NPDES Permit No. MA0101010 Page 1 of 23 AUTHORIZATION TO DISCHARGE UNDER THE NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM In compliance with the provisions of the Federal Clean Water Act, as amended, (33 U.S.C. §§1251 et seq.; the "CWA"), and the Massachusetts Clean Waters Act, as amended, (M.G.L. Chap. 21, §§ 26-53), City of Brockton is authorized to discharge from the facility located at Brockton Advanced Water Reclamation Facility 303 Oak Hill Way Brockton, Massachusetts 02301 to receiving water named Salisbury Plain River in accordance with effluent limitations, monitoring requirements and other conditions set forth herein. The Towns of Abington and Whitman are co-permittees for Parts 1.B. Unauthorized Discharges and 1.C. Operation and Maintenance of the Sewer System, which include conditions regarding the operation and maintenance of the collection systems owned and operated by the Towns. The responsible Town authorities are: Town of Abington Town of Whitman Sewer Department Department of Public Works 350 Summer Street 100 Essex Street, P.O. Box 454 Abington, MA 02351 Whitman, MA 02382 This permit will become effective on the first day of the calendar month immediately following sixty days after signature.* This permit expires at midnight, five (5) years from the last day of the month preceding the effective date. This permit supersedes the permit issued on May 11, 2005. This permit consists of Part I (23 pages including effluent limitations and monitoring requirements); Attachment A (USEPA Region 1 Freshwater Chronic Toxicity Test Procedure and Protocol, March 2013); Attachment B (USEPA Region 1 Freshwater Acute Toxicity Test Procedure and Protocol, February 2011); Attachment C (USEPA Region 1 Reassessment of Technically Based Industrial Discharge Limits); Attachment D (USEPA Region 1 NPDES Permit Requirement for Industrial Pretreatment Annual Report) and Part II (25 pages including NPDES Part II Standard Conditions). -

Executive Summary ES.1 Background ES.1 Background ES.2 Purpose ES.1.1 City of Fall River ES.3 Integrated Planning The City of Fall River (Fall River/City) is located in Bristol County, Approach in southeastern Massachusetts. As shown in Figure ES-1, the City ES.4 Project Issues and is located along the Taunton River and Mount Hope Bay shoreline. Goals Interstate 195 crosses through the City and provides access to Providence, Rhode Island to the west and Cape Cod to the east. ES.5 Problem Similarly, Route 24 provides access to the Boston area in the Identification and north. Several local routes (Routes 6, 79, 81 and 138) also pass Resolution Processes through the city, linking Fall River with its neighboring ES.6 Resolution Concepts communities. Chapter XVII WATER RIGHTS Watuppa Ponds and the Quequechan River The North Watuppa Pond and the South Watuppa Pond are eac h great ponds, situated along the east boundary of the City of Fall River . The North Pond is about four miles long and from three-quarters of a mile to a mile and a quarter wide and is fed by springs beneath it, by surface water from surrounding land and a few insignificant streams. It is connected with the South Watuppa Pond by a narrow passage . There is no per- ceptible current in the North Watuppa Pond, but at its south end there i s a slight current toward the South Watuppa Pond. The South Watuppa Pond is of approximately the same area as the North Watuppa Pond bu t at its north end it is considerably wider. It is fed by the waters from the Sawdy Pond and Davol Pond at its south end, by the waters from Staffor d Pond from the southwest, and the Christopher Borden Brook supplies som e water from the southeast. The Quequechan River is unnavigable and is th e only outlet of both ponds . It flows in a general westerly direction from th e northwest shore of the South Watuppa Pond and passes through the city , emptying into Mount Hope Bay with an average daily flow of twenty-si x million gallons. For a mile and a half the current of the Quequechan River is sluggish , but in the last half mile it falls rapidly for one hundred and twenty-nine feet, down a succession of ledges, into the Bay . -

Massdot Is Committed to Improving the Quality of Stormwater Runoff from Its Highways
MassDOT is committed to improving the quality of stormwater runoff from its highways. Through the “Impaired Waters Program,” MassDOT addresses stormwater runoff from its roadways draining to impaired water bodies as part of compliance with the NPDES Phase II Small MS4 General Permit. Stormwater best management practices (BMPs) are implemented to the maximum extent practicable through two methods: retrofit projects and programmed projects. Retrofit projects are tracked within the MassDOT Impaired Waters Program Database, while programmed projects are tracked through this Water Quality Data Form (WQDF). The goal of the WQDF is to raise awareness of the necessity to implement stormwater BMPs during programmed projects and to capture information about stormwater BMPs that are implemented. There are two WQDFs required as part of each programmed project. One is specific to the 25% design stage, and the other is specific to the 75% design stage. Please download the latest version of the WQDF from the MassDOT website. Please fill out the tab titled “75% Design Form,” and check the box at the bottom of the form to ensure that all questions have been answered adequately. Please submit this form in Excel format only and name your file with the convention WQDF25_projectnumber.xlsm. Submit the form to your MassDOT project manager as part of the project's electronic submittal. An interactive web map is available to aid in filling out the WQDF. It is available at http://mass.gov/massdot/map/wqdf. If MassDOT has requested that the form be revised and resubmitted, resubmit the form using the naming convention WQDF75_projectnumber_rev.xlsm. -
