    2016 COMMUNITY HEALTH NEEDS ASSESSMENT EXECUTIVE SUMMARY Community Health Needs Assessment, 2016 Update Purpose of the Community Health Needs Assessment This 2016 Community Health Needs Assessment (CHNA) updates the CHNA completed in 2013 to meet the requirements of Section 9007 of the 2010 federal law, The Affordable Care Act (“ACA”). The ACA requires that any tax-exempt, IRS- designated 501(c) (3) hospital complete or update a publicly-available, comprehensive CHNA every three years in order to document the extent to which it understands the unique characteristics and needs of the local communities it serves, and responds to these needs by delivering meaningful and effective community benefit through clinical services and other programming. Required Components A CHNA report has five required components: 1) Definition of community served 2) A prioritized description of the significant health needs of the community 3) Transparency in the process and methods used to conduct the CHNA, including how it took into account input from the community served and prioritized community health needs 4) A description of the resources potentially available to address the identified significant prioritized community health needs 5) An evaluation of the impact of actions taken to address the significant health needs identified in the previous CHNA report (June 2013). A CHNA report is considered complete when it is adopted by a governing body of the facility and made widely available to the public. Community Served NYC Health + Hospitals serve all New Yorkers in every neighborhood in New York City regardless of their ability to pay. Addressing disparity throughout New York City, NYC Health + Hospitals is the safety-net for the uninsured and underserved in New York City.

    For Immediate Release March 1, 2012 Contact: HHC press office, 212.788.3339 Nydia Negron-Rios, Lincoln Hospital, 718.579.4732 Lynn Schulman, Woodhull Hospital, 718.963.8124 HHC Hospitals in Brooklyn and the Bronx Receive Up To $1.8 Million State Grant to Improve Asthma Care Coordination and Health Outcomes for Patients Lincoln and Woodhull Hospitals Will Lead Community Based Coalitions to identify high risk asthma patients and link them to care (NY, NY) -- The New York City Health and Hospitals Corporation (HHC) has been awarded up to $1.8 million in New York State Department of Health funding to help create two community-based coalitions designed to improve the quality of life of individuals with asthma and reduce the burden of the disease among high-risk communities in Brooklyn and the Bronx. The awards were granted to Lincoln Medical and Mental Health Center and Woodhull Medical Center, which are located in counties with high rates of asthma-related hospitalizations and emergency department visits. The funds will be used to identify and monitor high-risk populations for asthma, help increase access to asthma care and services, expand community-based asthma education programs, create asthma-friendly environments, and reduce asthma related hospitalizations and emergency department visits in their respective communities. Both hospitals will each receive up to $900,000 over the next five years to organize regional networks of community stakeholders. The grant recipients will receive initial awards of $180,000 for a 12-month period, and may qualify for additional awards up to the $900,000 maximum. “Asthma continues to be a common disease among New York City's children and adults, and the communities served by Lincoln and Woodhull hospitals are disproportionally affected,” said HHC President Alan D.

Its members are primarily low-income and recent immigrant New Yorkers. Make the Road New York promotes economic justice, equity, and opportunity for all area residents through community and electoral organizing, strategic policy advocacy, leadership development, adult education, and citizen services. It provides free legal services for problems related to housing, employment, debt, disability, and government benefits. Healthy Homes Initiative THE PROBLEM ADDRESSED Bushwick has the highest rate of children hospitalized for asthma in Brooklyn—four times the citywide average. Asthma also is the leading cause for children to miss school. In low-income neighborhoods such as Bushwick, poor housing conditions and poor maintenance practices in rental apartments lead to rodent and insect infestations and the use of pesticides, which can trigger asthma flare-ups. With education, residents can remove some asthma triggers and improve indoor air quality and their living environment. PURPOSE OF THE PROJECT The Healthy Homes Initiative taught families with asthma how to reduce their exposure to indoor triggers such as dust, mold, mildew, rodent infestation, bedbugs, cockroaches, and pesticides.
