[1.鍵 入Introduction文字] 1.1 Sham Shui Po District

Sham Shui Po District (SSP District) is one of the 18 districts of the Special Administrative Region (HKSAR). Located in the northwestern part of the Peninsula and bounded by the Lion Rock Country Park to the north, the of the Mass Transit Railway to the east, to the south, and the waterfront area and to the west, SSP District covers an area of about 1 047 hectares. There are 2 hospitals ( and Precious Blood Hospital), 1 publicly-funded university (City University of Hong Kong) and around 60 primary and secondary schools in the district.

SSP District is probably the earliest industrial and commercial centre in Hong Kong. Wholesale and retail businesses of mainly textile and clothing, apparels, piece goods and non-staple food are concentrated in and . The district is also a distribution centre for computer and electronic goods, and its computer shopping centres are famous to all residents and visitors.

[鍵入Census文字] . In the 2016 Population By-census, SSP District has population of 405K, with 15.9% of population being at age 65 or above. The labour force participation rate is 60.8% and the median monthly domestic household income is HK$20,000.

1.2 Sham Shui Po District Healthy and Safe Community

The Sham Shui Po District Council set up the Healthy and Safe Community Steering Committee in 2001 to cultivate health and safety culture, as well as to create a healthy and safe environment for the people in the District.

With the assistance of the District Council and SSP District Office, the Steering Committee held the Sham Shui Po Healthy and Safe Community Inauguration Ceremony on 24 November 2002. During the Ceremony, representatives from various organisations signed up a charter and pledged to promote health and safety culture among their organisations.

In November 2006, the Sham Shui Po Healthy and Safe Community Promotional Station (「健康及安全社區宣傳站」) was set up and Sham Shui Po was designated as a “safe community” by the World Health Organization (WHO) in recognition of its achievements in promoting the concept of healthy and safe community.

The Healthy and Safe Community Steering Committee was renamed as Working Group on Healthy and Safe Community (WG on H&SC) in 2008. In 2011, Sham Shui Po was again recognised as a “safe community” by WHO. Over the years, 4

[鍵入WG文字 ] on H&SC adheres to its objectives and continuously cooperates with government departments, district organisations and hospitals to advocate works related to health and safety through community safety programs. Some of the program highlights are listed in section 2.4.

2. Safe Communities Criteria

Sham Shui Po Healthy and Safe Community follows the 6 criteria of the “Hong Kong Safe Community Scheme” of Occupational Safety and Health Council in developing its management system for community safety and health:

2.1 Establishment of Community Safety Policy

Purpose

 To define the Community Safety Policy for implementation of injury prevention and safety promotional programs.

Scope

 Applicable to the development and the review of the Community Safety Policy.

Responsibility

 Chairman of WG on H&SC, Sham Shui Po District Council

- To review the Community Safety Policy with other members of WG on H&SC. - To establish the community safety objective(s) and target(s) and to monitor the progress with other members of WG on H&SC. - To review the Community Safety Manual with other members of WG on H&SC.

 The Chairman of WG on H&SC reviews the Policy together with other members to ensure that it reflects the actual needs in community. The safety policy statements are presented to the public through the Sham Shui Po District Council website (link: http://www.districtcouncils.gov.hk/ssp/english/ dc/dc_structure_list.php?id=513&committee_type=workgroup), etc.

 Community Safety Objective(s) are discussed by the Chairman together with other members, in order to achieve continual improvement for injury prevention or enhancement of safety attitude of members in the Sham Shui Po Healthy and Safe Community.

 Community Safety Manual is established to state the arrangements for achieving Community Safety Policy and Objective(s), and the implementation of Community Safety Programs.

[鍵入文字] 2.3 Identification of Community Safety Needs

Purpose

 To define the mechanism of WG on H&SC for identifying safety needs in Sham Shui Po Healthy and Safe Community.

Scope

 Applicable to the identification of safety needs of people at different levels in Sham Shui Po Healthy and Safe Community.

Responsibility

 WG on H&SC

- To collect injury case data from different parties such as the , the Transport Department, the Labour Department, Caritas Medical Centre and conduct injury surveillance.

- To analyse the collected data to identify community safety needs.

Content

 The collected data will be reviewed and analysed periodically to identify high-risk groups and environments in the community. The results will help WG on H&SC prioritise the work to help people with needs and organise specific injury prevention or safety promotion programs to tackle the problems concerned.

Based on the collected data, characteristics of the population in Sham Shui Po and concerns of the general public, three areas are identified as

[鍵入文字] foci of the safety community programs. These areas are “Health Promotion”, “Occupational Safety” and “Psychological Support”. The data collected for each area are illustrated below:

 Health Promotion

Maintaining a healthy lifestyle is important for all, especially to the comparatively large elderly population in Sham Shui Po. According to the 2016 Population By-census, the percentages of population aged 45-64 and 65+ were 30.8% and 15.9% respectively. In order to encourage the residents, especially seniors, to monitor their health and exercise regularly, funds were allocated to different organisations to organise health talks on various diseases and provide health checks for the citizens.

According to a recent study released by the Faculty of Medicine of the Chinese University of Hong Kong in July 2017, 4.4 billion people worldwide are infected with Helicobacter pylori. In Hong Kong as well as the nearby countries and regions, such as the Mainland, Japan, Korea and Taiwan, over half of the population is infected.

[鍵入文字] High blood pressure is another health issue that is closely related to many Hong Kong citizens. The School of Public Health of the University of Hong Kong released a study which interviewed 16 039 adults from March 2009 to March 2011. The result showed that “32% of respondents suffered from high blood pressure but only 46% of them were ever diagnosed as hypertensive by a doctor”. The statistics for 2011 from the Centre for Health Protection of the Department of Health also showed that up to 26% of deaths in Hong Kong could be related to hypertension.

The above statistics prove that there is a need to organise activities to raise public awareness on these various health issues.

 Occupational Safety

According to the data from Caritas Medical Centre, industrial accidents constitute 22.55% of all accidents in 2011 and the number of cases ranked 2nd among all the accident types. In this regard, promoting occupational safety to both employers and employees is a must to ensure a safe working environment and lower the chances of occupational injuries and fatalities.

 Psychological Support

An article titled “Geography of suicide in Hong Kong: Spatial patterning, and socioeconomic correlates and inequalities” by academics from Hong Kong and Taiwan provides an analysis of the suicide data across 18 districts and illustrates the findings in a map. It is found that the suicide rates are higher in some of the inner city areas such as Sham Shui Po and Kwun Tong.

[鍵入文字] The population in Sham Shui Po is composed of people from diverse backgrounds. The 2011 Population Census showed that in Sham Shui Po, the number of persons from the Mainland having resided in Hong Kong for less than 7 years was 18 939 (11.1%) and the proportion is the second highest among all districts. Apart from Chinese, the population also consists of Filipino, Indonesian, White, Indian, Nepalese, Pakistani, Thai and Japanese people. According to the Population By-census in 2016, the population of non-Chinese ethnicity was 24 008.

While these newcomers are trying to adapt to living in Hong Kong, they may face many difficulties and pressure. Therefore, it is crucial to provide psychological support and opportunities for them to mingle with their neighbourhood.

[鍵入文字] 2.4 Implementation of Community Safety Programs

Purpose

 To outline the implementation of community safety programs based on the result of the analysis of injury surveillance data.

Scope

 Applicable to all safety programs organised by WG on H&SC in Sham Shui Po Healthy and Safe Community.

Responsibility

 WG on H&SC will develop and implement Injury Prevention / Safety Promotion Programs.

Content

 Community safety programs are established by WG on H&SC based on the identified community safety needs.

 The community safety programs promote different safety messages and organise different types of activities depending on the target age groups. Some programs target at the general public while some target at the elderly and the youth. The programs should achieve one or both of the following aims: - To prevent / reduce the chance of recurrence of injury case(s).

- To increase the community members’ safety awareness.  The progress and effectiveness of the community safety programs are reviewed periodically by WG on H&SC. Organisations responsible for organising the programs are invited to report on their progress at different stages and are required to submit reports to WG on H&SC after the programs are completed. Follow-up actions will be taken if necessary.

 As mentioned in the previous section, WG on H&SC focuses on three areas which are “Health Promotion”, “Occupational Safety”, and “Psychological Support”. These areas covered the identified safety needs such as workplace safety, elderly safety and diseases prevention. Examples of the community safety programs are illustrated below:

2.4.1 Health Promotion

Since 2007, various local organisations have been jointly organising a series of programs to raise public awareness of liver, stomach and intestine diseases. People aged 40-75 are the targets of the programs as they are occupied by their busy work life and may neglect the importance of frequent check-ups.

 Helicobacter pylori test program「幽門螺旋菌測驗計劃」

Around half of the Hong Kong population is infected with Helicobacter pylori. As the symptoms of the infection are not obvious at the initial stage, patients may not realise they are infected and fail to seek medical treatment promptly. The bacteria will cause unease and diseases in stomach, and in some serious cases, the bacteria may lead to stomach cancer. Over 60% of stomach cancer patients are Helicobacter pylori carriers. Therefore, it is important to have regular check-ups and get immediate treatment for the infection.

[鍵入文字] infection, two programs were organised in 2014-2015 and 2017-2018 to raise awareness of the bacteria. Information about the danger of and the treatment for the infection was disseminated to the participants by doctors and through exhibition. In 2014-2015, around 160 residents received 13C Urea Breath Test (碳十三尿素呼氣測試) and 30 of them were diagnosed with Helicobacter pylori. In 2017-2018, 510 residents received 13C Urea Breath Test and 103 of them were diagnosed with Helicobacter pylori.

 Liver fat level test program「肝臟『脂』多少檢測計劃」

Liver is the largest gland in the human body with functions such as detoxifying various metabolites, regulating glycogen storage, producing biochemical substances for digestion and breakdown of fat. A person is deemed to have fatty liver if the weight of liver fat is more than 5% of the liver weight. It can affect eyesight and lead to cardiovascular diseases and liver cancer.

[鍵入文字] In 2013, the above test program was initiated. Doctors and nutritionists provided detailed explanation on the formation and prevention of fatty liver. 376 participants accepted blood tests and knew more about their health condition. Among them, 191 participants had non- invasive liver scan and received necessary medical treatment. The whole program reached over 2 000 residents.

 Promotion of gastrointestinal health – community and test program 2012「腸道健康齊推廣–社區教育及測檢計劃 2012」

A series of events related to gastrointestinal health was organised targeting at people aged between 40 and 75. Apart from seminars, medical tests were offered to the target group for prevention of colorectal cancer. Around 350 residents participated in the activity. The contents of the project were also published in newspapers to raise the public’s awareness of the related issues.

 Treasure your health , measure your blood pressure「齊齊量血壓」

From 2011 to 2018, the above program was organised every year with the aim of disseminating information about hypertension and developing residents’ habits in monitoring blood pressure regularly. Extra funds were also allocated to purchase electronic blood pressure monitors.

There are currently 14 stations for providing blood pressure measuring service. From 2011 to 2018, the blood pressure measuring service was used by residents for over 2 million times. Residents who required treatment were referred to specialists for follow-up. The table below shows that the number of people receiving blood measuring service is on the increase and less people are being diagnosed as hypertensive.

Besides, Chinese medicine practitioners and nurses were invited as speakers for talks. Information about high blood pressure, such as preventive measures, exercise and diet that was suitable for patients, was delivered to the residents.

In order to further propagate the importance of preventing high blood pressure, promotional activities were carried out in areas with high pedestrian flow such as the Hong Kong Industrial Centre, Shun Ning Road Recreation Ground, Hoi Lai Shopping Centre, Wet Market, and the Pak Tin Public Transport Terminus where taxi and

 Living well and safeguard your well-being in SSP - community health promotion scheme「樂安深社區健康推廣計劃」

In order to encourage residents to exercise regularly and reduce the chances of various diseases, the above program was held for seven consecutive years. In the program, different sports classes including laughter yoga, African drums, pilates, tai chi, wing chun and fitness exercise were arranged. Participants gathered to demonstrate different exercises on Sports Fun Day.

[鍵入文字]  Community program to promote reduction of salt and sugar in food「我識煮 你識食 – 社區低糖低鹽起動計劃」

With funding from the Committee on Reduction of Salt and Sugar in Food, the above program was first launched in 2017-2018 to encourage reduction in salt and sugar consumption. Through organising seminars and distributing healthy soup, residents knew more about the importance of healthy diet.

2.4.2 Occupational Safety

 Program to promote occupational safety in restaurants 「你我齊心職安健,食肆安全一定掂」

Under the above program, a visit to Food Angel (惜食堂) was scheduled for participants to understand its environmental concepts and procedures in making lunchboxes with edible surplus food. Kitchen workers were also provided with utensils that complied with the safety standards to prevent accidents in the workplace. Apart from this, leaflets were distributed to workers in the catering industry to promote occupational safety and employment protection as stated in the Employment Ordinance. The leaflets were distributed to employees in around 70 restaurants.

[鍵入文字]  Program to promote occupational safety when working at height 「職業安全你要知 高空工作勿忽視」

Through a carnival, games and leaflets, residents and members of owners’ corporations had a deeper understanding about safety issues in building maintenance works and occupational safety, especially about the precautions to be taken when working at height. The whole project reached 2 300 persons.

2.4.3 Psychological Support

 Sham Shui Po well-being movement「幸福由『深』出發」

Sham Shui Po Well-Being Movement has been co-ordinated by the Sham Shui Po District Social Welfare Office of the Social Welfare Department (SWD) since 2009. In view of the success of the program, SWD continued to jointly organise it with the Sham Shui Po District Council. SWD also gained support from various government departments, social welfare organisations, schools and local organisations to launch the program, encouraging positive thinking and promoting the ideas of “gratitude”, “hope/resilience” and “open-mindness”.

[鍵入文字] In 2015-16, “gratitude” was chosen as the theme of the program and the major activities included the production of a photo album named “Appreciating Well- being of Sham Shui Po”; photography courses for the youth and the elderly; volunteer visits to private aged homes and photo-taking of Sham Shui Po community by the students and people with visually impairment. In 2016-17, “hope” was chosen as the theme of the program and the major activities included “Hope•Smile @ Sham Shui Po” Photography Competition; “Hope Infinity @ Sham Shui Po” Songs Production Competition and “Hope Tree Movement”.

 Ways to reduce pressure「釋放壓力出路」

Three talks were delivered by professionals on ways to prevent and eliminate work pressure. Apart from organising a slogan competition to stimulate participants’ creativity, a carnival related to work pressure was organised. Through different games, participants gained in-depth knowledge on the cause of work pressure and ways to reduce the problems concerned.

[鍵入文字] 2.4.5 Other Community Safety Programs

Apart from the community safety programs initiated by WG on H&SC, other community safety needs were satisfied by programs funded by the Sham Shui Po District Council.

Road Safety

 Program to promote road safety in Sham Shui Po「道路安全倍安心」 /「道路安全倍安『深』」 The program was held in 2012, 2013 and 2015. In 2015, six talks were organised in different elderly centres and eight street promotions were arranged to remind pedestrians to follow traffic rules and pay attention to traffic condition. These activities reached over 10 000 people in the district. Besides, a day camp and a quiz competition on road safety were organised to test the Elderly Road Safety Patrol Team (長者交通安全隊) members’ knowledge of road safety.

嘉年華」

Organised every year from 2012 to 2017, the carnival successfully disseminated messages about road safety to the elderly, children and non- ethnic minorities in the district. In 2017, around 2 000 people attended the carnival and knew more about the theme through booths, games and leaflets. With more residents obeying traffic rules, it is hoped that the number of traffic accidents could be reduced.

Fire Safety In these programs, promotional materials were distributed and seminars were organised for residents and owners’ corporations to remind them of the fire safety precautions.

Youth Education Through providing different activities and workshops such as war games, sports classes and consultation sessions, the programs aimed to develop youngsters’ positive attitude and remind them to stay away from drugs and crimes. Some of the activities were jointly organised by schools, social workers and the Police.

 Physiotherapy Department of Caritas Medical Centre: Hydrotherapy Pool

The hydrotherapy pool is on the 4th floor of Wai Ming block at Caritas Medical Centre and located to the right of the corridor leading to the Physiotherapy Department. With the size of 4 x 6 meter, it may look like a small swimming pool at first glance, but it is actually a very nice size for therapeutic activities. The pool is equipped with underwater cycling, underwater treadmills, a hydraulic massage system, a jet stream massage system and LED lamps, which allows users to receive a good variety of treatments in it. The usage rate is around 1,800 cases per year.

The pool has a sophisticated control and monitoring system which maintains the water temperature at around 32 degree celsius. Warm water can increase the body’s blood circulation, relax the muscles and reduce muscle pain, coupled with hydraulic massage or jet water massage, the treatment is excellent in reducing pain which simultaneously affect several regions of the body. It is also suitable for more specific areas, such as back, neck and shoulder.

The jet stream of the pool trains not only the patient’s balance but also the stability of their core muscles. The buoyancy of water reduces the load on joints thus reducing the patient’s pain and allowing joints to move more easily. The treatment is suitable for patients who have undergone operations for fractures or with joint arthritis. The underwater cycling and underwater treadmills could facilitate gait and walking training.

A major feature of the hydrotherapy pool is the presence of a glass wall, through which physiotherapists are able to observe patient’s gaits and help them make prompt corrections. Another feature is the availability LED lamp lights of different colors, which could stimulate the optic nerve to regulate the nerve hormone, resulting in muscle relaxation and mood improvement, 26

Purpose

 To outline the evaluation process of the organised community safety programs.

Scope

 Applicable to the community safety programs under WG on H&SC

Responsibility

 WG on H&SC evaluates the effectiveness of relevant community safety programs

Content

 The effectiveness of community safety programs will be evaluated by WG on H&SC. The evaluation focuses on the following one or both aspects: - Change of number in relevant injury cases.

- Change of safety attitude of participants of the program.

 Quantitative or qualitative method can be employed for the evaluation e.g. injury surveillance for period since the implementation of program, interview or questionnaire survey with program participants, etc. Responsible organisations are required to submit report on the programs after the end of the programs. They are also required to report the result of their programs in WG on H&SC meetings.

 The evaluation should be done as soon as possible after the completion of the program.

 Based on the evaluation results, WG on H&SC will decide on the appropriate follow-up actions, e.g. continue the program; implement new program for other aspects; or enrich the contents of the program for maximising the effectiveness.

[鍵入文字2.5.1] Examples of Evaluation on Safety Programs

 Treasure your health, measure your blood pressure「齊齊量血壓」is one of the major programs which has been organised for seven consecutive years. WG on H&SC evaluated the effectiveness of the program based on the number of new hypentension cases, feedbacks from participants and organisers. Over the years, it was found that participants had developed a healthy habit to monitor their blood pressure regularly. Participants provided positive feedbacks towards the program, especially for the one-on-one detailed consultation by the nurses and the arrangement of the fitness walking activities which encouraged the elderly to exercise more.

WG on H&SC and the organiser continuously reviewed the program effectiveness, and the locations and target groups were the considering factors when designing the program. The organiser once tried to promote in private housing estates but the response was less active. Therefore, the organiser changed the locations to other areas such as public housing estates and transport terminus.

Taken into account that more new residential buildings are being built with large number of residents moving into the district, WG on H&SC considers it necessary to continue the program for cultivating residents’ habit in monitoring blood pressure regularly and providing consultation service to the citizens. It is also important to ensure that the blood pressure measuring machines are in good condition for providing service to the residents.

 Another program called Living well and Safeguard your well-being in SSP - Community Health Promotion Scheme「樂安『深』社區健康 推廣計劃」 was organised to promote the importance of exercise regularly and to foster positive psychological health in elderly.

[鍵入文字] After joining the program, participants learned to use handy tools and simple movements to stretch and build up their muscle strength. The elderly experienced great fun when exercising with their friends and their muscle pain was greatly reduced. Through performing and sharing their learning, the participants felt confident and accomplished.

 For other programs that continued over the years, the contents had been enriched by expanding the scopes and diversities. For example, various topics had been included for the health promotion programs to deepen residents’ understanding on different diseases. For the programs that promote psychological health, more elements such as the microfilm production competition had been added to enrich the contents and attract more participants.

2.6 Sharing of Safety Community Experience

Purpose

 To outline the participation of WG on H&SC and its members in Safe Communities networks.

Scope

 Applicable to all members of WG on H&SC in participating activities in relation to safe community.

Responsibility

 Members of WG on H&SC participate in different activities to share and learn safety community experience.

 WG on H&SC participates in local and / or international safe community network(s) to obtain latest information for safe community development.

 Healthy and Safe City Newsletter「健康及安全社區特刊」

In order to showcase the achievements of Sham Shui Po Safety Community, WG on HSC published the Healthy and Safe City Newsletter to promote the related activities in the district. Over 14 000 booklets were published in 2017, reaching citizens from various age groups.

Additional information related to health such as depression, Ebola virus, Dengue fever was included in the booklet. The booklet helps raising the safety and health awareness on both personal and community levels.

In 2014, WG on HSC participated in The 6th Global Conference of the Alliance for Healthy Cities. In the Conference, members shared experience in developing Sham Shui Po as a safe community.

Besides, the information collected from representatives of different countries, such as Philippines, Korea and Taiwan, was beneficial for the future community safety activities in Sham Shui Po.

Sham Shui Po Healthy and Safe Community Community Safety Policy

The Working Group on Healthy and Safe Community (WG on H&SC) of the Sham Shui Po District Council is committed to achieve a safe and healthy environment for all citizens in Sham Shui Po District, by the implementation of Community Safety Programs based on the health and safety needs of the Sham Shui Po Healthy and Safe Community.

WG on H&SC’s Safety Policy statements are:  To formulate strategies and targets for development plans on healthy and safe community in the district;  To develop effective partnership network for community promotion activities and assist in promoting related activities;  To monitor and follow up the progress of work and evaluate the effectiveness of the plans; and  To report to the District Council on a regular basis.

WG on H&SC’s objectives are:  To reduce the number of injuries in Sham Shui Po Healthy and Safe Community.  To enhance the safety awareness of residents through safety promotion programs.
