Federal Bureau of Prisons Evaluation and Management of Chronic HCV Infection Clinical Guidance May 2017 WHAT’S NEW IN BOP GUIDANCE REGARDING HCV INFECTION? The major changes included in this May 2017 update to the BOP guidance on chronic HCV infection are based primarily on the April 2017 changes to the American Association for the Study of Liver Diseases (AASLD) guidelines, as follows: • The term resistance-associated substitutions (RASs) is now being used instead of resistance- associated variants (RAVs). • Anti-HBs and anti-HBc, in addition to HBsAg, are recommended for baseline testing of hepatitis B status (see LABORATORY TESTS under BASELINE EVALUATION). • Ledipasvir/sofosbuvir once daily for eight weeks is now an AASLD-recommended regimen for treatment in a subgroup of HCV-infected persons who have genotype 1a or 1b, have an HCV viral load <6 million IU/ml, and are treatment-naïve—but who are not black, are not HIV-coinfected, and do not have cirrhosis. -

Description Dasabuvir/paritaprevir/ritonavir/ombitasvir (Viekira Pak®) is a combination of ombitasvir, a hepatitis C virus (HCV) NS5A inhibitor, paritaprevir, an HCV NS3/4A protease inhibitor, ritonavir, a CYP3A inhibitor and dasabuvir, an HCV non-nucleoside NS5B palm polymerase inhibitor. FDA Approved Indication(s) Viekira Pak is indicated for the treatment of adult patients with chronic HCV: • Genotype 1b without cirrhosis or with compensated cirrhosis • Genotype 1a without cirrhosis or with compensated cirrhosis for use in combination with ribavirin (RBV) Policy/Criteria Provider must submit documentation (such as office chart notes, lab results or other clinical information) supporting that member has met all approval criteria. It is the policy of health plans affiliated with Centene Corporation® that Viekira Pak is medically necessary when the following criteria are met: I. Initial Approval Criteria A. Chronic Hepatitis C Infection (must meet all): 1. Diagnosis of chronic HCV infection as evidenced by detectable serum HCV RNA levels by quantitative assay in the last 6 months; 2. Received: 25 June 2020; Accepted: 20 July 2020; Published: 27 July 2020 Abstract: Severe acute respiratory syndrome (SARS), Middle East respiratory syndrome (MERS), and coronavirus disease 2019 (COVID-19) are emerging zoonotic diseases caused by coronavirus (CoV) infections. The viral RNA-dependent RNA polymerase (RdRp) has been suggested as a valuable target for antiviral therapeutics because the sequence homology of CoV RdRp is highly conserved. We established a cell-based reporter assay for MERS-CoV RdRp activity to test viral polymerase inhibitors. The cell-based reporter system was composed of the bicistronic reporter construct and the MERS-CoV nsp12 plasmid construct. Among the tested nine viral polymerase inhibitors, ribavirin, sofosbuvir, favipiravir, lamivudine, zidovudine, valacyclovir, vidarabine, dasabuvir, and remdesivir, only remdesivir exhibited a dose-dependent inhibition. Meanwhile, the Z-factor and Z0-factor of this assay for screening inhibitors of MERS-CoV RdRp activity were 0.778 and 0.782, respectively. -

Technivie™ (ombitasvir/paritaprevir/ritonavir) and Viekira XR™ (dasabuvir/ombitasvir/paritaprevir/ritonavir) – Product discontinuations • On May 22, 2018, the FDA announced the discontinuation of AbbVie’s Technivie (ombitasvir/paritaprevir/ritonavir) and Viekira XR (dasabuvir/ombitasvir/paritaprevir/ritonavir). — Both product discontinuations are voluntary and due to changes in treatment practices in the current chronic HCV market in the U.S. and are not related to quality, safety or efficacy issues. • Viekira Pak™ (ombitasvir/paritaprevir/ritonavir with dasabuvir) will continue to be available due to reliance by countries outside the U.S. on FDA approval to support post-approval change to the label. However, Viekira Pak will not be promoted. • Viekira XR and Viekira Pak are indicated for the treatment of adult patients with chronic hepatitis C virus (HCV): — Genotype 1b infection without cirrhosis or with compensated cirrhosis — Genotype 1a infection without cirrhosis or with compensated cirrhosis for use in combination with ribavirin. — Viekira XR is a 4-drug fixed-dose combination, extended-release tablet containing dasabuvir 200 mg, ombitasvir 8.33 mg, paritaprevir 50 mg and ritonavir 33.33 mg that is dosed once daily. — Viekira Pak is a 3-drug fixed dose combination tablet containing ombitasvir 12.5 mg, paritaprevir 75 mg and ritonavir 50 mg copackaged with dasabuvir 250 mg tablets. The combination tablet is taken once daily and the dasabuvir tablet is taken twice daily. • Technivie is indicated in combination with ribavirin for the treatment of patients with genotype 4 chronic HCV infection without cirrhosis or with compensated cirrhosis. • Until January 1, 2019, supply of Technivie and Viekira XR will be available for all patients who begin therapy prior to July 1, 2018 to ensure that they are able to complete their course of prescribed therapy. -

DRUG POLICY Hepatitis Virus Drug Therapy BENEFIT APPLICATION Benefit determinations are based on the applicable contract language in effect at the time the services were rendered. Exclusions, limitations or exceptions may apply. Benefits may vary based on contract, and individual member benefits must be verified. Wellmark determines medical necessity only if the benefit exists and no contract exclusions are applicable. This policy may not apply to FEP. Benefits are determined by the Federal Employee Program. DESCRIPTION The intent of the Hepatitis Virus Drug Therapy policy is to ensure clinically suitable, cost-effective therapy for members based on product labeling, clinical guidelines and clinical studies while maintaining optimal therapeutic results. The criteria will require the use of the health plan’s preferred agents for chronic Hepatitis C therapy (Harvoni and Sovaldi) before the use of other non-preferred agents, unless there are clinical circumstances that exclude the use of Harvoni or Sovaldi, or the patient is currently receiving treatment with a non-preferred agent. When a referral is received for a non-preferred agent, the requested agent is paid at the client’s standard specialty copay if the patient has a paid claim for the requested agent in the past 30 days. If the patient does not have a claim for the requested agent in the previous 30 days, the Post Step Therapy Critieria for Approval will be applied. Interferon-free Treatments for Chronic Hepatitis C Genotype 1 Infection 1 Introduction A. The hepatitis C virus (HCV) was discovered in 1989.2 B. Prior to the discovery, it was shown in the mid-1970’s that most post-transfusion cases of hepatitis were not due to either the hepatitis A or B virus, thus giving it its initial name, “non-A non-B hepatitis”.3 C. HCV is a leading cause of chronic hepatitis, cirrhosis, and liver cancer and a primary indication for liver transplantation in the Western world.4 Epidemiology A. As many as 185 million persons are chronically infected with HCV worldwide and approximately 3.4 million to 4.4 million individuals in the United States.5,6 B. Spontaneous clearance of chronic hepatitis C is relatively rare, but can occur.7 C. Approximately 18-34% of acutely infected HCV patients naturally clear the virus, but the majority of infected patients will progress to chronic infection (without treatment), 25% of whom will develop cirrhosis over 25-30 years, and 25% of patients with cirrhosis will develop hepatocellular carcinoma (HCC) and/or decompensated liver disease.5,7 a. -
