ANNUAL REPORT INFORMATION

This information regarding the Annual Compliance Status forms is intended to inform those holding General Construction Storm water Permits of possible questions about the reports that were not answered in the Annual Report letter or in the Important Information sheet that was sent, and/or to provide blank forms for those of you who do not have it, whether it was misplaced, never sent, or any other reason for that matter. All other forms (i.e.: Notice of Termination, Change of Information, etc.) can be found in the Construction Storm water section of our website: www.swrcb.ca.gov).

General Information

Annual Report Submittal date: September 15, 2001 Where to send them: Attn: Alexa La Plante Regional Water Quality Control Board 1515 Clay Street, Suite 1400 Oakland, CA 94612 Annual Report Form

If you need to use the blank form provided at this site, please indicate which, if any, information has changed (i.e.: contact information, site data, etc.) by a star or some uniform way that is easily understood. If there is a partial ownership change, fill out the Change of Information (COI) form and send it along with the report.

If you have a permit, yet have not yet started construction, write a letter explaining the situation, a time frame for construction activities, and the BMP’s you are planning on implementing.

* The Annual Compliance Status Report cannot be used in place of a SWPPP.

Color-coded Map

The color-coded map should be a simple map of the overall site (i.e.: no blue prints) on 8½x11 or 11x17 inch paper that indicates the gradient of the site and directional storm water flows, using arrows. To color-code the map, use some sort of color system (i.e.: highlighters, computer generated coding), indicating the active and completed portions of the site, and the Best Management Practices (BMP’s) that have been implemented on the site. For clarification on what these portions might entail, here are some examples:

Active portions: All disturbed areas, debris piles, construction piles, etc. Completed portions: Completed roads, buildings, landscaped areas, etc. BMP’s: Mulching, vegetated areas, straw waddles, silt fences, location of storm drains, etc.
