Email: [email protected]. Abstract: Synovial sarcoma is a malignant mesenchymal neoplasm that accounts for approximately 10% of all soft tissue sarcomas. These tumors most commonly occur in the extremities of young adults but are not restricted to this site and can arise in virtually any organ system. Synovial sarcomas originating in the mediastinum are exceptionally rare and are often mistaken for other, more common neoplasms in this location, especially since there are no specific imaging characteristics or clinical manifestations. Contrary to synovial sarcomas of the extremities, mediastinal tumors more commonly affect male patients. Histologically, these tumors can be divided into monophasic, biphasic and poorly differentiated variants, further complicating the diagnostic process. Recent advances in immunohistochemical and molecular techniques have provided useful tools to confirm the diagnosis and distinguish these tumors from other mediastinal neoplasms. The treatment of mediastinal synovial sarcomas often requires multimodal therapy, including surgery, chemotherapy and radiation. Despite this, the prognosis for synovial sarcomas in this location appears to be worse than for their analogues in the soft tissue, likely related to the often large size of the lesions and close proximity to critical anatomic structures making complete surgical resection difficult to achieve. This review summarizes the clinicopathological, immunohistochemical and molecular characteristics of these rare neoplasms. Histologically, the tumors can be divided into three different subtypes: monophasic, biphasic and poorly differentiated synovial sarcoma. Cytologic atypia is not a feature of these tumors but the mitotic activity is typically increased, ranging from 2 to more than 20 mitoses per 10 high power fields (HPF) (Figure 3D). Frequent stromal changes include myxoid change or hyalinization (Figure 4A), a hemangiopericytic (staghorn) vascular pattern (Figure 4B), foci of calcification, cystic change (Figure 4C), and areas of necrosis or hemorrhage. BERTALANFFYANDCHOSENLAU (Department of Anatomy, Faculties of Medicine and Dentistry, Unirersity of Manitoba, Winnipeg, Manitoba, Canada) SUMMARY The mitotic rates of transplantable Walker carcinosarcoma 256 and fibrosarcoma 1F16F were investigated in rats by the colchicine method. The mitotic rates of these tumors were apparently not influenced by the time of day. In female rats the estrous cycle did not seem to have appreciable effects on the mitotic rates of fibrosarcoma. During the period of active growth of the tumors a constant increase in the number of cells occurred each day until the onset of necrosis. During the 5th-10th day after transplantation about 60 per cent of the cells divided daily in Walker carcinosarcoma. During the growth period of the fibrosarcoma—i.e., from the 15th to the 32d day after transplantation—about 40 per cent of newly formed cells were daily added to this tumor. These figures imply that the cell population of Walker tumor doubles about every 1.7 days, of the fibrosarcoma every 2.5 days. The mitotic rates of these malig nant tumors exceed those of most normal tissues and are surpassed only by those of the epithelium (crypts) in the small intestine. Tumor growth has been quantitatively esti plicable only to solid tumors. Disadvantages were mated by a variety of methods. Chiappori, MD Background: Uterine carcinosarcoma, a rare gynecological malignancy, often presents at the advanced stage with a poor prognosis because current therapies have not improved rates of survival. Genetic characterization of this tumor may lead to novel, specifically targeted drug targets to provide better treatment options for patients with this malignancy. Methods: We present a case of a woman aged 61 years with uterine carcinosarcoma and retrospectively ana- lyzed 100 study patients with uterine carcinosarcoma. From this group, 9 study patients underwent targeted sequencing of 1,321 genes. Results: All 9 study patients had at least 1 mutation in JAK2, KRAS, PIK3CA, CTNNB1, PTEN, FBXW7, TP53, and ERBB2; of these, TP53 was the most frequently mutated gene (6/9). In addition, ARID1A and KMT2C, which have been described and identified as part of a set of chromatin-remodeling genes, were also found in our analyses. From our 100-person cohort clinical analyses, study patients with stage 1 cancer had a median survival rate of 33 months (95% confidence interval, 19–109) compared with a median survival rate of 6 months (95% confidence interval, 3–12) in those with stage 4 disease. Conclusions: Disease stage alone predicted the rate of clinical survival. Up to 50% in the study group were identified at having early stage disease (stage 1/2), indicating improved rates of overall detection compared with previously reported data. Our mutational analysis findings add to the number of tumors in which these mutations have been found and suggest that chromatin-remodeling dysregulation may play a role in the tu- morigenesis of carcinosarcoma.
