Speech Production in Amharic- Speaking Children with Repaired Cleft Palate Speech Production in Amharic- Speaking Children with Repaired Cleft Palate Abebayehu Messele Mekonnen A thesis submitted for the degree of Doctor of Philosophy Department of Human Communication Sciences University of Sheffield March, 2013 Abstract Cleft lip/palate is one of the most frequent birth malformations, affecting the structure and function of the upper lip and/or palate. Studies have shown that a history of cleft palate often affects an individual’s speech production, and similar patterns of atypical speech production have been reported across a variety of different languages (Henningsson and Willadsen, 2011). Currently, however, no such studies have been undertaken on Amharic, the national language of Ethiopia. Amharic has non-pulmonic (ejective) as well as pulmonic consonants, which is one of the ways in which it differs from other languages reported in the cleft literature. The aim of this study was therefore to describe speech production features of Amharic-speaking individuals with repaired cleft palate and compare and contrast them with cleft-related speech characteristics reported in other languages. Speech samples were obtained from 20 Amharic-speaking children aged between 5 and 14, with a repaired cleft palate, and a control group of 5 typically-developing children, aged between 4;0 and 6;0, all resident in Ethiopia. Audio and video recordings were made of the participants’ speech production in a variety of contexts including single word production, sentence repetition and spontaneous speech, using a version of the GOS.SP.ASS (Great Ormond Street Speech Assessment: Sell, Harding and Grunwell, 1999) modified for Amharic. A descriptive research design, which involved a combination of perceptual and acoustic phonetic analysis, was employed. The results showed that in addition to the features of speech production associated with cleft palate which are common across languages, there were also language- specific speech production characteristics related to the phonetic and phonological system of Amharic. The atypical speech production patterns identified here suggest that Amharic-speaking children with cleft palate employed various strategies in order to manage the particular speech production challenges posed by the Amharic i phonological system. In particular, in maintaining segmental contrasts, they exhibited a range of unusual airstream mechanisms. In common with children speaking other languages, the children in this study used a range of ingressive articulations (clicks and implosives) in order to avoid nasal escape of air during segmental articulation. Also, however, for ejective versus pulmonic contrasts, they used various atypical realisations (e.g., a preference for glottal realisations of ejectives) and atypical airstream mechanisms (e.g., realisation of ejectives as pulmonics). ii Dedication This thesis is dedicated to my mother, Adanech Birhanu, an astoundingly strong woman , whose advice to work hard at everything I do, to never be afraid to fail, to try and to persevere through difficult times has remained my most important asset! Severinghaus When, in 1969, John Severinghaus penned that conclusion to his foreword for the first edition of John Nunn’s Applied Respiratory Physiology (Nunn 1969), he probably did not have in mind a potential interaction between surgery, anaesthesia, analgesia and postoperative sleep. It is only since then that we have identified the importance of sleep after surgery and embarked upon research into this aspect of perioperative medicine. Johns and his colleagues (Johns 1974) first suggested and examined the potential role of sleep disruption in the generation of morbidity after major surgery in 1974. It soon became clear that sleep-related upper airway obstruction could even result in death after upper airway surgery (Kravath 1980). By the mid-eighties, sleep had been implicated as a causative factor for profound episodic hypoxaemia in the early postoperative period (Catley 1985). The end of that decade saw the first evidence for a rebound in rapid eye movement (REM) sleep that might be contributing to an increase in episodic sleep- related hypoxaemic events occurring later in the first postoperative week (Knill 1987; Knill 1990). Since then, speculation regarding the role of REM sleep rebound in the generation of late postoperative morbidity and mortality (Rosenberg-Adamsen 1996a) has evolved 1 into dogma (Benumof 2001) without any direct evidence to support this assumption.

Sullivan, M.D., Background: Velopharyngeal insufficiency occurs in 5 to 20 percent of children M.P.H. following repair of a cleft palate. The pharyngeal flap is the traditional secondary Eileen M. Marrinan, M.S., procedure for correcting velopharyngeal insufficiency; however, because of M.P.H. perceived complications, alternative techniques have become popular. The John B. Mulliken, M.D. authors’ purpose was to assess a single surgeon’s long-term experience with a Boston, Mass.; and Syracuse, N.Y. tailored superiorly based pharyngeal flap to correct velopharyngeal insufficiency in nonsyndromic patients with a repaired cleft palate. Methods: The authors reviewed the records of all children who underwent a pharyngeal flap performed by the senior author (J.B.M.) between 1981 and 2008. The authors evaluated age of repair, perceptual speech outcome, need for a secondary operation, and complications. Success was defined as normal or borderline sufficient velopharyngeal function. Failure was defined as borderline insufficiency or severe velopharyngeal insufficiency with recommendation for another procedure. Results: The authors identified 104 nonsyndromic patients who required a pharyngeal flap following cleft palate repair. The mean age at pharyngeal flap surgery was 8.6 Ϯ 4.9 years. Postoperative speech results were available for 79 patients. Operative success with normal or borderline sufficient velopharyngeal function was achieved in 77 patients (97 percent). Obstructive sleep apnea was documented in two patients. Conclusion: The tailored superiorly based pharyngeal flap is highly successful in correcting velopharyngeal insufficiency, with a low risk of complication, in non- syndromic patients with repaired cleft palate.

Abstract: AIM: We evaluated the use of computerized quantification of wheezing and crackles compared to a clinical score in assessing the effect of inhaled albuterol or inhaled epinephrine in infants with RSV bronchiolitis. METHODS: Computerized lung sounds analysis with quantification of wheezing and crackles and a clinical score were used during a double blind, randomized, controlled nebulized treatment pilot study. Infants were randomized to receive a single dose of 1 mgr nebulized l-epinephrine or 2.5 mgr nebulized albuterol. Computerized quantification of wheezing and crackles (PulmoTrack) and a clinical score were performed prior to, 10 minutes post and 30 minutes post treatment. Results were analyzed with Student's t-test for independent samples, Mann-Whitney U test and Wilcoxon test. RESULTS: 15 children received albuterol, 12 received epinephrine. The groups were identical at baseline. Satisfactory lung sounds recording and analysis was achieved in all subjects. There was no significant change in objective quantification of wheezes and crackles or in the total clinical scores either within the groups or between the groups. There was also no difference in oxygen saturation and respiratory distress. CONCLUSION: Computerized lung sound analysis is feasible in young infants with RSV bronchiolitis and provides a non-invasive, quantitative measure of wheezing and crackles in these infants 2.
