Every family we serve and each life lived is, in some way, special and unique. It is our desire to help you plan a funeral or memorial service which is meaningful to you, one which satisfies your emotional and aesthetic needs; one which genuinely represents the life and values of the deceased. This may involve a “traditional” type service or creative ideas, some of which are listed in our “Additional Service Selections”. For example, it may include displaying some special objects during the visitation, such as hobbies or pictures of the deceased and the family or it may include gathering for a meal following the funeral in the “Leavitt Family Center”. Take time to choose what services and tributes would best fit your needs.

It is important that you make decisions that are meaningful to you. It is also important that the funeral costs are acceptable to you. We feel a strong commitment to meet your financial needs and have developed several price options and packages to achieve this.

Our commitment to you is summarized in your “Funeral Planning Guide” given to you by your funeral director. We want to know if we do not meet the standards or principles of excellence in our service. We want our service to be the highest level of excellence to you, our family.

GENERAL PRICE LIST Caring For Families & Celebrating Lives

These Prices are Effective as of February 1, 2018 and are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you, such as cemetery or crematory services, flowers and newspaper notices. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected. We will charge for our services in buying these items.

A. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF (AND OVERHEAD) $ 1495 Our charge includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining death certificates and other forms and claims, preparation of necessary notices, coordination of service plans with parties involved in the final disposition of the deceased, necessary administrative, accounting, and clerical services. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains. B. EMBALMING $ 595 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. C. PERSONAL CARE & PREPARATION ______$ 395 Includes washing, disinfecting and holding of the deceased for cremation or burial for up to 5 days when there is no embalming. The deceased is bathed and placed in refrigeration until cremation or burial. Additional fees may apply after 5 days. D. FUNERAL CEREMONY, VISITATION AND GRAVESIDE $ 4,795 This charge includes: local transfer of deceased to the funeral home, other preparation of deceased, basic services of staff, necessary authorizations, bookkeeping & accounting, full use of facilities and staff for funeral ceremony, visitation, utility vehicle, local transportation (airline or other shipping charges are not included), embalming, graveside service, hearse transportation to local cemetery, family sedan, memorial folders, register book, and acknowledgment cards.

FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME $ 2,535 This charge includes: local transfer of deceased to the funeral home, other preparation of deceased, basic services of staff, necessary authorizations, bookkeeping & accounting, use of facilities and staff, local transportation using a hearse (airline or other shipping charges are not included), and embalming. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME $ 3,115 This charge includes: basic services of funeral director and staff, full use of facilities for one-day visitation and funeral ceremony, graveside service, hearse transportation to local cemetery and utility vehicle (family car and transportation to our funeral home are not included) IMMEDIATE BURIALS: Ranges of Prices ($16,880 to $2,630) Our charge for immediate burial includes: local transfer of deceased to funeral home, hearse transportation to cemetery, graveside services, necessary services of staff and authorizations, dressing deceased and placing in container or casket. If unembalmed, viewing must be done within 24 hours and is limited to 6 family members for up to one hour. If embalmed, viewing time can be scheduled at family’s convenience with unlimited number of family and friends attending for up to two hours- additional $200. DIRECT CREMATIONS: Range of Prices ($ 5,675 to $ 2,225) Our charge for direct cremation (without ceremony) includes: local transfer of deceased to the funeral home, care in lieu of embalming, basic service of funeral director and staff, obtaining necessary permits and authorizations, and cremation fee. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering) or pouches of canvas. The containers we provide are constructed of heavy cardboard. FOR THOSE FAMILIES WHO NEED FINANCIAL ASSISTANCE It is our sincere desire to provide a dignified funeral for every single family. We recognize that some families are unable to pay our full traditional service fees. We also realize that some of those families do not qualify for assistance from the Department of Human Services. If you need financial assistance please ask your funeral director about our unique Alternative Funeral Service Plan.

5 SIGNATURE SERVICES Creating and Sharing Memories during and after the funeral ceremony

• Bagpipe Tribute $ 175 The bagpipes will be played as the casket is being carried to the grave and will conclude the service with special music, most often “Amazing Grace”.

• Balloon Release $135 At an appropriate time during the graveside service at the cemetery, balloons will be released during special scripture, signifying the spirit and memories of your loved one. Includes 30 Balloons.

• Chamber Music $200 (two hours) ($75 each additional hr.) The sound of a violin, harp or piano playing in the background creates a soothing atmosphere and touch of class for any event. We will arrange to have a musician play during the visitation or prior to the service.

• Candle Reflection Table $105 A place of tribute, the Candle Reflection Table will provide family and friends with an area of remembrance of their loved one. A burning candle, vase of flowers and a portrait from the family will create this special place for visitation or services.

• Candle Light Service $105 A quiet and reflective service. This service is held at the funeral home or a place of family importance. The lights are dimmed and candles glow to create a peaceful environment as you remember and pay tribute to the life you have lost.

• Dove Release- (weather permitting) $ 175 At an appropriate time during the graveside service at the cemetery, doves will be released during special scripture, signifying the spirit and memories of your loved one.

• Floral Picture Album $75 A photo album created to include a digital picture of each floral arrangement sent by family or friends along with the flower card. This is helpful for sending thank you cards and acknowledging the floral arrangement with a specific description. Includes up to 45 pictures. Each additional picture is $ 1.00.

• Housekeeping Service $95(four hours) ($30 each additional hr.) Time with family and friends is valuable. A professional housekeeper will come and spend four hours cleaning your house for the arrival of family and friends that you may be hosting over the next several days of a funeral. Additional services can be arranged.

• Home Security Service $125 (four hours) ($30 each additional hr.) If you are concerned about leaving your home unattended during the visitation, service or reception; a professional security guard can come keep watch.

• Home Reception Package $ 125 A package created to help host family and friends at your home. We bring to your residence; 50 Styrofoam cups, plates, utensils, napkins, an assortment of coffee, tea, hot chocolate, sugar, creamer and stirrers and 2 eight- foot tables and twenty chairs.

• Leavitt Family Center $ 225 -following a Visitation / Funeral provided by Leavitt $ 295 –Facility Rental without staff for event $ 450 -Facility Rental with staff (required for memorial service or gathering)

A space specifically designed for families and friends to come together for fellowship and food following a service or between visitation hours. Our new facility offers a warming kitchen, seating for dining, restrooms, coffee, condiments and one of our staff members will attend to your needs. Families or churches are welcome to bring food or a catering menu is available.

• Memory Portrait $ 295– 16” x 20” $250 – 11” x 14” or 8’ x 10” Your family’s snapshot can become a Rembrandt Style simulated oil painting. It can be displayed during a memorial service or visitation and then taken home to place in a special area. Can include one or two people in the portrait.

• Pall Bearer Recognition Tribute $ 295 Honor the special role of your pallbearers by giving them a rose boutonniere, pallbearer gloves and limousine for a special way for them to be together in the procession to the cemetery.

• Patriotic Memorial Table $ 205 A respectful way to pay tribute and honor a family member who served in the armed forces. We will set up a memorial table with a US flag in an oak flag case, scan and display a military photo on a memory plaque with a patriotic eagle statue attached. These keepsakes are for the family to take home and put in a special place. We will also light a candle symbolizing the eternal flame in honor of the veteran.

• Vehicle Cleaning $ 75 (per car) We will pick up your vehicle, clean the inside and outside so it will be ready for the day of the service.

• Video Tape Service Ask your funeral director for details A tasteful and respectful video recording of the funeral or memorial service to keep as a keepsake or share with family or friends unable to attend the service. Includes a DVD copy.

• Video Tribute- A Life in Pictures $175 (up to 30 pictures) / $50 (each additional 30 pic.)

This unique Tribute Program format visually blends photographs with music. You can immediately share this keepsake with family and friends during visitation, the funeral services or at any other time family and friends would like to honor, remember and affirm the life of your loved one. Includes a DVD copy of the tribute.

Green Burial Services

The Leavitt Family is the first funeral provider in West Virginia and Southeastern Ohio to have earned accreditation from the Green Burial Council to provide options for natural burial. The Green Burial Council is a not-for-profit organization that encourages the use of burial as a means of facilitating the restoration, acquisition and stewardship of natural areas throughout the United States. A natural burial, sometimes called a green burial, does not contemplate the use of formaldehyde-based embalming, the use of a metal casket or the placement of concrete burial vault, unless cemetery requirements state otherwise. The practices of natural burial do allow families to choose from a wide variety of services and products that align with the standards of the Green Burial Council, such as cotton or wool clothing, bio-degradable caskets and often is considered more “environmentally friendly” than cremation, due to the use of gas and emissions into the air caused by the cremation process.

A. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF (AND OVERHEAD)

B. Minimum Sanitary Care Includes washing, disinfections and holding of the deceased for burial for up to 5 days when there is no embalming. Includes preparation for 1 person identification of the deceased. The deceased is bathed, features set with cotton, , and placed in refrigeration.

C. Transportation of deceased from place of death to the funeral home

D. Transportation of deceased from the funeral home to the burial site and Hearse This contemplates utilizing a vehicle and equipment to transport the deceased, directly fro m refrigeration to the cemetery. It does not contemplate additional staffing to serve as pallbearers for the deceased. If additional staff is required for burial, or to serve as pallbearers, an additional $30 per person will be added.

E. Use of staff and equipment to facilitate a graveside service

• Leavitt Family Center - A space specifically designed for families and friends to come together for fellowship and food following a funeral or between visitation hours. Our facility offers a catering kitchen, warming oven, refrigerator, seating for dining, restrooms, coffee, condiments and one of our staff members will attend to your needs.
