Introduction The directory is for service providers and health care providers seeking support for people living with HIV (PLWH)–as well as for PLWH seeking services for themselves. The programs listed here receive Ryan White Part A-funding to provide:

 Be HIV+;  Have a total household income below 435% of the federal poverty level (FPL) (this is the same as the income eligibility guidelines for the New York State AIDS Drug Assistance Program (ADAP) and higher than the income eligibility guidelines for Medicaid in New York State);  Reside in the New York Eligible Metropolitan Areas (NY EMA) which includes the five counties/boroughs of and the adjacent Tri-County region of or Westchester, Rockland, and Putnam counties. New York City Services for People Living with HIV Food & Nutrition Services

Food and Nutrition programs provide food and meals as well as services designed to build clients’ knowledge, skills, and independence around healthy eating and nutrition. Services include home-delivered meals, food pantries, congregate meals, and vouchers to purchase food and nutritional services, like supplements and counselling, that are delivered by a registered dietitian. To be eligible for home-delivered meals, clients must have medical documentation substantiating that they are unable to shop or prepare meals for themselves. The Positive Life Workshop

The Positive Life Workshop (TPLW) is a 20-hour health education intervention for PLWH and led by PLWH. The workshop empowers participants to improve their health through educational presentations, small group work, activities, and social support. TPLW is presented over 7 sessions which cover topics such as engagement in health care, treatment adherence, social health, staying safe, HIV disclosure, and mental health. Those who are dealing with a new diagnosis, struggling with retention or treatment adherence, and those who are interested in learning more about managing HIV may find TPLW especially helpful. Harm Reduction Services

The goal of the Harm Reduction program is to provide accessible harm reduction and substance use services, reduce negative impact of alcohol and drug use, promote access to and maintenance in HIV primary care, and to improve adherence to treatment for HIV. Harm Reduction services are provided by credentialed counselors to address substance use problems through a range of strategies including safer use, managed use, and abstinence while also recognizing and meeting the various psychosocial needs of PLWH. Harm reduction services providers funded by the RWPA program also employ patient navigators to help clients’ access health and supportive services, maintain engagement in services, and support retention in care and adherence to treatment. Care Coordination Services

Care Coordination is a medical case management programs that supports retention in care by offering PLWH home- and field-based patient navigation services, coordinated medical and social services, counseling and coaching in adherence to treatment, daily observed therapy (DOT) for treatment, and health education to build the skills and knowledge needed to maintain stable health. Care Coordination programs are associated with clinics. In order to enroll in a care coordination program, PLWH have to be – or become – patients of the clinic with which the program is associated. Care coordination services are intended for those who have demonstrable challenges to engagement in care, retention in care and adherence to treatment, the newly diagnosed or those with co-occurring conditions who may benefit from help coordinating care. This includes those who have not achieved viral load suppression, have been out of care or new to care, new to antiretroviral treatment, co-infected with Hepatitis C or pregnant. Short-term Rental Assistance Short-Term Rental Assistance programs provide financial assistance on behalf of PLWH in the form of rental payments made directly to landlords, which is used to secure or maintain stable housing. Short-term rental assistance services are only available to persons who are HIV positive, homeless or at-risk of homelessness and who meet the medical and financial eligibility criteria for the HIV/AIDS Services Administration (HASA) program but who are otherwise not eligible due to administrative reasons. Medical Case Management Medical case management programs support retention in care by offering PLWH home- and field-based patient navigation services, coordinated medical and social services, counseling and coaching in adherence to treatment, daily observed therapy (DOT) for treatment, and health education to build the skills and knowledge needed to maintain stable health. In order to enroll in a medical case management program, PLWH have to be assessed for eligibility. Medical case management services are intended for those who have demonstrable challenges to engagement in care, retention in care, and adherence to treatment, the newly diagnosed or those with co-occurring conditions who may benefit from help coordinating care. This includes those who have not achieved viral load suppression, have been out of care or new to care, new to antiretroviral treatment, co-infected with Hepatitis C or pregnant. Mental Health Services Mental health services provide psychiatric treatment and counseling services to those with a diagnosed mental illness.
