All applicants are required a to submit a Scoutreach Application, a current resume that includes professional references, the Boy Scouts of America (2021) Adult Application, and Background Check Authorization form (https://filestore.scouting.org/filestore/pdf/524-501.pdf). The Scoutreach application, resume, references, BSA Adult application and background check authorization form can be dropped off to the office (5600 W Sylvania Ave; Toledo, Ohio 43623) or emailed to [email protected]. I certify that the above statements are true. I have a valid driver’s license, and my own vehicle for traveling to sites. I understand this is a position of youth leadership, and will do my best to set a positive example to the youth I serve.
