ABSTRACT AIM: The plexus brachialis is a complex structure with anatomical variations and connections with neighboring tissues. These variations may cause disparity in the motor and sensorial innervations of the upper extremity. The knowledge of anatomy and possible variations are important for performing surgical procedures in the neck, shoulder and axilla. This study was planned to demonstrate the anatomical variations of the infantile brachial plexus. MaterIAL and METHODS: A total of 20 plexus brachialis from 11 fetal cadavers were dissected and examined microscopically. The branching patterns and variations were evaluated. The width of the nerves was assessed at the level of the nerve root, trunk and cord on the basis of all brachial plexuses and they were arranged in terms of thickness. -

It is a rare variation with few reports of its existence and incidence. We present a case of the accessory subscapularis muscle in a male cadaver fixated with a 10% formalin solution. The muscle passed anteriorly to the axillary nerve, thus, predisposing an individual to quadrangular space compression syndrome. -

Hammond, MD,a,b Denver, Colo Background: Unilateral arm swelling caused by subclavian vein obstruction without thrombosis is an uncommon form of venous thoracic outlet syndrome (TOS). In 87 patients with venous TOS, only 21 patients had no thrombosis. We describe the diagnosis and treatment of these patients. Material and Methods: Twenty-one patients with arm swelling, cyanosis, and venograms demonstrating partial subclavian vein obstruction were treated with transaxillary first rib resection and venolysis. Results: Eighteen (86%) of 21 patients had good-to-excellent improvement of symptoms. There were two failures (9%). Conclusions: Unilateral arm swelling without thrombosis, when not caused by lymphatic obstruction, may be due to subclavian vein compression at the costoclavicular ligament because of compression either by that ligament or the subclavius tendon most often because of congenital close proximity of the vein to the ligament. Arm symptoms of neurogenic TOS, pain, and paresthesia often accompany venous TOS while neck pain and headache, other common symptoms of neurogenic TOS, are infrequent. Diagnosis was made by dynamic venography. First rib resection, which included the anterior portion of rib and cartilage plus division of the costoclavicular ligament and subclavius tendon, proved to be effective treatment. Scapular Winging Is a Rare Disorder Often Caused by Neuromuscular Imbalance in the Scapulothoracic Stabilizer Muscles
SCAPULAR WINGING Scapular winging is a rare disorder often caused by neuromuscular imbalance in the scapulothoracic stabilizer muscles. Lesions of the long thoracic nerve and spinal accessory nerves are the most common cause. Patients report diffuse neck, shoulder girdle, and upper back pain, which may be debilitating, associated with abduction and overhead activities. Accurate diagnosis and detection depend on appreciation on comprehensive physical examination. Although most cases resolve nonsurgically, surgical treatment of scapular winging has been met with success. True incidence is largely unknown because of under diagnosis. Most commonly it is categorized anatomically as medial or lateral shift of the inferior angle of the scapula. Primary winging occurs when muscular weakness disrupts the normal balance of the scapulothoracic complex. Secondary winging occurs when pathology of the shoulder joint pathology. Delay in diagnosis may lead to traction brachial plexopathy, periscapular muscle spasm, frozen shoulder, subacromial impingement, and thoracic outlet syndrome. Anatomy and Biomechanics Scapula is rotated 30° anterior on the chest wall; 20° forward in the sagittal plane; the inferior angle is tilted 3° upward. It serves as the attachment site for 17 muscles. The trapezius muscle accomplishes elevation of the scapula in the cranio-caudal axis and upward rotation. The serratus anterior and pectoralis major and minor muscles produce anterior and lateral motion, described as scapular protraction. Normal Scapulothoracic abduction: As the limb is elevated, the effect is an upward and lateral rotation of the inferior pole of scapula. It is a very rare syndrome resulting from anatomical abnormalities of the thoracic outlet, causing thrombosis of the deep veins draining the upper extremity. This disease is also called “effort thrombosis― because of increased association with vigorous and repetitive upper extremity activities. Symptoms include severe upper extremity pain and swelling after strenuous activity. A 31-year-old female with a history of vascular thoracic outlet syndrome, two prior thrombectomies, and right first rib resection presented with symptoms of loss of blood sensation, dull pain in the area, and sharp pain when coughing/sneezing. When the patient had her first blood clot, physical exam was notable for swelling, venous distension, and skin discoloration. The patient had her first thrombectomy in her right upper extremity a couple weeks after the first clot was discovered. Thrombolysis with TPA was initiated, and percutaneous mechanical thrombectomy with angioplasty of the axillary and subclavian veins was performed. Almost immediately after the thrombectomy, the patient had a rethrombosis which was confirmed by ultrasound. -

Alteration in the normal static or dynamic position or motion of the scapula during coupled scapulohumeral movements. Other names given to this catch-all phrase include: “floating scapula” and “lateral scapular slide”.1, 2 1 Alterations in scapular position and motion occur in 68 – 100% of patients with shoulder injuries. Scapular Dyskinesis Classification System 1, 3 Pattern Definitions Inferior angle At rest, the inferior medial scapular border may be prominent dorsally. During arm motion, the inferior (type I) angle tilts dorsally and the acromion tilts ventrally over the top of the thorax. The axis of the rotation is in the horizontal plane. Medial border At rest, the entire medial border may be prominent dorsally. During arm motion, the medial scapular (type II) border tilts dorsally off the thorax. The axis of the rotation is vertical in the frontal plane. Superior border At rest, the superior border of the scapula may be elevated and the scapula can also be anteriorly (type III) displaced. During arm motion, a shoulder shrug initiates movement without significant winging of the scapula occurring. The axis of this motion occurs in the sagittal plane. Symmetric At rest, the position of both scapula are relatively symmetrical, taking into account that the dominant scapulohumeral arm may be slightly lower. During arm motion, the scapulae rotate symmetrically upward such that the (type IV) inferior angles translate laterally away from the midline and the scapular medial border remains flush against the thoracic wall. The reverse occurs during lowering of the arm. -

Since then many attempts have been made to revise and extend the official terminology as several inconsistencies have been noted. Moreover, numerous crucial terms were either omitted or deliberately excluded from the official terminology, like sulcus popliteus and diaphragma urogenitale, respec- tively. Furthermore, several synonyms are to be discarded. Notwithstanding the criticism, the use of the current version of terminology is strongly recommended. Although the Terminologia Anatomica is open to future expansion and revision, every change should be made after a thorough discussion of the historical context and scientific legitimacy of a given term. The anatomical nomenclature must be as simple as possible but also precise and coherent. It is generally accepted that hasty innovation ought not to be endorsed.
