During this time much has been acco m plis h ed to im prove the qua li ty of hi gh er s p eci a list medi cal tra ining in the Uni ted Kin gd o m. The new speci a list regis trar tra ining pro gra mm es were first in tro d uced in two " va n g uard speci a l ti es" - General Sur gery and Di ag n o s tic Radi o l o gy - on 1 D ece mb er 19 95. B uil ding on the success of th ese speci a l ti es, a ro lling pro gra mme to in tro d uce the grade in all oth er speci a l ti es began on 1 April 19 96, w hi ch was kn own as th e G rade Com mi s sioning Date. This tra n s i tion pro cess was co m pl e ted on 31 March 19 97. This new edi tion of A Guide to Sp ecia list Regi s t rar Training re fl ects exp eri e n ce g a in ed from op era ting the speci a list regis trar grade over the last two years and has als o b een inform ed by a wide co n s ul ta tion exercise whi ch was co n d ucted last year. It covers all aspects of the speci a list regis trar grade from entry to exi t , and appli es to th e whole of the Uni ted Kin gdom as we ll as to all speci a l ti es. Cl earl y, this is still an evo lv i ng pro ces s, and alth o u gh this gui da n ce is very co m pre h e n s ive, it is likely th a t furth er modifi ca tion will be neces s ary over tim e.     MEDICAL PRACTITIONERS 2 Hospital Staff The people who work in any type of workplace, including hospitals, are called the staff. The medical staff in a British hospital belong to one of four main groups: • A pre-registration house officer (PRHO), or house officer, is a newly graduated doctor in the first year of postgraduate training. After a year, he or she becomes a registered medical practitioner. In the current system of training, the Foundation Programme, the name for these junior doctors is Foundation Year 1 doctor (FYI). • A senior house officer (SHO) is in the second year of postgraduate training. The title is now Foundation Year 2 doctor (FY2), but the old terms senior house officer and SHO are still used. • A specialist registrar (SpR) is a doctor who has completed the Foundation Programme, and is training in one of the medical specialties. There are also some non-training registrars -- doctors who have completed their training but do not wish to specialize yet. • A consultant is a fully qualified specialist. There may also be some associate specialists – senior doctors who do not wish to become consultants. In addition, there is at least one medical (or clinical) director, who is responsible for all of the medical staff. Medical Teams Consultant physicians and surgeons are responsible for a specific number of patients in the hospital. Each consultant has a team of junior doctors to help care for those patients. In many hospitals, there are multidisciplinary teams which consist not only of doctors but also of physiotherapists and other allied health professionals. When patients enter- or are admitted to –hospital, they are usually seen first by one of the junior doctors on the ward where they will receive treatment and care.

    Intelligence Unit Research Specialties, sub-specialties and progression through training the international perspective August 2011 Introduction In the UK, it is a legal requirement that a doctor who wishes to practise as a substantive, fixed term or honorary consultant in the NHS must hold specialist registration. Similarly, in order to practise as a GP, a doctor must hold GP registration. A Certificate of Completion of Training (CCT) confirms that a doctor has completed an approved training programme and is eligible for entry onto the GP or Specialist Register. Between the end of the first foundation year, when doctors are fully registered with the GMC, and the granting of a CCT, there is no recognised intermediate ‘waypoint’ for doctors. There are approximately 20,000 Staff Grade and Associate Specialist (SAS) doctors not in training who are providing care to patients in specialty areas. The skills, knowledge and experience that these doctors have is not formally recognised by the GMC. Due to the lack of regulatory recognition, no credit is given for prior learning. Furthermore, the movement of doctors between specialties, as well as the ability to stop and, at a later date, re-enter a training programme may not always be suitably efficient or effectively supported. One major strand of this research was, therefore, to find out whether other regulators (or equivalent) recognise clinical training and experience which surpasses compulsory medical education but is not necessarily undertaken in the pursuit and eventual attainment of a specialist qualification. The second strand of this research was to assess the specialty and sub-specialty systems in other countries.

    Open Letter to NHS Leaders and the UK Government, May 28th 2020 We, NHS doctors and staff, are writing to urge NHS leaders and the Government to pass bold post- COVID-19 legislation to allow for rapid, nationwide changes to the obesogenic and unsustainable food environment in which we currently live. This environment has added to the UK’s COVID-19 pandemic death toll. As healthcare professionals we are concerned that business will restart as usual after the pandemic. It is not acceptable that the NHS is collapsing under the burden of chronic diseases, the majority of which could be prevented and treated by addressing diet and lifestyle factors. We are willing to take on the task of shifting the NHS focus from a reactive healthcare system to one that promotes health and prevents chronic illness. However, this action needs to be fully embraced by NHS leaders and the Government, who should now place the nation’s long term health at the centre of their policy. The proposed changes include: 1. Further taxation and cessation of subsidies for junk food/fast foods/soft drinks producers as well as industrial animal farming. 2. Ban on junk food advertising and any market manipulations which aim to increase consumption of unhealthy, processed foods or drinks. 3. Subsidies to support the UK population in adopting a predominantly whole food plant based diet for both human and planetary health. 4. Promotion of a plant-based food system through the adoption of Conservation Agriculture (No-Till) systems to lower carbon emissions and reduce the risk of entering a ‘post-antibiotic era’.
