ANEURIN BEVAN DRAFT PNA 2021

BACKGROUND

As you may be aware, the way in which pharmacy applications are granted has changed and the National Health Services (Pharmaceutical Services) (Wales) Regulations 2013, which included the “necessary and expedient” test, has been replaced by the NHS (Pharmaceutical Services) (Wales) Regulations 2020. As of 1st October 2021, Heath Boards in Wales are required to publish a Pharmaceutical Needs Assessment (PNA) and it is the PNA which will be used by the Health Board to determine applications from community pharmacy contractors or dispensing appliance contractors for new, additional or relocated premises, changes to opening hours or the provision of more pharmacy based services, to meet local health priorities. The PNA will be valid for a period of five years unless a significant change is identified during the lifetime of the PNA which would require a subsequent PNA to be published sooner.

As part of the PNA the Health Board will set out current pharmaceutical services provided for its population from both within and outside its area and, assess the need for pharmaceutical services in the future. Pharmaceutical Services include Essential, Advanced and Enhanced Services provided by pharmacy contractors and the Health Board has undertaken an exercise examining the provision of each of these services in every community pharmacy (using the information provided by pharmacies as part of the All Wales Pharmacy Database (AWPD) submission at the end of 2020). In addition, the Health Board has looked at access to these services by examining travel times to pharmacies and opening hours.

Analysis of the provision of pharmaceutical services and access to those services is used to identify any gaps or need in services and so inform the planning and commissioning of additional pharmaceutical services. Where gaps in service provision are identified the Health Board may grant an application to fill the gap.

In order that the final PNA is as robust as possible a wide range of stakeholders, including CPW and individual pharmacy contractors, are consulted on a draft of the PNA. This provides an opportunity for pharmacy contractors to correct any information they feel is incorrect or misleading and to offer to provide a service that has been identified as causing a gap in service provision.

The Draft PNA itself is a very lengthy document (over 500 pages). ABUHB has helpfully provided a summary document (Draft ABUHB Summary Document) which clearly identifies gaps in current and futures services. These are set out below: CURRENT GAPS IN SERVICE PROVISION

Within its summary document Aneurin Bevan UHB has identified the following current needs in relation to the provision of pharmaceutical services, which has been broken down into Neighbourhood Care Networks:

Blaenau Gwent East

• Smoking cessation level 3 - there is a current need for a provider of this service in Blaina. Blaenau Gwent West

• Discharge medicines review - there is a current need for this service in the south of the locality in Cwm. • Emergency hormonal contraception - there is a current need for this service in the south of the locality in Cwm. • Smoking cessation level 3 - there is a current need for this service in the south of the locality in Cwm. Caerphilly North

• No current needs have been identified in this locality Caerphilly South

• Smoking cessation level 3 - there is a current need for this service in the north of the locality in Senghenydd and Llanbradach. The Health Board has not identified any future needs for this service. Caerphilly East

• Emergency hormonal contraception - there is a current need for the service to be provided in Cwmfelinfach. • Smoking cessation level 3 - there is a current need for a provider of this service in Cwmfelinfach. Monmouthshire North

• No current needs have been identified in this locality. Monmouthshire South

• No current needs have been identified in this locality. Newport East

• Smoking cessation level 3 - there is a current need for the provision of this service in Llanmartin and . Newport West

• Emergency hormonal contraception - there is a current need for a pharmacy to provide the service in Marshfield. • Smoking cessation level 3 - there is a current need for this service to be provided in and Marshfield, and for a better spread of pharmacies in Newport itself. • Flu vaccination - there is a current need for a pharmacy to provide this service in Bettws and Marshfield.

Torfaen North

• Smoking cessation level 2 - there is a current need for all the pharmacies to provide this service within the locality. Torfaen South

• No current needs have been identified in this locality.

FUTURE GAPS IN SERVICE PROVISION

During the development of the draft Pharmaceutical Needs Assessment, Aneurin Bevan UHB has taken into account the following known future developments:

• The forecasted population growth and the ageing population,

• Housing developments

• Relocation of GP practices, and

• The Clinical Futures Strategy.

In addition the Health Board has considered the document “Pharmacy; Delivering A Healthier Wales” which sets outs the long-term goals for service transformation to ensure the most health gain from prescribed medicines.

Within their summary document the Health Board has identified the following future needs in relation to the provision of pharmaceutical services, which has been broken down into Neighbourhood Care Networks:

Blaenau Gwent East

• Smoking cessation level 3 (PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at seven locations spread evenly throughout the locality, in areas of greater population density.

• Flu vaccination there is a future need for this service to be provided by all the pharmacies in the locality with effect from the next flu season. Should that not happen then, from 1 September 2022, there will be a future need for this service to be provided at seven locations spread evenly throughout the locality, in areas of greater population density. Blaenau Gwent West

• Smoking cessation level 3 (PGD Varenicline) there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at nine locations spread evenly throughout the locality, in areas of greater population density.

Caerphilly North

• Smoking cessation level 3 (PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 15 locations spread evenly throughout the locality, in areas of greater population density. Caerphilly South

• Smoking cessation level 3 - (PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 14 locations spread evenly throughout the locality, in areas of greater population density. Caerphilly East

• Smoking cessation level 3 (PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 14 locations spread evenly throughout the locality, in areas of greater population density. Monmouthshire North

• Smoking cessation level 3(PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 11 locations spread evenly throughout the locality, in areas of greater population density. Monmouthshire South

• Smoking cessation level 3 (PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at seven locations spread evenly throughout the locality, in areas of greater population density. Newport East

• There is a future need for a pharmacy within the development from 1 April 2023 which, as a minimum, has core opening hours of: • 09.00 to 18.30 Monday to Friday, and • 09.00 to 13.00 on Saturdays.

There is a future need for this pharmacy to provide the following services from the point it is included in the pharmaceutical list:

o All essential services o The medicine use review and discharge medicines review services, and o The enhanced services of emergency hormonal contraception, smoking cessation levels 2 and 3, supervised consumption, the common ailment service, the emergency medicine supply service, flu vaccinations, the waste reduction service and the medicine administration services.

• Smoking cessation level 2 - there is a current need for the provision of this service in Llanmartin and Lliswerry.

• Smoking cessation level 3 (PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 15 locations spread evenly throughout the locality, in areas of greater population density.

Newport West

• Smoking cessation level 3 (PGD Varenicline) - there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 18 locations spread evenly throughout the locality, in areas of greater population density. Torfaen North

• There is a future need for a pharmacy within the Mamhilad Urban Village development, once it is completed, which, as a minimum, has core opening hours of: • 09.00 to 18.30 Monday to Friday, and • 09.00 to 13.00 on Saturdays.

There is a future need for this pharmacy to provide the following services from the point it is included in the pharmaceutical list:

o All essential services, o The medicine use review and discharge medicines review services, and o The enhanced services of emergency hormonal contraception, smoking cessation levels 2 and 3, supervised consumption, the common ailment service, the emergency medicine supply service, flu vaccinations, the waste reduction service and the medicine administration services.

• Smoking cessation level 3 patient group direction There is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 11 locations spread evenly throughout the locality, in areas of greater population density.

Torfaen South

• Smoking cessation level 3 (PGD Varenicline) there is a need for all pharmacies to provide this service by 1 April 2022. Should that not happen then, from 1 April 2022, there will be a future need for this service to be provided at 15 locations spread evenly throughout the locality, in areas of greater population density.

Community Pharmacy Wales will be working with existing contractors to address Advanced and Enhanced service provision in the areas identified above (NB there has been no confirmation that MURs will be reintroduced). The aim is to assist pharmacy contractors in providing the service wherever possible and, where it is not possible to understand the reasons for the failure of the service to be provided in order that we can establish whether or not, that service is actually needed. The final PNA, published on 1st October 2021, will be updated to reflect the updated position in terms of pharmaceutical provision. RESPONDING TO THE CONSULTATION AS A CONTRACTOR

The consultation is now open and will close on 31st July 2021. Consultation Questions are being used to provide feedback or comments on the assessment of pharmaceutical services within Aneurin Bevan University Health Board. These can be accessed via the following links.

ABUHB Draft PNA Survey

You will have noted that ABUHB has identified a future need for two additional pharmacies. If you have a pharmacy which you believe may be adversely affected by the opening of a proposed pharmacy it is suggested that you take the time to submit your views to the Health Board as there will be no opportunity to do so once the PNA is published on 1st October unless there is a significant change in circumstances.

CPW will be responding as an organisation to the consultation. Pharmacy contractors may also wish to respond to the consultation as individuals, but even if you do not it is suggested that you take the time to consider if the document:

• Has a clear and accurate description of the location of all pharmacies, appliance contractors and dispensing doctors.

• Has accurately identified pharmaceutical services known as Advanced and Enhanced Services.

• Has accurately identified the opening hours of pharmacies.

• Has adequately examined the physical accessibility of pharmaceutical services by patients.

• Has adequately examined the planning/build progress of any future housing developments (or any other large scale factory/office, educational, sporting development etc)

• Where a current or future pharmaceutical need is identified in respect of Essential, Advanced or Enhanced services, the reasoning is robust and challenged as necessary.
