Fuller An abstract of a thesis submitted in partial fulfillment of the requirements for the degree of Master of Science in Global Health in the Duke Global Health Institute in the Graduate School of Duke University 2021 Copyright by Alvan-Emeka Kelechi Ukachukwu 2021 Abstract Background/Objectives Africa has only 1% of the global neurosurgery workforce, despite having 14% of the global population and 15% of the global neurosurgical disease burden. Also, neurosurgical training is hampered by paucity of training institutions, dearth of training faculty, and deficiency of optimal training resources. The study appraises the current specialist neurosurgical workforce in Africa, evaluates the major neurosurgery training programs, and projects the 2030 workforce capacity using current growth trends. Methods The study involved systematic and gray literature search, with quantitative analysis of retrospective data on the neurosurgery workforce, qualitative evaluation of the major neurosurgery training programs for their strength, weaknesses, opportunities, and threats, and projection modeling of the workforce capacity up to year 2030. Results 1,974 neurosurgeons serve 1.3 billion people (density 0.15/100,000; ratio 1:678,740), in Africa, with the majority (1,271; 64.39%) in North Africa. There are 106 specialist neurosurgery training institutions in 26 African countries, with North Africa having 52 (49.05%) of the training centers. Training is heterogenous; the major programs being the West African College of Surgeons (WACS) - 24 centers across 7 countries, and iv the College of Surgeons of East, Central and Southern Africa (COSECSA) - 17 centers in 8 countries. At the current linear growth rate of 74.2 neurosurgeons/year or exponential growth rate of 6.81% per annum, Africa will have 2,716 - 3,813 neurosurgeons by 2030, and a deficit of 4,795 - 11,953 neurosurgeons. The continent requires a scale-up of its linear growth rate to 663.4 - 1269.5 neurosurgeons/year, or exponential growth rate to 15.87% - 22.21% per annum to meet its needs. While North African countries will likely meet their 2030 workforce requirements, sub-Saharan African countries will have significant workforce deficits. Conclusion Despite a recent surge in neurosurgery residency training, the current state of Africa’s neurosurgery workforce is dire, and many countries will be unable to meet their workforce requirements by 2030 at current growth trends. He garnered panoply of academic and professional qualifications in the course of his career, matched by an equally prolific array of scholarly publications on diverse subjects. The high points of his career would include the landmark description of the Adeloye-Odeku disease in 1971, helping to institutionalise local Neurosurgical training in Nigeria, helping to set up a Surgery department in the then fledgling Medical School in Malawi, his election as an honorary Fellow of the American College of Surgeons and honorary President of the World Federation of Neurosurgical Societies, as well as his appointment as an Emeritus Professor of Neurosurgery by the University of Ibadan. An altruistic and far-sighted man, he ensured that the first Nigerian Neurosurgeon who died prematurely and other Nigerian/African pioneers of Medicine/Neurosurgery are properly immortalised by a painstaking and selfless documentation of their lives and contributions.

Emmanuel Babatunde Omobowale Department of English, University of Ibadan Abstract In this paper, I examine Twilight: Out of the Night , an anthology of poetry, written by Latunde Odeku, Africa’s first Professor of Neurosurgery, who unknown to many people was also a poet. In the collection, Odeku looks at life from a myriad of perspectives, which graphically show his artistic versatility and creative ingenuity. Acting as Conscience of Society, Odeku uses the poems to highlight the multifarious socio- political problems, which have been the bane of Nigeria’s development since she became an independent country in 1960. The poems also establish a point of convergence between two seemingly disparate disciplines, literature and medicine, as they vividly show the poet’s dependence on medicine for poetic inspiration. Latunde Odeku: A Brief Biographical Account In his foreword to Adeloye Adelola's An African Neurosurgeon , a biography of Professor Latunde Odeku, Oritsejolomi Thomas, former Vice-Chancellor of the University of Ibadan, writes: Professor Latunde Odeku during his relatively short life managed to achieve something of worth in many areas. He belonged to that chosen group of neurologists and neurosurgeons, who perhaps observing the unfathomable wisdom of the Divine Creator through the complexity of the human brain, inevitably arrived at the conclusion that there was more to life than the mere temporal and spiritual. Deeper insight into COVID-19 research at regional, and national levels through bibliometric research has revealed different levels of research evolution, depth, contribution, and collaboration patterns. Such reliable and evidence- based information is important for health research planning and policy making. This study aims at providing some evidence-based insight into Sub-Saharan Africa’s preliminary COVID-19 research by evaluating its research contributions, patterns of collaboration, and funding sources. COVID-19 publication data from all the 41 Sub-Saharan African countries was collected from Scopus for analysis. Results show that Sub-Saharan Africa contributed about two percent to global COVID-19 research. South Africa contributed 50.95% of all the COVID-19 publications from Sub-Saharan Africa while USA (28.48%) and the UK (24.47%), the top two external contributors, collaborated with Sub-Saharan African countries three times more than other countries. Collaborative papers between Sub-Saharan African countries - without contributions from outside the region- made up less than five percent of the sample, whereas over 50% of the papers were written in collaboration with researchers from outside the region.

This plan will guide examinations, organizational effectiveness and sustainability. The plan also builds on the progress and impressive results achieved by COSECSA over the past sixteen years. From its inception, COSECSA has been focused on quality surgical care and practice and is committed to building and strengthening quality surgical care for the neglected patient. Access to quality surgical care is a vital component of universal health coverage and an effective means of realizing many of the United Nations (UN) Sustainable Development Goals (SDGs). COSECSA will continue to increase access to surgical care by training and credentialing the surgical health workforce, setting and maintaining standards, and conducting research which will inform policy. As a “College without Walls” we also need to ensure that we use our resources in the most effective manner possible. COSECSA looks forward to working collaboratively and in partnership with national governments, like-minded institutions and partners who share our vision. In particular I take this opportunity to thank the Royal College of Surgeons in Ireland who have supported the development of this document and with whom we continue to work in collaboration with.
