Our commitment comes from James 1:27 which states, “Pure and unspoiled religion in the eyes of God our Father is this; coming to the aid of the orphans and widows when they need it.....” Every family we serve and each life lived is, in some way, special and unique. It is our desire to help you plan a funeral or memorial service which is meaningful to you, one which satisfies your emotional and aesthetic needs; one which genuinely represents the life and values of the deceased. This may involve a “traditional” type service or creative ideas, some of which are listed in our “Additional Service Selections”. For example, it may include displaying some special objects during the visitation, such as hobbies or pictures of the deceased and the family or it may include gathering for a meal following the funeral in the “Leavitt Family Center”. Take time to choose what services and tributes would best fit your needs. It is important that you make decisions that are meaningful to you. It is also important that the funeral costs are acceptable to you. We feel a strong commitment to meet your financial needs and have developed several price options and packages to achieve this. Our commitment to you is summarized in your “Funeral Planning Guide” given to you by your funeral director. We want to know if we do not meet the standards or principles of excellence in our service. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you, such as cemetery or crematory services, flowers and newspaper notices. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected. We will charge for our services in buying these items. A. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF (AND OVERHEAD) $ 1495 Our charge includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining death certificates and other forms and claims, preparation of necessary notices, coordination of service plans with parties involved in the final disposition of the deceased, necessary administrative, accounting, and clerical services. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains. B. EMBALMING $ 595 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. C. PERSONAL CARE & PREPARATION _____________________ $ 395 Includes washing, disinfecting and holding of the deceased for cremation or burial for up to 5 days when there is no embalming. The deceased is bathed and placed in refrigeration until cremation or burial. Additional fees may apply after 5 days. D. CREMATION SERVICE FEES Standard Cremation (included in cremation price packages on Price List) $ 265 The cremated remains will be returned within 7 days after the doctor signs the death certificate and coroner or county sign permits. Priority Cremation (In addition to the above Standard Cremation Fee) _______________ $ 350 The cremated remains will be returned within 24 hours after the doctor signs the death certificate and coroner or county sign permits. FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME $ 2,535 This charge includes: local transfer of deceased to the funeral home, other preparation of deceased, basic services of staff, necessary authorizations, bookkeeping & accounting, use of facilities and staff, local transportation using a hearse (airline or other shipping charges are not included), and embalming. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME $ 3,115 This charge includes: basic services of funeral director and staff, full use of facilities for one-day visitation and funeral ceremony, graveside service, hearse transportation to local cemetery and utility vehicle (family car and transportation to our funeral home are not included) IMMEDIATE BURIALS: Ranges of Prices ($16,880 to $2,630) Our charge for immediate burial includes: local transfer of deceased to funeral
