Hello, my name is ______, and I am helping the <> complete a survey of homeless people in our community. Do you consider yourself to be homeless? (you may need to inquire further by asking questions such as “Do you have a place to stay tonight?”, “Are you sleeping outside tonight?”, “Where did you sleep last night?”, etc.)

If NO, discontinue introduction and thank them for their time.

If YES, continue reading the introduction:

We would like to ask you some questions to help us better understand the needs of homeless people? You do not have to be interviewed if you do not want to. If you do participate, you do not have to answer all the questions in the survey. Also, if you agree to participate, we will keep your responses confidential.

If YES, discontinue the survey If NO, continue…

Would you be willing to spend a few minutes to answer our questions?

If NO, discontinue introduction and record the person(s) on the Homeless Count Form.
