Immediately after assuming the responsibility for SBM(G) implementation, the Rural Development Department orientation the District Magistrates and Deputy Development Commissioners through a one day workshop on the 18 th June 2016. The institutional rearrangements created a positive vibe and much needed momentum in many Districts. Several districts are establishing a pool of CLTS Resource Persons and Masons with support of UNICEF and other development partners. The PRIs and SHGs are better linked with the District ODF drive. JEEViKA’s strong community presence with about 40 Lakh members is an opportune platform in the Di ht a ainst open defecation. 0112i3A has been desi nated as the nodal SBM(G) implementation a ency. 0112i3A, besides stron community base, has a rounded or aniEational structure from State to the Panchayat level. Further, 0112i3ACs familiarity with the participatory development approaches provides scope to adopt the community centered mobiliEation and supply mana ement to implement SBM(G). The technical assistance of the development partners in capacity buildin , social - community mobiliEation, cross learnin , knowled e sharin , etc. will supplement the SBM(G) implementation. One patient from Hariharpur Lalgarh Village of Siwan was Zika Positive at SMS Hospital, Jaipur. Both Dengue & Zika virus is transmitted by same vector Ades Aegypti. The urban areas of Siwan like Puranikila, Siwan Nagar, Dakhintola, Sukultola, saikhtola, kagazi mohalla had reported increased number of fever cases & confirmed Dengue positive in Microbiology lab of PMCH, Patna. Entomological findings confirmed the presence of Aedine vector in the affected area. Finding confirms the factor leading to the present outbreak and highlights risk factor & control strategies to prevent future in the Siwan district. Community support and participation is also crucial for the prevention of future outbreaks and improving the health and well being of population in the Siwan district.     DISTRICT PROFILE GOPALGANJ INTRODUCTION Gopalganj district is one of the thirty-eight administrative districts of Bihar. Gopalganj sub-division was notified as a district on October 2, 1973. Gopalganj district falls in Saran division. Gopalganj is bounded by the districts of Patna, Samastipur, Khagaria, Munger and Lakhisarai. The river flowing through Gopalganj district is Gandak. HISTORICAL BACKGROUND The composite Saran district lies on one of the main lines of the eastwards progression of the Aryans. According to the Vedic traditions, Videhas marched eastwards from the Saraswati and made the eastern banks of river Gandak their home. After crossing river Gandak, the Videhas founded a powerful kingdom on its eastern bank. It is likely that some of them settled in Saran, while the majority of them crossed the river Gandak. In pre- historic times, the present districts of Gopalganj and Siwan up to the bank of river Saryu which bounds Siwan district on its south, was a part of Nepal. The name Siwan means the boundary i.e. the southern boundary of the erstwhile Nepal kingdom. The history of Gopalganj is the history of Saran district. The ancient state of "Mallyas“ served as the frontier of Gopalganj. Saran was part of Kaushal kingdom. During the medieval period, Gopalganj came under the control of the Afghans and Mughals. Under the British, the district of Gopalganj was a Kingdom of Maharaja Fateh Shahi of Hussaipur. Until 1875 A.D., the tiny hamlet of Gopalganj was an Anchal within the erstwhile Saran district. In 1875, Gopalganj Anchal was upgraded to the status of a sub- division of the erstwhile Saran district.

    DISTRICT PROFILE SIWAN INTRODUCTION Siwan district is one of the thirty-eight districts of Bihar. Situated in western Bihar, Siwan was originally a sub-division of Saran district. It was notified as a district on 11th December, 1972. The district of Gopalganj was carved out of Siwan district in 1973. Siwan district is bounded by Saran and Gopalganj districts of Bihar and Deoria and Balia districts of Uttar Pradesh. The rivers flowing through Siwan district are Ghaghara(Gogra or Saryu), Jharahi, Daha, Gandaki, Dhamati (Dhamahi), Siahi, Nikari and Sona. HISTORICAL BACKGROUND Siwan derives its name from "Shiv Man", a Bandh Raja whose heirs ruled this area till the arrival of Babur. Siwan is also known as Aliganj Siwan after the name of Ali Bux, a feudal lords of the area. Maharajganj may have found its name from the seat of the Maharaja there. Historically, Siwan formed a part of Kosala kingdom and Banaras kingdom. The statue of Lord Vishnu excavated in village Bherbania from underneath a tree suggests that people were devotees of Lord Vishnu. The Muslims rule began in Siwan during the 13th century. Sikandar Lodi ruled in Siwan in the 15th century. Babur crossed Ghaghra river near Siswan on his return journey from Bihar. After the battle of Buxar in 1765, Siwan became a part of Bengal. Siwan played an important role in the Sepoy Mutiny of 1857. A sizable number of Sepoys of East India Company drawn from this region- the stalwart and sturdy ‘Bhojpuris’, noted for their martial spirit and physical endurance, rebelled against the Company and joined Babu Kunwar Singh, the Zamindar of Jagdishpur, who lead the mutiny in Bhojpur.

Singh Department of Earth and Environmental Studies, Montclair State University, Montclair, NJ 07043, USA Available online at: www.isca.in, www.isca.me Received 21 st November 2014, revised 25 th January 2015, accepted 17 th February 2015 Abstract This study presents a review of the arsenic contamination of drinking water, soil, and the food chain in the Middle- Gangetic Plain in India’s Bihar state. We identify challenges for arsenic-mitigation and recommend solutions for this problem. Approximately 46% of the geographical regions, 72 of 532 community blocks, are arsenic contaminated. More than 10 million people in rural Bihar are exposed to elevated levels of arsenic through naturally contaminated drinking water. Arsenic levels exceed the United Nations Food and Agricultural Organization’s standards for irrigation water. Arsenic contamination in soil and in the food chain could be potential threats to the area’s inhabitants. Children are exposed to arsenic contaminated soil while playing in their backyards. Children who do not wash their hands thoroughly may unintentionally ingest soil, which over the long term may increase the risk of developing cancer. People with several stages of arsenicosis symptoms and suspected arsenic-induced cancers were found in the state. Recently discovered arsenic contaminated areas in Bihar are far from the River Ganges, which suggests that other River basins, such as the River Ghaghara and Gandak, are potential sources of arsenic.

NRHM recognizes that until better health facilities reaches the last person of the society in the rural India, the social and economic development of the nation is not possible. The District Health Action Plan of Siwan district has been prepared keeping this vision of mind. The DHAP aims at improving the existing physical infrastructures, enabling access to better health services through hospitals equipped with modern medical facilities, and to deliver with the help of dedicated and trained manpower. It focuses on the health care needs and requirements of rural people especially vulnerable groups such as women and children. The DHAP has been prepared keeping in mind the resources available in the district and challenges faced at the grass root level. The plan strives to bring about a synergy among the various components of the rural health sector. In the process the missing links in this comprehensive chain have been identified and the Plan will aid in addressing these concerns. The plan has attempts to bring about a convergence of various existing health programmes and also has tried to anticipate the health needs of the people in the forthcoming years.
