Online percentage rate tenders in 'B-1' Form are invited by the Secretary of Dist Integreted Health & family welfare Society Zilla Parishad, Osmanabad. for the following work from Contractors registered in appropriate class of the Public Works Department State / Zilla Parishad . Bidding document can be seen and downloaded from the website http://mahatenders.gov.in from 11.00 AM on 19/08/2016 to 12/09/2016 up to 17.00 PM. 3. The bid can be submitted in electronic format on the website http://mahatenders.gov.in from 15.00 PM on 26/08/2016 . The deadline for submission of bid is up to 17.00 PM on Dt. 12/09/2016. 4. Bids must be accompanied with online payment through Net Banking. 5. The Tender Fee & EMD will be paid via online mode and scan copy of receipt is to be attached . 6. Condition for payment of additional security deposit, if offer quoted by the tenderer is lesser than 10% below the cost put to tender as per Maharashtra Govt. PWD GR No.BDG 2016/P.K.2/Em-.2 Dated 12/02/2016

• If the contractor does not deposit this additional security deposit (if applicable) within stipulated

• time then his earnest money deposit will be forfeited and his tender will not be considered for acceptance.

• This additional security deposit shall be extendable upto expiry of valid extensions if any and it shall be refunded along with the final bill, after satisfactory completion of work.

• In addition to above if contractor put tender cost more less than 10% below then Contractor have to submit additional Security deposite for extra below % as per above in Envelop No.2 For (For Ex. below 14%: Contractor has to submit additional SD as mentioned below 1 to 10% Add.SD. is = 1% and for (14%-10%=4%) Add.SD. is = 4% like this total additional SD is = 5%

1.17.13 Certified Balance Sheet under the hand and seal of Chartered Accountant for three years i.e. 2013-14 and 2014-15 , 2015-16 for verification of bid capacity. (Format given on Page No. 91) 1.17.1 5 Minimum Eligibility Criteria for Post Qualification ( Available Bid Capacity) To Quality for a pakage of contract made up of this and other contracts the bidder must demonstrate having experience and resources sufficient to meet the agreegate of the qualifying Criteria for the individual contracts. The applicant must be well established contractor with experience and capability in construction of works similar to the work incorporated in this tender. The applicant must provide evidence of having adequate experience in carrying out work like, building etc. with adequate control aided by upto the testing laboratory. In order to assess the experience and capability of contractor for execution of the work

CONTRACTOR NO OF CORRECTION DISTRICT HEALTH OFFICER 5 under construction a minimum criateria has been specified. These minimum Criteria will be judged from post experience of contractor. The applicant bidder have adequate bid capacity, should have executed the work as stated belowin any one year during a period of the last three years. The other requirements such as that of turnover and of execution of similar work is also stated below. a) Average Annual Turnover : The average annual turn over of the contractor for last three financial years shall not be less than the 75% of Estimated Cost of work put to tender.( i.e 468.87 LakhsX 0.75)= 351.652 Lakhs. As regards one similar works, the work done certificates clearly stating the scope of the work, the date of the work order, stipulated and actual date of completion, levy of penalty, if any, etc, shall invariably be submitted. In case of the private works, the work done certificate under the hand and seal of the owner, Architect cum PMC and structural design engineer can be submitted. Also, he shall submit commencement & completion certificate of Municipal Corporation, Municipal Council or town planning authority etc b) Similar Type of Work :- One similar type of Building work costing of 30% of the amount put to tender for this work. The applicant bidder have completed at least one similer work in last three financial year. (The information should be submitted in the form No. 6 ) (Amount Put to tender 468.87 Lakhs X0.30 = 140.661 lakh sa 141.00 lakhs)

c) Available Bid Capacity :- Bidder who meet the minimum qualification criateria will be qualified only if their available bid capacity is more than the total value of the works for which he has offered his bid. The available bid capacity will be calculated as under. Assessed available Bid Capacity = (A x N x 2) – B A= Maximum Value of civil engineering works executed in last three years (updated to current year level) taking into account the completed as well as works in progress (Computed form no. VI) N = Number of year prescribed for completion of the work for which bids are invited. B = Value of current price level of existing commitment and on going to be completed during the next fifteen months (Period of completion of the works for which bids are invited. Even though the bidders meet the above qualifying criateria they are subject to be disqualified if they have made misleading or false representation in the forms, statements and attachments submitted in favour of the qualification requirement.

Small Needle 2 Nos. (Needle size 30 mm) Big Needles 2 Nos. ( Needle size 60 mm) ( with equal no. of stand by Vibrators) 3) Furmas - 5 Nos 4) Water motor pump 5 H.P.with pipe of required length. - 1 Nos 5) Portable power generator for electricity 10 KV - 1 Nos 6) Truck & Tractor - 1 Nos 7) Centering material 500 Squaremeter - 500 sqm 8) Concrete Mixer – 2 nos. 9) Lift Machine/Hoist – 1 nos . e) Information Regarding Key Technical & administration Personnel. The Contractor should have following personnel having qualification and experience as shown below. Graduates in Civil Engineering or equivalent. .- 01 Nos Diploma Engineer.- 02 Nos Civil Engineer Assistants trained in I.T.I or equivalent.- 02 Nos Other Staff. (Lab Technician, Site Supervisor & Store Keeper etc) Maximum 50% in each category should have minimum experience of 3 years of construction of similar work. Information regarding technical personnel should be given in form no. V attached with the document.On the basis of the information submitted by the contractor required as stated in Sr. No. a to f above post qualification criteria will be decided. All above mentioned documents original copy are to be carried at the time of Pre-Bid meeting.
