.

b) Review roadway segments identified for the County's annual Pavement Preservation and Rehabilitation Program (PPRP) and prepare a recommendation on the scope of improvements (including addressing Americans with Disabilities Act (ADA) compliance). Identify and obtain the associated environmental clearances. Upon concurrence from the Department of Transportation and Highways, prepare 30% design plans and associated technical documents for each location.

c) Perform a structural survey and prepare an Environmental Survey Request for bridge repairs or deck replacement. Obtain all required environmental clearances.

d) Prepare a feasibility study for intersection improvements or a new grade separation. Prepare conceptual designs and compare options with respect to the following impact categories: 1) utility conflicts, 2) drainage, 3) right-of-way needs, 4) environmental impacts, and 5) construction/maintenance costs.

e) Perform topographic survey services to document existing conditions and create CADD base files for a new multi-use path.

f) Create ArcGIS maps and analyze the project attributes using existing datasets provided by the Cook County Department of Transportation and Highways. The final deliverable is a package of GIS shapefiles which digitize the projects as lines or points, applies buffers, creates new attribute data, and appends the performance data to each project.

g) Prepare a drainage analysis with supporting calculations for a specified location within the County.

4. Question: It appears to us that Appendix IV describing insurance requirements differs from the sample contract, can the County clarify which section the Consultant should consider for purposing of offering comment or proposed exceptions.

Response: Correct, the sample contract provides the general terms and conditions. Appendix IV will be the required insurance under this contract.

5. Question: RFP, page 16 - MBE/WBE UTILIZATION PLAN - FORM 1. Is it expected that the proposer will provide a dollar amount participation for each subconsultant at the time of the proposal submission for a Various Contract? Will the County accept a percentage?

6. Question: RFP, page 17 - MBE/WBE LETTER OF INTENT - FORM 2 Is it expected that the proposer will provide a dollar amount participation for each subconsultant at the time of the proposal submission for a Various Contract? Will the County accept a percentage?

Response: See response to Question No. 1.

7. Question: RFP, page 29 - Cook County Office of the Chief Procurement Officer Identification of subcontractor/Supplier/Subconsultant Form. Is it expected that the proposer will provide the total price of subcontract for supplies or services at the time of the proposal submission for a Various Contract? Will the County accept a percentage?

Response: No it is not expected to have a price for the subconsultant. Percentage will be sufficient.
